Dizziness and spatial disorientation in patients with transient global amnesia: clinical and vestibular correlates.

OBJECTIVE: To determine the prevalence and clinical significance of dizziness and spatial disorientation in patients with transient global amnesia (TGA), and to examine their relationships with vestibular function and imaging findings. METHODS: We retrospective analyzed 64 TGA episodes from 63 patients [46 (71.9%) women, mean age ± SD = 58.6 ± 9.4]. Vestibular and ocular motor function test [including vestibular-evoked myogenic potentials (VEMPs)], neuropsychological assessments, and brain imaging were evaluated. Logistic regression was used to identify factors associated with dizziness and s

Demographics, presentation and symptoms of patients with Klippel-Feil syndrome: analysis of a global patient-reported registry.

INTRODUCTION: Klippel-Feil syndrome (KFS) occurs due to failure of vertebral segmentation during development. Minimal research has been done to understand the prevalence of associated symptoms. Here, we report one of the largest collections of KFS patient data. METHODS: Data were obtained from the CoRDS registry. Participants with cervical fusions were categorized into Type I, II, or III based on the Samartzis criteria. Symptoms and comorbidities were assessed against type and location of fusion. RESULTS: Seventy-five patients (60F/14M/1 unknown) were identified and classified as: Type I, n

Pathogenesis, Diagnosis, and Treatment of Cervical Vertigo.

Cervical vertigo is characterized by vertigo from the cervical spine. However, whether cervical vertigo is an independent entity still remains controversial. In this narrative review, we outline the basic science and clinical evidence for cervical vertigo according to the current literature. So far, there are 4 different hypotheses explaining the vertigo of a cervical origin, including proprioceptive cervical vertigo, Barré-Lieou syndrome, rotational vertebral artery vertigo, and migraine-associated cervicogenic vertigo. Proprioceptive cervical vertigo and rotational vertebral artery vertigo h

Nonsurgical spinal decompression system traction combined with electroacupuncture in the treatment of multi-segmental cervical disc herniation: A case report.

RATIONALE: With the spread of computers and mobile phones, cervical spondylosis has become a common occupational disease in clinics, which seriously affects the quality of life of patients. We used a nonsurgical spinal decompression system (SDS) combined with physical therapy electroacupuncture (EA) to treat a case of mixed cervical spondylosis caused by multi-level cervical disc herniation, and we achieved satisfactory results. PATIENT CONCERNS: A 44-year-old Caucasian Asian woman presented with neck pain and numbness on the left side of the limb. MRI showed the patient's C3-C7 segment cervi

Lateral ventriculo-peritoneal shunt treating delayed cerebrospinal fluid leak after cervical laminoplasty: Case report.

RATIONALE: Delayed cerebrospinal fluid (CSF) leak is a rare and terrible complication after cervical surgery, potentially causing morbidity or even threatening life. PATIENT CONCERNS: A 55-year-old Chinese male, with a history of hypertension for 8 years, complained of numbness and weakness of both upper limbs for 10 years. Strength of right upper limb triceps and wrist extensor muscle was slight weakness. Bilateral Hoffman sign was positive. Cervical magnetic resonance imaging (MRI) indicated multilevel cervical spondylotic myelopathy. We performed cervical laminoplasty for that patient on A

Cervical Posterior Spinal Artery Syndrome: A Case Report and Literature Review.

We report a case of left upper cervical posterior spinal artery (PSA) syndrome caused by atherosclerosis of the left vertebral artery. A 70-year-old female experienced sudden dizziness and paralysis of the left upper and lower limbs. Diffusion-weighted magnetic resonance imaging (DWI) of the brain showed high signal intensity at the vermis and lower left hemisphere of the cerebellum, and magnetic resonance angiography showed that the entire left vertebral artery was thin. The patient was treated with an intravenous infusion of tissue plasminogen activator 2 hours after symptom onset and made a

Anxiety- and depression-related somatic symptoms and atlantoaxial disorders: potential mechanistic links and clinical implications.

BACKGROUND: Anxiety and depressive disorders often include pain, dizziness, headache, fatigue, nausea, and sleep disturbance. These symptoms … ies support associations between psychological distress and pain-related outcomes, while studies of cervical dizziness and proprioception provide possible adjacent mechanisms. Autonomic, central pain-processing, inflammatory, sleep-related, and biomechanical pathways remain predominantly indirect or hypothetical in relation to atlantoaxial disease. CONCLUSIONS: Atl …

Occipitalization of the atlas.

This paper discusses occipitalization of the atlas and its relationship to the chiropractic practitioner. Patients commonly consult a chiropractor with complaint of headache, suboccipital stiffness, restricted motion, dizziness and other symptoms related to the upper cervical region. Differential diagnosis of the exact etiological factor of these symptoms must be made via a thorough history, physical examination, and roentgenological examination. If occipitalization of the atlas is detected on the initial roentgenological examination, then follow-up magnetic resonance imaging, computerized tom

[Chronic dizziness in elderly people: its clinical characteristics and magneto-encephalographic findings].

