The pharmacist's role in transforming nephrology care.

PURPOSE: To describe how pharmacists can enhance the care of individuals with chronic kidney disease by examining the role of the pharmacist in new care models. SUMMARY: The implications of chronic kidney disease for the healthcare system, particularly for the Centers for Medicare and Medicaid Services, are profound. New payment models and the development of medications that slow progression of kidney disease are transforming the landscape of nephrology care in the US. Pharmacists can play a pivotal role as part of the interdisciplinary care team to optimize medication therapy for this popula

Evaluation of an integrated HIV and hypertension management model in rural south africa: a mixed methods approach.

Background: A summary of Soter Ameh's PhD thesis titled, 'An integrated HIV and hypertension management model in rural South Africa: A mixed methods approach' is presented here. In responding to the dual high burden of non-communicable diseases (NCDs) and HIV in South Africa, the national government initiated an integrated chronic disease management (ICDM) model in health facilities as a pilot programme. The aim of the ICDM model is to leverage the successes of the innovative HIV treatment programme for NCDs to improve the quality of care and health outcomes of adult patients. Objectives: The s

The comorbidity of HIV, hypertension and diabetes: a qualitative study exploring the challenges faced by healthcare providers and patients in selected urban and rural health facilities where the ICDM model is implemented in South Africa.

BACKGROUND: PLWH are living longer as a result of advancement and adherence to antiretroviral therapy. As the life expectancy of PLWH increases, they are at increased risk of hypertension and diabetes. HIV chronic co-morbidities pose a serious public health concern as they are linked to increased use and need of health services, decreased overall quality of life and increased mortality. While research shows that integrated care approaches applied within primary care settings can significantly reduce hospital admissions and mortality levels among patients with comorbidities, the primary care sy

Optimizing Cost-Effective Care for Chronic Constipation in General Gastroenterology Using an Office-Based, Point-Of-Care Test (RED): Cost-Effectiveness and Cost-Minimization Analysis.

INTRODUCTION: We aimed to evaluate the cost-effectiveness/cost-savings of point-of-care anorectal function testing with an investigational device (RED) to triage therapy for chronic constipation in general gastroenterology. METHODS: A Markov model was constructed to evaluate cost-effectiveness/cost-savings over a 1-year time horizon comparing empiric drug/pelvic floor physical therapy to testing guided care. RESULTS: RED appears to inform the cost-effective strategy for chronic constipation. Compared with usual care without RED, it reduces insurer costs by $810 and patient costs by $6,903.

Pathways of health care for people living with multimorbidity in two Southern African countries.

Multimorbidity, the presence of multiple chronic conditions in one person, is a growing global health concern. Integration of chronic care services is urgently needed, especially in low-resource settings including in Southern Africa, where care has been fragmented by vertical and siloed disease approaches. Many countries share similar challenges of integration, presenting rich opportunities for shared learning. Yet, rarely are these opportunities capitalised upon, in part because of a lack of systematic knowledge about the similarities and differences in health system contexts, challenges and

Enhancing Patient-Centered Home Palliative Care for Heart Failure: A Concept Analysis to Guide Nursing Practice and Care Coordination.

To identify and describe components of the home-based palliative care model for patients with heart failure, re-conceptualize the process, and update the existing model to inform nursing research and clinical practice. Heart failure, as a chronic progressive disease, has a continuously increasing incidence and medical burden, and requires multidimensional clinical management strategies. Although palliative care is recommended for improving the quality of life of heart failure patients, its utilization rate is still low, and the core components and implementation pathways of home-based palliati

Processes and Outcomes of Congestive Heart Failure Care by Different Types of Primary Care Models.

BACKGROUND: Having nurse practitioners (NPs) as primary care providers for patients with congestive heart failure (CHF) is 1 way to address the growing shortage of primary care physicians (PCPs). METHODS AND RESULTS: We used inverse probability of treatment weighted with propensity score to examine the processes and outcomes of care for patients under 3 care models. Approximately 72.9%, 0.8%, and 26.3% of CHF patients received care under the PCP model, the NP model, and the shared care model, respectively. Patients under the NP or shared care models were more likely than those under the PCP m

Understanding chronic itch: a mechanistic framework for primary care.

BACKGROUND: Chronic itch is common in primary care but is often under-examined, partly because clinicians lack a clear mechanistic framework for interpreting its diverse presentations. AIM: To provide generalists with a practical, mechanism-based model for assessing and managing chronic itch. METHODS: Evidence from neurobiology, dermatology, and internal medicine was synthesised into a four-driver framework describing skin-generated, systemic, peripheral nerve, and centrally mediated itch. Clinical patterns, mechanistic pathways, and aligned treatments were integrated into a unified model fo

When Outcomes Become Currency: Reconsidering Value in Interventional Pain Management Under Emerging Episode-Based Payment Models.

The transition toward value-based care in the United States has introduced episode-based payment models that increasingly tie physician reimbursement to longitudinal costs and standardized outcomes. The Centers for Medicare & Medicaid Services (CMS) Ambulatory Specialty Model (ASM), targeting chronic low back pain (cLBP), represents a pivotal extension of this framework into interventional pain management. While intended to reduce low-value utilization, such models risk redefining clinical success in ways that may not align with the heterogeneous and biopsychosocial nature of chronic pain. Thi

'I worked until my body was broken': an ethnomedical model of chronic pain among North Korean refugee women.

OBJECTIVES: While increasing numbers of North Korean refugees (NKRs) are migrating to South Korea, different understandings of physical pain between NKR patients and the South Korean healthcare providers may create miscommunication and less satisfaction with their care management. To identify strategies to improve care, this study elicits and presents an ethnomedical model of chronic pain among NKR women. DESIGN: Twenty semi-structured individual interviews were conducted with NKR women with chronic pain living in South Korea. Inductive and deductive thematic analysis was performed on three t

Chronic Lower Limb Wound Outcomes Among Rural and Urban Veterans.

PURPOSE: Veterans in rural areas generally have lower health care utilization than veterans in urban areas, but the impact of this difference on health outcomes has received little study. Chronic wounds provide a model for studying access to complex chronic care since they often are related to underlying health conditions and require lengthy treatment. Our goals were to describe chronic wound care utilization among rural and urban veterans and to determine the association between rural residence and wound healing. METHODS: We conducted a retrospective cohort study of 160 rural and 160 urban v

An economic evaluation of a specialist preventive care clinician in a community mental health service: a randomised controlled trial.

BACKGROUND: Clinical practice guidelines and policies direct community mental health services to provide preventive care to address chronic disease risks, however, such care is infrequently provided in routine consultations. An alternative model of care is to appoint a clinician to the dedicated role of offering and providing preventive care in an additional consultation: the 'specialist clinician' model. Economic evaluations of models of care are needed to determine the cost of adhering to guidelines and policies, and to inform pragmatic service delivery decisions. This study is an economic e
