A reconsideration of the relevance of systemic low-dose ketamine to the pathophysiology of fibromyalgia.

UNLABELLED: Fibromyalgia is a common disorder characterized by chronic widespread pain that affects an estimated 2% of the general population. Recent advances have shed insight on this mysterious disorder, leading to the growing conclusion that disturbances of pain-related processes within the central nervous system, termed central sensitization, represent its most likely source. The phenomenon of central sensitization depends on plasticity in function of N-methyl-D-aspartate (NMDA) subtype glutamate receptors. Earlier studies implicated increased sensitivity of central NMDA receptors as playi

Neuroplasticity and Pain Perception: Exploring the Complexities of Temporomandibular Disorders.

Temporomandibular disorders (TMDs) are prevalent conditions affecting the temporomandibular joint (TMJ), masticatory muscles, and associated structures, leading to pain, restricted movement, and joint noises. These disorders are multifactorial in origin, involving structural, functional, and psychological components. This review delves into the neurophysiological mechanisms of pain perception in TMDs, focusing on peripheral and central processes, including the role of neural plasticity in chronic pain. Peripheral mechanisms involve nociceptors in the TMJ, activated by inflammatory mediators, m

[Neuropathic pain due to malignancy: mechanisms, clinical manifestations and therapy].

INTRODUCTION: Neuropathic pain in cancer patients requires a focused clinical evaluation based on knowledge of common neuropathic pain syndromes. DEFINITION: Neuropathic pain is a non-nociceptive pain or "deafferentation" pain, which suggests abnormal production of impulses by neural tissue that is separated from afferent input. Impulses arise from the peripheral nervous system or central nervous system. CAUSES OF NEUROPATHIC PAIN DUE TO MALIGNANCY: Neuropathic pain is caused directly by cancer-related pathology (compression/infiltration of nerve tissue, combination of compression/infiltratio

Central Sensitization in Vulvodynia and Endometriosis: What Have We Been Overlooking So Far?

IMPORTANCE: Women experience more frequent and greater pain than men, although they receive less adequate treatment and are perceived as more anxious than males. Recent clinical research has lead to hypothesize a common etiology for overlapping chronic pain conditions and mood disorders, namely, central sensitization, which originates from an alteration of pain processing pathways in the central nervous system. OBJECTIVE: The aim of this review was to collect all available evidence regarding the potential role of central sensitization in vulvodynia and endometriosis. EVIDENCE ACQUISITION: A

Chronic Pain in Inflammatory Arthritis: Mechanisms, Metrology, and Emerging Targets-A Focus on the JAK-STAT Pathway.

Chronic pain is nowadays considered not only the mainstay symptom of rheumatic diseases but also "a disease itself." Pain is a multidimensional phenomenon, and in inflammatory arthritis, it derives from multiple mechanisms, involving both synovitis (release of a great number of cytokines) and peripheral and central pain-processing mechanisms (sensitization). In the last years, the JAK-STAT pathway has been recognized as a pivotal component both in the inflammatory process and in pain amplification in the central nervous system. This paper provides a summary on pain in inflammatory arthritis, f

Pharmacologically-mediated reactivation and reconsolidation blockade of the psychostimulant-abuse circuit: a novel treatment strategy.

Psychostimulant abuse continues to present legal, socioeconomic and medical challenges as a primary psychiatric disorder, and represents a significant comorbid factor in major psychiatric and medical illnesses. To date, monotherapeutic drug treatments have not proven effective in promoting long-term abstinence in psychostimulant abusers. In contrast to clinical trials utilizing monotherapies, combinations of dopamine (DA) agonists and selective 5-HT(3), 5HT(2A/2C), or NK(1) antagonists have shown robust efficacy in reversing behavioral and neurobiological alterations in animal models of psycho

Encoding of the cough reflex in anesthetized guinea pigs.

We have previously described the physiological and morphological properties of the cough receptors and their sites of termination in the airways and centrally in the nucleus tractus solitarius (nTS). In the present study, we have addressed the hypothesis that the primary central synapses of the cough receptors subserve an essential role in the encoding of cough. We found that cough requires sustained, high-frequency (≥8-Hz) afferent nerve activation. We also found evidence for processes that both facilitate (summation, sensitization) and inhibit the initiation of cough. Sensitization of cough

Use of antiepileptic mood stabilizers in dermatology.

Several antiepileptic drugs (AEDs) are approved by the US Food and Drug Administration for the treatment of bipolar disorder (valproic acid, divalproex, lamotrigine, carbamazepine) and some cutaneous neuropathic pain syndromes (carbamazepine, gabapentin, pregabalin). The AEDs may be effective in the management of (1) chronic pruritus, including pruritus due systemic disease, including uremia, neuropathic pain, neuropathic pruritus, and complex cutaneous sensory syndromes, especially where central nervous system (CNS) sensitization plays a role; (2) management of emotional dysregulation and the

CSI and Musculoskeletal Pain: Clinical Validity and Neurophysiological Implications.

BACKGROUND AND AIM: Chronic pain affects about 20% of adults worldwide, severely impacting quality of life and functioning. Many chronic pain syndromes share symptoms such as fatigue, poor sleep, anxiety, and depression, suggesting a common mechanism. Central sensitization, an abnormal amplification of pain by the nervous system, is considered a key underlying factor. Assessment tools like the Central Sensitization Inventory (CSI) and laser-evoked potentials (LEPs) may improve diagnosis and guide targeted treatment strategies. This review evaluates the validity of the CSI for assessing central

Sex-Specific Mechanisms of Central Sensitization: A Narrative Review of the Neuroimmune Architecture of Chronic Pain.

