We describe the case of a 34-year-old man who presented intermittent faecal incontinence as a manifestation of cataplexy. Zimelidine, a selective inhibitor of serotonin (5-HT) reuptake in the CNS, was administered to narcoleptic patients. This medication has a potent anticataplectic action without improving daytime somnolence. These results suggest that 5-HT neuronal systems are involved in the physiopathology of cataplexy. We studied a patient with cataplexy secondary to a surgical lesion that involved the perichiasmal hypothalamus. We believe that this lesion interfered with the hypothalamic mechanism for timing sleep and wakefulness, whereas the pontine mechanism for generating sleep cycles remained relatively intact.

Cataplexy is a transient loss of muscle tone that can be triggered by emotions such as laughter, excitement or fear. Other causes of cataplexy include Niemann-Pick type C Disease, Angelman Syndrome, Norrie Disease, Prader-Willi Syndrome. We evaluated fluoxetine in six patients with poorly controlled cataplexy by beginning patients on a single 20 mg dose without any change in their current medications. Once benefit occurred, we discontinued or reduced other anticataplectic medication. Reduction in the number of cataplectic episodes per week after 20 weeks of treatment constituted the basis for efficacy determination. Seven narcoleptic patients had serial measurements of blood pressure taken during nocturnal sleep over a period of 2 or 3 successive nights. Blood pressure was measured using a Doppler system with a cuff that automatically inflates every 15 min. There was no difference in the blood pressure measurements throughout the night compared with normal control subjects. Blood pressure followed the patterns of normal nocturnal variation; there was a nonsignificant increase in REM sleep compared with stage 3-4 NREM sleep.