Many elderly people complain dizziness which may continue occasionally for months or years. According to epidemiological studies, 25-29% of subjects with more than 60 years of age have the experience of dizziness. Dizziness occurs most commonly during head positional changes or walking. Clinical studies have indicated that causes of dizziness are nonspecific and multi-factorial; cerebrovascular diseases, cervical spondylosis, depressive state, poor vision, orthostatic hypotension, whiplash injury, or low cerebrospinal fluid syndrome may play a role in the development of dizziness. Patients wit

Clinical control study of traditional Chinese medicine hot compress combined with traction in the treatment of cervical spondylotic radiculopathy: Study protocol.

AbstractCervical spondylotic radiculopathy (CSR) is the most common type of cervical spondylosis, accounting for about 60% of the incidence of cervical spondylosis. Both cervical traction and traditional Chinese medicine hot compress are common and effective treatment for CSR. This study will be performed to investigate the effect of a combination of cervical traction and traditional Chinese medicine hot compress on CSR. In this non-blinded, randomized controlled trial, 100 eligible patients will be randomly divided into a treatment group (intermittent cervical traction combines with tradition

Application of bilateral transcranial Doppler ultrasound in diagnosing vertebral artery compression syndrome.

BACKGROUND: Vertebral artery compression syndrome (VACS) is characterized by recurrent and reversible dizziness, consistently triggered by head rotation or hyperextension. Currently, no simple yet effective screening and diagnostic methods are available. This study aimed to explore the application of transcranial Doppler ultrasound with dual transducers (TCD-D) and a head stent in the screening and diagnosis of VACS. METHODS: From April 2018 to September 2024, three patients with dizziness upon head rotation, diagnosed as VACS, were enrolled. TCD-D was used to simultaneously monitor the real-t

Symptom relief after treatment of temporomandibular and cervical spine disorders in patients with Meniere's disease: a three-year follow-up.

This study describes the coordinated treatment of temporomandibular disorders (TMD) and cervical spine disorders in patients diagnosed with Meniere's disease. The aim was to follow up treatment outcomes for three years with regular follow-up examinations every six months. Of the 31 patients with Meniere's disease who participated in a controlled comparative study on the signs and symptoms of TMD, 24 participated in a subsequent controlled comparative study on the signs and symptoms of cervical spine disorders (CSD). These 24 Meniere's disease patients (ten males and 14 females) agreed to parti

Approach to cervicogenic dizziness: a comprehensive review of its aetiopathology and management.

PURPOSE: Though there is abundant literature on cervicogenic dizziness with at least half a dozen of review articles, the condition remains to be enigmatic for clinicians dealing with the dizzy patients. However, most of these studies have studied the cervicogenic dizziness in general without separating the constitute conditions. Since the aetiopathological mechanism of dizziness varies between these cervicogenic causes, one cannot rely on the universal conclusions of these studies unless the constitute conditions of cervicogenic dizziness are separated and contrasted against each other. Pulsatile Tinnitus as a Manifestation of Radiation-Induced Internal Carotid Artery Stenosis in a Nasopharyngeal Carcinoma Survivor: A Case Report.

Nasopharyngeal carcinoma (NPC) is prevalent in East and Southeast Asia, with radiotherapy being the primary treatment modality. Carotid artery stenosis is a serious but often overlooked complication of radiotherapy. We present the case of a middle-aged NPC survivor who developed progressive symptoms, including left facial numbness, dysphagia, tongue atrophy, postural dizziness, and pulsatile tinnitus, 18 years after concurrent chemoradiation. Carotid ultrasound revealed high-grade stenosis of the left cervical internal carotid artery. Following carotid stenting, his dizziness and tinnitus reso

Frequency of Oral Behaviors as a Risk Factor for Somatosensory Tinnitus.

Introduction  Somatosensory tinnitus is a type of tinnitus that can be modified by somatosensory stimuli from the cervical spine and temporomandibular area. Temporomandibular disorders and oral behaviors are associated with a higher prevalence of somatosensory tinnitus, and this association is described in the literature as part of the diagnosis of this condition. Objective  To verify the association between somatosensory tinnitus and oral behaviors. Methods  Patients were recruited from an Orofacial Pain outpatient clinic and from Head and Neck Unit. All participants underwent anamnesis, phys

[Severe orthostatic hypotension and intramedullary tumor: a case report and review of the literature].

A 55-year-old woman presented with bilateral neuropathic pain of the upper limbs, motor palsy of the right arm, urinary dysfunction, and postural dizziness. MRI showed an intramedullary cervical tumor with a solid portion extended from C1 to C3 surrounded by a cystic portion. A macroscopic complete resection was performed and histological examination confirmed the diagnosis of ependymoma. Postoperatively, the patient's neuropathic pain and postural dizziness worsened, with syncopal attack while upright because of severe orthostatic hypotension (OH). On physical examination, her supine systolic

[Giant thrombosed aneurysm of the left vertebral artery developing in the fourth ventricle].

A case of giant, thrombosed, non haemorrhagic aneurysm of the distal portion of the left vertebral artery is reported. The patient came to medical attention with an acute cervical pain after a minimal cervical traumatism and a diagnosis of torticollis from rheumatologic cause was made. In fact, a few weeks before, he had suffered three episodes of right homonymous hemianopsia. Subsequently, hiccup, vomiting, orthostatic dizziness with postural hypotension appeared, suggesting a medullary lesion. CT scan showed a round, heterogeneous high-density lesion near the fourth ventricle. Angiography wa
