OBJECTIVE: To report a case of possible clonidine-induced hypertension (by Naranjo score) in a patient with a C4 spinal lesion.

Clonidine, an effective antihypertensive drug acting on central presynaptic alpha 2-adrenoreceptors, is commonly used in opiate withdrawal. There is recent evidence of advantages in using clonidine also in alcohol withdrawal. The results of a randomized study on 44 alcoholics, admitted without withdrawal symptoms, are shown. The alcohol withdrawal syndrome appeared in 50% of the patients in the placebo group and in 9% of the clonidine treated patients. Clonidine is an effective tool in the hospital preventive treatment of alcohol withdrawal.

Morphine and clonidine both elevated plasma levels of lidocaine to the same extent in mice while slowing lidocaine metabolism to deethylated products. The effects of clonidine on lidocaine disposition were reversed by yohimbine. Mice given morphine, 20 mg/kg sc, or clonidine, 0.2 mg/kg sc, had similar, 30-50%, elevation of plasma lidocaine levels at 15 min after lidocaine, 15 mg/kg iv, when compared to saline-treated animals. Clonidine hydrochloride (an alpha-2 adrenergic agonist) and naltrexone hydrochloride (an opioid antagonist), given in combination, provided a safe and effective treatment of abrupt opioid withdrawal over 5 days in an outpatient/day setting. Systemic and regional hemodynamic changes (microsphere technique) were studied in urethan-chloralose anesthetized cats after intravenous clonidine and naloxone administration in different sequence. It was shown that antihypertensive effect of clonidine was caused by a decrease in the cardiac output and an increase in the total peripheral vascular conduction. The ability of clonidine to increase total peripheral conduction was completely blocked with naloxone preadministration. It was concluded that endogenous opiate receptors take part in the vasodilating clonidine effect in abdominal organs.

The role of opioidergic system in the antihypertensive effect of clonidine was investigated in albino normotensive and renal-DOCA-salt hypertensive models of rats. There are conflicting data concerning the efficacy of clonidine in TS. Some TS patients, probably 50% or more, experience substantial, long-term symptomatic improvement with minimal side effects. However, their profile of response is often variable, with behavioral symptoms appearing to show the most consistent improvement. Maximal benefit may not be evident for 4 to 6 months. Twenty-three cases of the climacteric syndrome were analysed in a double-blind study comparing the effect of conjugated equine oestrogen (Premarin) and medrogestone (Colpro) with clonidine (Dixarit) on various clinical parameters. The treatment lasted 20 weeks. Statistical analysis of the results indictaed that opposed oestrogen therapy was effective in reducing hot flushes (P < 0,05) whereas clonidine was not. The other variables tested did not attain statistical significance.

CONTEXT: Following clonidine ingestion, naloxone is seldom administered as it is considered ineffective in reversing somnolence, bradycardia, or hypotension. However, this conclusion has been based on administration of small doses (2 mg or less) of naloxone.
