Bacteria were isolated from deep terrestrial subsurface sediments underlying the coastal plain of South Carolina. A total of 163 isolates from deep sediments, surface soil, and return drill muds were examined for plasmid DNA content and resistance to the antibiotics penicillin, ampicillin, carbenicillin, streptomycin, kanamycin, and tetracycline. Forty febrile patients during several hematologic diseases (28 acute leukemias, 8 lymphocytic chronic leukemias, 3 drug induced agranulocytosis, 1 myeloid aplasia) received intensive combination antibiotherapy including cefoxitin (with gentamicin in all cases : Carbenicillin was added in 17 cases). Thirty-five patients had severe neutropenia before treatment. Infection's regression was obtained in 35 cases (in spite of persistence of neutropenia in 20 cases). No local, general or biological toxicity was observed.

A case of hypokalemic metabolic alkalosis precipitated by high-dose intravenous ampicillin sodium is discussed. Cases of hypokalemic metabolic alkalosis attributable to ampicillin sodium have not been reported previously. There have been reports of this phenomenon associated with high doses of penicillin sodium and carbenicillin disodium. The possible mechanism of antibiotic-induced hypokalemic metabolic alkalosis is discussed. It is suggested that most cases of antibiotic-induced hypokalemia respond to oral or intravenous potassium chloride.

Pseudomonas aeruginosa strains, 1285 in total, were isolated from hospitalized children in the period 1981-1984. Serotypes 011 and 06 and nontypable strains were the most frequently identified. 44.6% of serotype 011 strains and between 12.4 and 37.7% of other serotypes were resistant to gentamicin, or carbenicillin, or both agents. The aminoglycosides with the highest activity against P. aeruginosa isolates were amikacin and netilmicin. Among the seven beta-lactam antibiotics tested the most active were cefsulodin, piperacillin and azlocillin.

Clinical studies were conducted on 466 patients waiting for senile cataract surgery and receiving chloromycetin, gentamicin, or carbenicillin subconjunctivally and through New Sauflon 70 and New Sauflon 85 lenses. The aqueous drug levels were biologically estimated at various time intervals. Soft contact lenses provided significantly higher drug penetration than subconjunctival therapy. Both modes of treatment provided therapeutically effective levels against most of the common ocular pathogens for varying intervals of 2 to 12 hours.

In recent years, advances in chemotherapy for malignant disease have resulted in a greater population of patients with granulocytopenia whose course in often complicated by fever. We reviewed the current medical literature in an attempt to identify the most appropriate management of this special patient population. Our findings suggest that patients with fever and granulocytopenia and without a diagnosis of infectious disease who improve on empiric antimicrobial therapy should continue to receive empiric therapy until their granulocytes recover.
