Workplace AED resuscitation of a patient with a rare congenital anomalous coronary circulation.

We describe the case of a 44-year-old male with anomalous pulmonary artery origin of the left main coronary artery who was successfully resuscitated after sudden cardiac death with use of an automated external defibrillator (AED) in the workplace. The diagnosis was made on emergency cardiac catheterization and treated with surgical intervention. This case illustrates: 1) that congenital anomalies, although rare, must continue to be considered in the differential diagnosis of sudden cardiac death, and 2) that AEDs in the workplace can be a crucial, lifesaving intervention in the time-dependent

Pharmacologic assessment of the functional state in stenosed coronary circulation of the dog.

In open chest dogs under sodium pentobarbital anaesthesia the interaction of mechanical constriction on a large coronary branch and autoregulatory capacity of the relevant small resistance vessels was analyzed. Coronary blood flow (CBF) was measured with an electromagnetic flowmeter. Step-by-step mechanical constriction gradually abolished adenosine-induced coronary vasodilation, whereas the resting level of mean CBF remained unaltered. At this point verapamil (0.2 mg/kg i.v.), a vasodilator with a strong potency of blocking adenosine action, eventually decreased CBF and increased coronary res

[Hypothalamic mechanisms of regulating blood circulation in the coronary and brachial artery systems].

In anesthetized curarized cats effects of high frequency stimulation of anterior, ventromedial, and posterior hypothalamus on arterial blood pressure (ABP) and on perfusion pressure of the coronary and brachial arteries were studied. The stimulation induced vasoconstriction of coronary vessels and increased the ABP. In most experiments, especially in case of stimulation of anterior hypothalamus, simultaneously with increase of perfusion pressure of the coronary artery, a decrease of the brachial artery perfusion pressure was revealed. The mechanism of vasodilatation of the brachial artery seem

Accuracy of exercise tolerance test in the diagnosis of coronary artery disease in patients with left dominant coronary circulation.

BACKGROUND: Exercise is a physiologic stress that helps the physicians to clarify the presence or absence of cardiovascular disease which may be obscure at rest. Although it is sensitive, its specificity is affected by several parameters, such as some metabolic conditions, some structural heart diseases, and some baseline electrocardiogram abnormalities. Currently, the relationship between coronary dominance and accuracy of EET is not examined. Therefore, this study was conducted to determine the potential impact of coronary dominance on the accuracy of EET. METHODS: In this retrospective stu

Significance of soluble lectin-like oxidized LDL receptor-1 levels in systemic and coronary circulation in acute coronary syndrome.

BACKGROUND: Soluble lectin-like oxidized low-density lipoprotein receptor-1 (LOX-1) level is a novel biomarker for diagnosis of acute coronary syndrome (ACS); however, this level in the coronary circulation has yet to be examined. METHODS: Twenty-seven consecutive patients with ACS and 40 patients with effort angina pectoris (EAP) undergoing percutaneous coronary intervention (PCI) had levels of soluble LOX-1 and LOX-1 index measured in paired blood samples from aorta (Ao) and coronary sinus (CS) just prior to the PCI. RESULTS: We found positive correlations between soluble LOX-1 levels in t…

Circulating Exosomal CircRNAs as Diagnostic Biomarkers for Chronic Coronary Syndrome.

Circular RNA (circRNA) has been reported to be involved in the pathogenesis of cardiovascular disease; however, it is unclear whether circRNA carried by exosomes (exos) can be used as biomarkers for chronic coronary syndrome (CCS). High-throughput sequencing was carried out in the plasma exosomal RNA of 15 CCS patients and 15 non-cardiac chest pain patients (NCCP, control group) to screen for differentially expressed circRNAs. Selected differentially expressed exo-circRNAs were further verified by real-time polymerase chain reaction in a small-sample cohort and a large-sample cohort. A total o

Circulating miR-214 is associated with the severity of coronary artery disease.

OBJECTIVE: To study whether miR-214 is regulated in coronary artery disease (CAD) patients and whether placental growth factor (PLGF) is a possible target for miR-214 in atherosclerosis. METHODS: Circulating miR-214 was measured by quantitative PCR using RNA isolated from 40 patients with CAD, including 12 with stable angina pectoris, 16 with unstable angina pectoris and 12 with acute myocardial infarction, and 15 controls without CAD. Plasma level of PLGF was measured by ELISA. RESULTS: The miR-214 level was significantly lower in CAD patients compared with that in controls (P < 0.01). Comp

Circulating Th22 and Th9 levels in patients with acute coronary syndrome.

