Modern analysis of pathologic uterine rings.

A computerized medical record search of 61,406 live births from January 1, 1990, through December 31, 1994, identified 14 cases of pathologic uterine rings. This yielded an incidence of pathologic uterine rings of 0.02% of all live births. In a retrospective, matched study design, three control subjects having vaginal delivery and three having cesarean delivery were matched with each case of pathologic uterine ring for age, race, parity, estimated gestational age, single or multiple gestation, primary or repeat cesarean section, and indication for the cesarean delivery. Comparison with control

Unexpected Uterine Rupture-A Case Report, Review of the Literature and Clinical Suggestions.

BACKGROUND AND OBJECTIVES: Women with a history of cesarean section are a high-risk group because they are likely to develop uterine rupture during their next pregnancy. Current evidence suggests that a vaginal birth after cesarean section (VBAC) is associated with lower maternal mortality and morbidity than elective repeat cesarean delivery (ERCD). Additionally, research suggests that uterine rupture can occur in 0.47% of cases of trial of labor after cesarean section (TOLAC). CASE DESCRIPTION: A healthy 32-year-old woman at 41 weeks of gestation, in her fourth pregnancy, was admitted to the

An Analysis of the Rate, Indications, and Associated Maternal Mortality for Cesarean Sections at a Tertiary Care Hospital, First Report from Somalia.

BACKGROUND: There has been an increase in worldwide cesarean section rates, although they remain low in most Sub-Saharan African countries, including Somalia. The present is the first hospital-based study that analyzes the rate, indications, and associated maternal mortality of cesarean deliveries reported from Somalia. METHODS: This retrospective study reviewed data of all deliveries from 2015 to 2021 using electronic medical records in the hospital information system. Retrieved data include baseline demographic characteristics, mode of delivery, indications and the type of cesarean section,

Risks associated with subsequent pregnancy after one caesarean section: A prospective cohort study in a Nigerian obstetric population.

CONTEXT: Aversion for cesarean delivery is common in our practice and risks associated with caesarean section may contribute to this phenomenon. OBJECTIVE: The objective of this study was to estimate the risks associated with subsequent pregnancies in women with one previous cesarean section in a low resource setting. SETTING AND DESIGN: A prospective cohort study carried out at two major tertiary maternity centers in Enugu. MATERIALS AND METHODS: Maternal and perinatal outcomes were compared between women with one previous caesarean and women who had only previous vaginal deliveries. STAT

What We Have Learned About Trial of Labor After Cesarean Delivery from the Maternal-Fetal Medicine Units Cesarean Registry.

The cesarean delivery rate in the United States has risen steadily over the past 5 decades such that approximately one in three women now undergo cesarean section. The rise in repeat operations and accompanying decline in trial of labor after cesarean (TOLAC) have been major contributors to this phenomenon. The appropriate use of TOLAC continues to be a topic of interest with the recognition that most women with a history of prior cesarean are candidates for trial of labor. The NICHD MFMU Network Cesarean Registry conducted from 1999 to 2002 provided contemporary data concerning the risks and

Conservative management of a Cesarean scar ectopic pregnancy: a case report.

INTRODUCTION: Cesarean scar pregnancy is the rarest kind of ectopic pregnancy. The immediate prognosis depends on the risks associated with uterine rupture and massive bleeding. CASE PRESENTATION: A 32-year-old woman (gravida 2, para 1) presented with massive vaginal bleeding. A Cesarean scar pregnancy was diagnosed. She was treated by local methotrexate injection, followed by uterine artery embolization. Recurrence of bleeding necessitated two repeat embolizations. Hysteroscopy four months later revealed the presence of a uterine defect within the Cesarean section scar. CONCLUSION: Cesarean

Labor induction for planned vaginal delivery in patients with previous cesarean section.

The role of labor induction in patients scheduled for vaginal delivery following a previous lower segment cesarean section has been controversial. In Hong Kong, we have managed these patients by induction when there were obstetric or medical indications. A review of 137 patients who had labor induced for a trial of scar showed that induction of labor was a safe procedure and the rate of repeat caesarean section in these patients (18.2%) was similar to that in other patients with a trial of scar who had spontaneous onset of labor (12.9%). There were no serious maternal or fetal complications. A

Duration of a vaginal birth after cesarean section depending on the degree of cervical dilation in the prior cesarean delivery: A retrospective study.

