Screening is a useful tool for identifying potential health issues; however, it can also lead to overtreatment. Consequently, patients are sometimes harmed by unnecessary treatments and there are cost implications. Overtreatment can also occur in other areas of medicine besides screening and sometimes medical interventions are used to improve performance rather than to treat disease. In this paper, a distinction is made between the perspectives of the patient and the government. For patients, autonomy is important, and they can refuse life-saving treatments, assuming they have decision-making

Litigation forced the dissolution of three major tobacco industry-funded organisations because of their egregious role in spreading scientific misinformation. Yet in 2017, a new scientific organisation-the Foundation for a Smoke-Free World (FSFW)-was launched, funded entirely by tobacco corporation Philip Morris International (PMI). Experts fear FSFW similarly serves to benefit its funder's scientific and political agenda. We present three case studies of FSFW's publishing practices to explore: whether FSFW and its affiliates are acting with scientific integrity in their attempts to publish re

Within the context of journal peer review, when high-quality referee input is coupled with appropriate editorial oversight from a journal's leadership, the result can be the selection and publication of high-quality manuscripts that are ideally targeted to the journal's readership's interests. However, in its worse forms, flawed referee input and indifferent- or misdirected journal leadership can result in damage to the quality of materials published and the relationships among, and careers of, the involved parties. This article identifies methods that journals can employ to select journal lea

INTRODUCTION: Financial conflicts of interest (fCOI) present well documented risks to the integrity of biomedical research. However, few studies differentiate among fCOI types in their analyses, and those that do tend to use preexisting taxonomies for fCOI identification. Research on fCOI would benefit from an empirically-derived taxonomy of self-reported fCOI and data on fCOI type and payor prevalence.
METHODS: We conducted a content analysis of 6,165 individual self-reported relationships from COI statements distributed across 378 articles indexed with PubMed. Two coders used an iterative c

BACKGROUND: Financial ties between healthcare professionals and the medical-device industry raise conflict-of-interest and transparency concerns; despite Japan's 2012 self-regulatory guidelines, device-related payments receive less scrutiny than pharmaceutical payments globally, including in Japan.
OBJECTIVE: To provide the first detailed analysis of honoraria from medical device companies to Japanese physicians in 2019, offering a baseline before the COVID-19 pandemic.
METHODS: We retrospectively examined 2019 payment data from the Yen for Docs database, compiled from disclosures by 118 com

Industry interference remains a major barrier to effective tobacco control. WHO Framework Convention on Tobacco Control (FCTC) Article 5.3 obliges Parties to protect public health policies from industry influence, yet systematic monitoring of interference is often lacking. The emergence of new nicotine products and actors further complicate implementation and monitoring. Within Joint Action PreventNCD, we assessed monitoring and information exchange practices related to tobacco and nicotine industry interference in five European countries (Belgium, Estonia, Finland, Iceland, and Slovenia). Patient and public involvement is increasingly advocated in health policy, research and practice. Patients and people with lived experience, carers and the general public should have a say in how policy is generated, how services are delivered and how research is conducted. Through this perspective article, we hope to stimulate discussion and debate around industry influence in patient and public involvement, specifically pertaining to patient organizations, which often play a key role in patient and public involvement activities. As momentum gathers around patient and public involvement in ma

Financial contributions from industry for physician-led research have been historically challenging to study in plastic surgery. However, as mandated by the Physician Payments Sunshine Act of 2013, the Open Payments Database (OPD) has increased transparency in payments from industry to physicians. This study aimed to analyze trends in industry-sponsored research funding for plastic surgeons. Using the OPD, research payments from industry made to plastic surgeons from 2014 to 2018 were examined. Total payments and number of payments were recorded by recipient's census region (e.g., Northeast, M

Medical professionalism faces distinctive challenges in the 21st century. In this chapter, we review the history of professionalism, address specific challenges physicians face today, and provide an overview of efforts to address these issues, including behavioral and virtue ethics approaches. First, we discuss core features professions share and the development of codes of medical ethics that guide the practice of western medicine. Second, we address challenges related to the doctor-patient relationship, continuity of care, cultural competence, conflicts of interest, and the regulation of qua

BACKGROUND AND AIM: Transparency regarding experts' conflicts of interest is important for the credibility of national clinical guidelines. We examined conflicts of interest among authors of the Norwegian Directorate of Health's action plans for cancer.
MATERIAL AND METHOD: We asked the Norwegian Directorate of Health for access to disclosure forms completed by the authors of all 26 national action plans for cancer. We compared the information in these forms with information on potential conflicts of interest in the published action plans, the authors' PubMed-indexed articles published during

We appreciate Reierson's thoughtful commentary on our 2019 paper, which described our experiences, ethical process, judgment calls, and lessons from a 2016-2017 data-sharing pilot between Crisis Text Line and academic researchers. The commentary raises important questions about the ethical conduct of health research in the digital age, particularly regarding informed consent, potential conflicts of interest, and the protection of vulnerable populations. Our article focused specifically on the noncommercial use of Crisis Text Line data for research purposes, so we restrict our reply to points r
