For surgical treatment of the unstable Colles fracture we developed a new form of osteosynthesis, which consists in a modification of the dynamic Kirschner wire fixation described by Kapandji. We followed forty women with functional deficits in the wrist and hand after sustaining a Colles' fracture. The women participated in occupational therapy three times a week for three weeks. At the initial evaluation, after three weeks, and at a three month evaluation, we measured the following: range of joint movement, grip strength, hand volume (oedema), pain and ADL. In a prospective population-based investigation, we measured bone mineral density (BMD) of the forearm using single-photon absorptiometry at both a distal and a more proximal site in 74 Colles'-fracture patients who were compared with controls matched for age, sex, and years after menopause. For both groups there was a marked inverse relationship between age and bone mass. However, over the entire age range, the probands had 11 percent reduced BMD when compared with the controls. A single-blind study was carried out in 52 patients with severe pain after Colles' fracture to assess the analgesic efficacy and tolerability of 500 mg diflunisal twice daily compared with that of 500 mg mefenamic acid given 3-times daily over a period of 5 days. The results showed that both treatments were effective in relieving pain, night pain and limitation of movement by pain, and there was no significant difference between the response in the two groups. Both drugs were tolerated and only 3 patients (2 on diflunisal) reported mild drug-related side-effects.