OBJECTIVE: Labor and birth lead to significant immunological and regenerative changes in the uterus, such that multiparous women typically experience faster and more effective labor than primiparous women. In this retrospective study, we examined the influence of labor activity and childbirth progress in the previous pregnancy on the duration of vaginal delivery of a second child in women with a history of one cesarean section. METHODS: A total of 200 women bearing their second child with a trial of labor after cesarean who presented for delivery between January 2018 and December 2022 at the

Cesarean section.

Although the cesarean section rate has increased steadily for the past 12 years, further increase seems unlikely since the indications for performing the operation are already broadly defined. Most of the earlier indications will remain unchanged (eg, the presence of placenta previa and cephalopelvic disproportion). The trend toward vaginal delivery in perhaps 30% to 40% of breech births will probably have no material effect on the number of cesarean sections performed, and the present use of cesarean section for multiple pregnancy will probably continue. The two conditions under which cesarea

Uterus-preserving management of a high-flow uterine artery pseudoaneurysm with arteriovenous fistula following cesarean section: a case report.

Postpartum uterine vascular lesions are a rare but important cause of secondary postpartum hemorrhage. This report describes the case of a 29-year-old woman presenting five weeks after repeat cesarean section with recurrent vaginal bleeding. Transvaginal color Doppler ultrasound demonstrated a 2.7 cm anechoic lesion adjacent to the cesarean scar with marked hypervascularization and a high-flow pattern (peak systolic velocity up to 310 cm/s, low resistance indices) with pulsatile venous outflow, suggestive of a combined uterine artery pseudoaneurysm and arteriovenous fistula. Computed tomograph

[Anesthetic management in case of previous cesarean section].

An efficient communication between the obstetrics and anesthesiology teams is a prerequisite for an optimal management of a woman with a previous cesarean section (professional agreement). Epidural analgesia should be encouraged in this context due to a high risk of emergency obstetrical procedures, in order to avoid general anesthesia (professional agreement). When possible, spinal anesthesia is the technique of choice for elective repeat cesarean delivery even in case of morbidly adherent placenta (professional agreement).

Trial of labor after cesarean section in twin pregnancies: maternal and neonatal safety.

OBJECTIVE: To assess maternal and perinatal morbidity in patients undergoing a trial of labor after cesarean section (TOLAC) in twin gestations. METHODS: A retrospective study including all twin pregnancies with a single prior cesarean section was performed. Stratified analysis using a multiple logistic regression model was performed to control for confounders. Patients who had a clear medical indication for a cesarean section (i.e. previous corporeal cesarean section, breech or transverse presentation, placenta previa, placental abruption, and herpes infection) were excluded from the analysi

Planned mode of birth after previous cesarean section and risk of undergoing pelvic floor surgery: A Scottish population-based record linkage cohort study.

BACKGROUND: The global rise in cesarean sections has led to increasing numbers of pregnant women with a history of previous cesarean section. Policy in many high-income settings supports offering these women a choice between planned elective repeat cesarean section (ERCS) or planned vaginal birth after previous cesarean (VBAC), in the absence of contraindications to VBAC. Despite the potential for this choice to affect women's subsequent risk of experiencing pelvic floor disorders, evidence on the associated effects to fully counsel women is lacking. This study investigated the association bet

Shearing-force injury of a kidney transplant graft during cesarean section: a case report and review of the literature.

BACKGROUND: With an increasing number of reproductive-aged women undergoing renal transplantation coupled with improved fertility post-transplantation, more women are becoming pregnant with a kidney transplant in place. This leads to increased risk of perinatal complications such as pre-eclampsia, gestational diabetes, preterm delivery and Cesarean section. Given that kidney transplants are often placed extra-peritoneally in the iliac fossa, there is also a risk of damage to the transplanted kidney at the time of Cesarean section. CASE PRESENTATION: We present a case of shearing-force injury

A post-operative reaction to povidone‑iodine in a postpartum woman: A case report.

INTRODUCTION: High infection rates after cesarean section are a major concern in the United States. A majority of antiseptics have irritant properties if they are not used as specified in companies' guidelines. Allergic contact dermatitis caused by povidone‑iodine is infrequent and is misdiagnosed by clinicians who fail to differentiate allergy from irritation. This case report describes an allergic reaction to povidone‑iodine after cesarean section. CASE PRESENTATION: A 28-year-old underwent a repeat low transverse cesarean-section at 39 weeks of gestation. The patient informed the medical t
