Risk factors for fractured clavicle in the newborn.

AIM: To disclose potential risk factors for sustaining a fractured clavicle in the newborn. METHOD: A retrospective case-control analysis of women who gave birth to an infant with a fractured clavicle during a four-year period (2003-2006) was performed. A control group of newborns who did not sustain a fractured clavicle was formed (2:1) matched for maternal age, parity and gestational age at delivery. RESULTS: The rate of fractured clavicle was 0.35%. Heavier newborns' birth weight (3632.9 ± 376.1 g vs. 3429.5 ± 513.0 g, P < 0.05) and the use of oxytocin (91.3% vs. 69.5%, P < 0.05) were ass

Resection of radiation-induced sarcoma of the clavicle.

We report here the resection of a radiation-induced sarcoma (RIS) of the left clavicle developed in a 59-year-old woman 13 years after radiation for breast cancer. Surgery consisted of extirpation of the tumor with a combined resection of the total layer of the chest wall, the left brachiocephalic vein, and the left subclavian vein, reconstructed with a pediculated musculocutaneous graft using the right latissimus dorsi muscle. RIS of the clavicle is rare, and the prognosis might be poor. However, a complete removal of the tumor is feasible and can be performed safely.

[Congenital pseudarthrosis of the clavicle in two siblings].

Congenital pseudarthrosis of the clavicle is a rare disorder of unknown etiology. Among nearly 200 cases hitherto reported, only a few cases have familial coexistence, and none are first-degree relatives. A nine-year old girl had complaints of weakness in the right arm and swelling in the right shoulder. On physical examination, a mass-like lesion in the right clavicle, abnormal clavicular movement, and asymmetric shoulders were noted. The range of motion of the shoulder was in normal range on both sides. A plain radiogram showed a defect in the diaphysis of the right clavicle and computed tom

Congenital pseudarthrosis of the clavicle: a case report.

Congenital pseudarthrosis of the clavicle is a rare entity of unknown aetiology. Its pathogenesis is related to the embryology of the clavicle. We present a 6-year-old girl with congenital pseudarthrosis of the right clavicle. A prominence was noticed at birth between the middle and distal ends of the clavicle that increased in size when the right shoulder was actively mobilised. Radiographic examination revealed a hypertrophic pseudarthrosis of the clavicle. The pseudarthrosis was resected and the clavicular segments were fixed with an external fixator for 2 months until union. Clinical resul

Midshaft malunions of the clavicle. Surgical technique.

BACKGROUND: The purpose of this study was to analyze the functional results of corrective osteotomy of a malunited clavicular fracture in patients with chronic pain, weakness, neurologic symptoms, and dissatisfaction with the appearance of the shoulder. METHODS: We identified fifteen patients (nine men and six women with a mean age of thirty-seven years) who had a malunion following nonoperative treatment of a displaced midshaft fracture of the clavicle. The mean time from the injury to presentation was three years (range, one to fifteen years). Outcome scores revealed major residual deficits

Hydatid disease at the proximal end of the clavicle.

We report a case of hydatid disease at the proximal end of the clavicle of a 47-year-old woman. In an open biopsy, laminated layers, characteristic of Echinococcus multilocularis larvae, were revealed by periodic acid-Schiff (PAS) staining. A marked foreign-body reaction was observed in the pathological findings. The patient was definitely diagnosed as having osseous hydatid disease. We performed resection of the infected part of the clavicle, and after the operation, she has been cured under medication with albendazole for 9 years, with no finding of recurrence in the clavicle.

Single or double plating for nonunion of the clavicle.

Between January 1982 and January 1999, 684 patients presented with a fracture of the clavicle to the accident and emergency departments of the Tamside and Bury District General Hospitals. Twenty patients (3%) subsequently developed symptomatic nonunion of the clavicle. The original injury resulted from a road traffic accident in 13 patients, from a fall on an outstretched hand in five patients, and two patients had sports related injuries. Mean age of the patients was 39 years (range, 17 to 76 years). Mean time from injury to surgery was 2.5 years (range, 6 months to 8 years). Fifteen patients

Fractures of the clavicle in the adult. Epidemiology and classification.

From 1988 to 1994 a consecutive series of 1000 fractures of the adult clavicle was treated in the Orthopaedic Trauma Clinic of the Royal Infirmary of Edinburgh. In males, the annual incidence was highest under 20 years of age, decreasing in each subsequent cohort until the seventh decade. In females, the incidence was more constant, but relatively frequent in teenagers and the elderly. In young patients, fractures usually resulted from road-traffic accidents or sport and most were diaphyseal. Fractures in the outer fifth were produced by simple domestic falls and were more common in the elderl

'Duplication' of the clavicle or ossification in coracoclavicular ligament.

A case is presented in which plain films revealed a short band of ossification adjacent to the inferior aspect of the lateral part of the clavicle. CT findings indicated that the ossification was situated in the coracoclavicular ligament and was not a manifestation of clavicular 'duplication'.

[Surgical repair of fractures of the clavicle in the adult].

Clavicular fractures make up 45% of shoulder girdle fractures. The clavicle's susceptibility to injury is due to its subcutaneous position and its role as the bony connection between the thorax and the shoulder. In 95% of cases the mechanism of injury is a direct blow to the shoulder. These fractures are usually treated conservatively without surgery. But there are a few such fractures that require surgical repair in order to unite well. 9 patients were operated on for clavicular fractures during 1991-1995. The indications for surgical repair were lateral-third fracture, floating shoulder, neu

Fractures of the distal clavicle: a case for fixation.

We present a series of 43 Neer type 2 fractures of the distal one-third of the clavicle. These displaced fractures are associated with a high incidence of non-union. In those patients treated non-operatively there was a higher incidence of local complications, residual shoulder dysfunction and non-union than in those treated by open reduction and internal fixation. We recommend that type 2 fractures should be treated by open reduction and internal fixation.

Current concepts in the treatment of fractures of the clavicle.

Clavicle fracture is a common injury in all age groups. Injuries can be classified into groups. Group I includes fractures of the middle one third, the most frequent site. Group II fractures account for 10% of fractures of the clavicle and involve the clavicle lateral to the coracoclavicular ligament and are caused by direct violence. These injuries are divided into two subsets. Group II Type I fractures occur lateral to the coracoclavicular ligaments and are stable. Group II Type II fractures occur just medial to the coracoclavicular ligaments and are unstable. These latter injuries require s

Fracture of the clavicle in the newborn. An ultrasound diagnosis.

Forty-one cases of clavicle fracture in newborn babies were examined by both radiographic and ultrasonic methods. No substantial difference has been found between these two modalities. It is suggested that ultrasound should be the procedure of choice in the diagnosis of clavicle fracture. This is especially true in those cases where impaired movement of the arm is the only clinical sign.

[Treatment of dislocations of the acromial end of the clavicle].

Based upon an analysis of observations of 93 patients treated for dislocation of the acromial end of the clavicle and a study of long-term results of the treatment of 68 of them, the authors recommend using the method of a one-step closed setting the acromial end of the clavicle followed by a percutaneous transarticular fixation of the clavicular-acromial junction with the Kirschner wire and plaster bandage for 6 weeks.

Post-traumatic osteolysis of the distal clavicle.

Five cases of painful bone resorption from the distal clavicle following shoulder trauma are reported to demonstrate that the lesion is generally benign. Surgical excision is curative. The multiplicity of circumstances leading to clavicular osteolysis suggest that the balance between bone accretion and resorption is precarious in this region of the skeleton.