BACKGROUND: CD4+ T helper (Th) cells play critical roles in the development and progression of atherosclerosis and the onset of acute coronary syndromes (ACS, including acute myocardial infarction (AMI) and unstable angina pectoris (UAP)). In addition to Th1, Th2, and Th17 cells, Th22 and Th9 subsets have been identified in humans. In the present study, we investigated whether Th22 cells and Th9 cells are involved in the onset of ACS. METHODS: The frequencies of Th22 and Th9 cells were detected using a flow cytometric analysis and their related cytokine and transcription factor were measured

Increased circulating soluble LR11 in patients with acute coronary syndrome.

BACKGROUND: LR11 (also so called SorLA or SORL1) is a novel marker of intimal smooth muscle cell (SMC) proliferation. Vascular SMCs play important roles in the development of atherosclerosis interacting with macrophages in a vulnerable plaque of patients with acute coronary syndrome (ACS).The present study determines whether soluble LR11 (sLR11) is associated with ACS. METHODS: We studied 100 patients with coronary artery disease (CAD) comprising 50 consecutive patients with acute coronary syndrome (ACS; mean age 62.3±13.0 years; male 78.0%) who were successfully treated with percutaneous cor

Increased platelet binding to circulating monocytes in acute coronary syndromes.

BACKGROUND: Present therapies for acute coronary syndromes aim toward limiting platelet-platelet adhesion and aggregation processes. However, platelet-leukocyte interactions may contribute importantly to disease progression in the arterial wall. Recent studies suggest that prevention of platelet-leukocyte binding via P-selectin glycoprotein ligand-1 (PSGL-1) may be beneficial in animal models of vascular injury. METHODS AND RESULTS: P-selectin-PSGL-1 interactions were found to account for most platelet-monocyte binding observed in peripheral blood samples from healthy donors. However, a signi

The role of adenosine in functional hyperaemia in the coronary circulation of anaesthetized dogs.

1. The aim of this investigation was to determine the contribution of adenosine to coronary active hyperaemia in the dog denervated heart by using adenosine deaminase. 2. Beagles were anaesthetized with thiopentone sodium (500 mg, I.V.) and chloralose (100 mg kg-1, LV.) and artificially ventilated. The hearts were denervate by bilateral cervical vagotomy and cardiac sympathectomy. Blood samples were collected from the coronary sinus via a cannula passed through the right external jugular vein. The anterior descending or circumflex branch of the left coronary artery was cannulated and perfused

Effect of percutaneous transluminal coronary angioplasty on circulating endothelin levels.

Percutaneous transluminal coronary angioplasty (PTCA) is frequently associated with vasoconstriction involving large vessels as well as microcirculation, but the potential mechanisms remain poorly defined. In this study, we tested the hypothesis that endothelial disruption during PTCA is associated with an increase in circulating levels of endothelin, a potent endothelium-derived vasoconstrictor peptide. Circulating levels of endothelin and other potential vasoactive mediators such as atrial natriuretic factor, epinephrine and norepinephrine were measured immediately before and after PTCA in 2

Development of circulating antiheart antibodies as a result of coronary bypass surgery.

Antiheart antibodies have been implicated as a marker of postcardiotomy syndrome in patients undergoing coronary artery bypass. To assess the frequency of and contributory factors in the development of antiheart antibodies after coronary bypass procedures, 33 patients were examined for evidence of antiheart antibodies before and for seven days after routine coronary artery bypass operations. Overall titers of antiheart antibodies rose in approximately 66% of the patients. However, this rise was not accompanied by any major clinical symptoms. History of previous myocardial infarction or myocard

[Adaptive structures of the arteries of the heart participating in the regulation of coronary circulation].

By means of a complex of anatomical, histological and histochemical methods cardiac vessels have been studied in 20 control dogs and in 84 d … disturbances in the heart vascular system. The active role of these formations in regulation of the coronary circulation is demonstrated owing to a high content of ribonucleinic acid and glycogene in their smooth muscle cells and also an elevated succinate dehydrogenase, cytochromoxydase, acid and alcaline phosphatase activities.

Elevated Uric Acid/Albumin Ratio Predicts Poor Coronary Collateral Circulation in Type 2 Diabetic Patients With Stable Coronary Artery Disease.

BACKGROUND: The uric acid/albumin ratio (UAR) has been shown to correlate with coronary disease severity and clinical outcome. This study investigated the predictive value of UAR on coronary collateralization (CC) in patients with type 2 diabetes mellitus (T2DM) and stable coronary artery disease. METHODS: Serum levels of uric acid and albumin were determined and UAR was calculated in 1558 T2DM patients with chronic total occlusion of at least one major coronary artery. The degree of collaterals supplying to the distal occluded bed from the contra-lateral vessel was graded by Rentrop scoring