Chronic pain disproportionately affects women, yet analgesic development has been built predominantly on male biological substrates, reflecting a now-refuted assumption of biological symmetry that excluded female subjects from preclinical investigation. Central sensitization, the pathological amplification of central nervous system signalling that produces pain hypersensitivity, is increasingly recognized as a sex-dependent neuroimmune phenomenon with distinct cellular and molecular signatures in males and females. This narrative review, prepared in accordance with the Scale for the Assessment

Potential Mechanisms Underlying Centralized Pain and Emerging Therapeutic Interventions.

Centralized pain syndromes are associated with changes within the central nervous system that amplify peripheral input and/or generate the perception of pain in the absence of a noxious stimulus. Examples of idiopathic functional disorders that are often categorized as centralized pain syndromes include fibromyalgia, chronic pelvic pain syndromes, migraine, and temporomandibular disorder. Patients often suffer from widespread pain, associated with more than one specific syndrome, and report fatigue, mood and sleep disturbances, and poor quality of life. The high degree of symptom comorbidity a

[Advances in innate immune-mediated neuroinflammatory mechanisms of migraine].

Migraine is a highly disabling neurological disorder in which activation of the trigeminovascular system and the ensuing peripheral and central sensitization are major pathological foundations for pain initiation, recurrent attacks, and disease chronification. In recent years, innate immune-mediated sterile neuroinflammation has emerged as a key mechanism linking trigeminal neurovascular system activation to maladaptive remodeling of nociceptive pathways. During the early phase of a migraine attack, trigeminal nerve endings release calcitonin gene-related peptide, pituitary adenylate cyclase-a

The Microbiota-Gut-Brain Axis in Disorders of Gut-Brain Interaction: Mechanisms and Emerging Therapeutic Strategies.

Disorders of gut-brain interaction (DGBIs), traditionally termed functional gastrointestinal disorders, affect approximately 15% of adults worldwide and are characterized by chronic digestive symptoms without identifiable structural abnormalities. According to the recently updated Rome V framework, DGBIs are characterized by pathogenic dysregulation of bidirectional communication among the central nervous system, enteric nervous system, and intestinal microbiota. This review examines the anatomical and molecular foundations of gut-brain axis (GBA) communication, emphasizing the roles of microb

Neutrophils infiltrate sensory ganglia and mediate chronic widespread pain in fibromyalgia.

Fibromyalgia is a debilitating widespread chronic pain syndrome that occurs in 2 to 4% of the population. The prevailing view that fibromyalgia results from central nervous system dysfunction has recently been challenged with data showing changes in peripheral nervous system activity. Using a mouse model of chronic widespread pain through hyperalgesic priming of muscle, we show that neutrophils invade sensory ganglia and confer mechanical hypersensitivity on recipient mice, while adoptive transfer of immunoglobulin, serum, lymphocytes, or monocytes has no effect on pain behavior. Neutrophil de

Prevention of autoimmune demyelination in non-human primates by a cAMP-specific phosphodiesterase inhibitor.

Experimental allergic encephalomyelitis (EAE) is an autoimmune disease of the central nervous system that serves as a model for the human disease multiple sclerosis. We evaluated rolipram, a type IV phosphodiesterase inhibitor, for its efficacy in preventing EAE in the common marmoset Callithrix jacchus. In a blinded experimental design, clinical signs of EAE developed within 17 days of immunization with human white matter in two placebo-treated animals but in none of three monkeys that received rolipram (10 mg/kg s.c. every other day) beginning 1 week after immunization. In controls, signs of

The brain-gut axis and chronic pain: mechanisms and therapeutic opportunities.

The brain-gut axis (BGA) is emerging as a critical mediator in chronic pain, involving bidirectional communication between the central nervous system and the gastrointestinal system. The "Pain Matrix" is associated with microbial dysbiosis, vagus nerve dysfunction, and hypothalamic-pituitary-adrenal (HPA) axis dysregulation, driving neuroinflammation and central sensitization. Key mechanisms include microbial diversity loss, leaky gut, and altered neuroactive signaling via short-chain fatty acids (SCFAs) and vagal pathways. This narrative review explores the intricate interplay between BGA mec

Neurologic complications of respiratory disease.

The respiratory and central nervous systems are intimately connected through strict control of ventilation by central mechanisms. The exquisite sensitivity of central chemoreceptors and cerebral blood vessels to changes in central nervous system oxygenation mandate this type of control to maintain proper brain function. When diseases of the lung and respiratory system interfere with this fine balance, neurologic symptoms, sometimes severe, may develop. This article deals with the effects of abnormal ventilation on the nervous system.

Pain is Modulated Differently Between Females With and Without Patellofemoral Pain: Factors Related to Sensitization.

CONTEXT: Patellofemoral pain (PFP) has poor long-term recovery outcomes. Central sensitization describes central nervous system changes altering pain modulation, which can complicate recovery (poorer prognosis and worse function). Signs of central sensitization include amplified pain facilitation, pain hypersensitivity, and impaired pain inhibition, which can be measured with temporal summation of pain (TSP), pressure pain thresholds (PPTs), and conditioned pain modulation (CPM), respectively. Sex differences exist for these test responses, but female-only PFP investigations of sensitization a

Central sensitization as the mechanism underlying pain in joint hypermobility syndrome/Ehlers-Danlos syndrome, hypermobility type.

BACKGROUND: Patients with joint hypermobility syndrome/Ehlers-Danlos syndrome, hypermobility type (JHS/EDS-HT) commonly suffer from pain. How this hereditary connective tissue disorder causes pain remains unclear although previous studies suggested it shares similar mechanisms with neuropathic pain and fibromyalgia. METHODS: In this prospective study seeking information on the mechanisms underlying pain in patients with JHS/EDS-HT, we enrolled 27 consecutive patients with this connective tissue disorder. Patients underwent a detailed clinical examination, including the neuropathic pain questi
