Older adults with type 2 diabetes (T2D) tend to have normal or greater areal bone mineral density (aBMD), as measured by DXA, than those who do not have diabetes (non-T2D). Yet risk of fracture is higher in T2D, including 40% to 50% increased hip fracture risk. We used HR-pQCT to investigate structural mechanisms underlying skeletal fragility in T2D. We compared cortical and trabecular bone microarchitecture, density, bone area, and strength in T2D and non-T2D. In secondary analyses we evaluated whether associations between T2D and bone measures differed according to prior fracture, sex, and o

The aim of the present study was to investigate the relationships between physical activity and metacarpal cortical bone mineral density (BMD) and bone resorption in hemiplegic patients. Seventy-two male hemiplegic patients with cerebrovascular disease (mean age, 67.0 years; range 48-83 years) were recruited. Metacarpal cortical BMD in bilateral hands was measured by computed X-ray densitometry, and the levels of urinary cross-linked N-telopeptides of type I collagen (NTx), as a bone resorption marker, were measured by an enzyme-linked immunosorbent assay (ELISA). BMD (mean +/- SD) in the para

Secondary osteoporosis is a major global health problem and can be caused by endocrine diseases, including hyperthyroidism. Of note, hyperthyroid mice exhibit both trabecular and cortical bone loss, accompanied by reduced bone strength, as a consequence of a high bone turnover in which bone resorption exceeds bone formation. Recently, we discovered that osteocytes in bones obtained from hyperthyroid mice develop osteoclast-like features. However, the role of thyroid hormone receptor α (TRα) in hyperthyroidism-driven bone resorption remains ill-defined. Here, we rendered male Thra 0/0 mice (whi

This study aimed to evaluate vertical bone gain in the anterior edentulous mandible using the split cortical bone plate technique with 2 different graft and membrane combinations: autogenous particulate grafts with titanium-reinforced polytetrafluoroethylene membrane (T1) and a mixture of autogenous and xenograft particulate grafts with a collagen membrane (Bio-Gide) (T2). Twenty-two patients presenting with severe vertical alveolar ridge atrophy were randomly allocated into 2 equal groups with 11 participants in each group. Cone beam computerized tomography was used to assess vertical bone ga

Atypical femoral fractures are rare fractures that occur in the subtrochanteric or diaphyseal region of the femur with minimal or no trauma. Though the association of atypical femoral fractures (AFFs) and bisphosphonate (BP) use is a growing concern in the management of osteoporosis, currently there is little knowledge about which patients may be at risk for an atypical femoral fracture. Given that these fractures initiate in the femoral cortex, we aimed to determine whether cortical bone tissue properties (bone material strength index; BMSi), as measured by in vivo impact microindentation, ar

Deletion of connexin (Cx) 37 in mice leads to increased cancellous bone mass due to defective osteoclast differentiation. Paradoxically; however, Cx37-deficient mice exhibit reduced cortical thickness accompanied by higher bone strength, suggesting a contribution of Cx37 to bone matrix composition. Thus, we investigated whether global deletion of Cx37 alters the composition of organic bone extracellular matrix. Five-month-old Cx37-/- mice exhibited increased marrow cavity area, and periosteal and endocortical bone surface resulting in higher total area in tibia compared to Cx37+/+ control mice

OBJECTIVES: Metacarpal radiogrammetry is widely used by anthropologists and archaeologists to assess cortical bone loss. However, the dynamics of metacarpal cortical bone acquisition, as it relates to epiphyseal fusion during late adolescence, requires greater clarification. This research uses the Burlington Growth Study, a longitudinal dataset of digitized hand-wrist radiographs from a known age and sex sample, to investigate cortical bone growth in the second (MC2) and third (MC3) metacarpals of adolescents aged 16-20 years old.
MATERIALS AND METHODS: Medullary widths (MW), total widths (TW

The cortical bone trajectory (CBT) is a novel technique in lumbar fixation and fusion. The unique caudocephalad and medial-lateral screw trajectories endow it with excellent screw purchase for vertebral fixation via a minimally invasive method. The combined use of CBT screws with transforaminal or posterior lumbar interbody fusion can treat a variety of lumbar diseases, including spondylolisthesis or stenosis, and can also be used as a remedy for revision surgery when the pedicle screw fails. CBT has obvious advantages in terms of surgical trauma, postoperative recovery, prevention and treatme

Rib fractures are a common and serious outcome of blunt thoracic trauma and their likelihood is greater in older individuals. Osteoporotic bone loss is a well-documented aging phenomenon with sex-specific characteristics, but within rib bones, neither baseline maps of regional thickness nor the rates of bone thinning with age have been quantified across whole ribs. This study presents such data from 4014 ribs of 240 adult subjects aged 20-90. A validated cortical bone mapping technique was applied to clinical computed tomography scans to obtain local rib cortical bone thickness measurements ov

Poor bone quality induced by non-enzymatic glycation (NEG) of bone tissue in patients with type 2 diabetes mellitus (T2DM) is regarded as the major factor of bone fragility and affecting bone mechanical properties. A comprehensive and systemic mechanical investigation for evaluating the effect of NEG on bone was still lacking. In order to provide additional information for the bone quality of T2DM, the effects of NEG on mechanical properties of cortical bone were investigated in terms of elastoplasticity, fracture toughness and viscoelasticity. All samples of cortical bone, including the sampl

Physiological wear and tear causes bone microdamage at several hierarchical levels, and these have different biological consequences. Bone remodeling is widely held to be the mechanism by which bone microdamage is repaired. However, recent studies showed that unlike typical linear microcracks, small crack damage, the clusters of submicron-sized matrix cracks also known as diffuse damage (Dif.Dx), does not activate remodeling. Thus, the fate of diffuse damage in vivo is not known. To examine this, we induced selectively Dif.Dx in rat ulnae in vivo by using end-load ulnar bending creep model. Ch

Ontogenetic growth processes in human long bones are key elements, determining the variability of adult bone structure. This study seeks to identify and describe the interaction between ontogenetic growth periods and changes in femoral and tibial diaphyseal shape. Femora and tibiae (n = 46) ranging developmentally from neonate to skeletally mature were obtained from the Norris Farms No. 36 archeological skeletal series. High-resolution X-ray computed tomography scans were collected. Whole-diaphysis cortical bone drift patterns and relative bone envelope modeling activity across ages were asses

Ridge resorption following edentulation has been documented clinically, but the effects of tooth loss on the material properties of mandibular cortical bone have received little study. Material properties and their structural basis are essential for our understanding of bone quality in the edentulous mandible and are of interest as a tissue-level model for functional adaptation. This study's aim was to determine material property variability in the edentulous mandible, and to compare it with data from a previous study of dentate mandibles. Forty-four cortical samples were removed from each of

We have treated osteochondral lesions of the talus using cortical bone pegs. We examined 27 ankles (27 patients) after a mean follow-up of 7.0 years (2 to 18.8). The mean age of the patients was 27.8 years (12 to 62). An unstable osteochondral fragment or osteosclerotic changes in the bed of the talus were regarded as indications for the procedure. The clinical results were good in 24 ankles (89%) and fair in three (11%); none had a poor result. There was also radiological improvement in 24 ankles. Repair of the articular surface and stability of the lesion can be achieved even in unstable chr

Calcific tendinitis of the gluteus maximus tendon is unusual and only eight cases associated with erosions of cortical bone have been reported. We present three further cases which demonstrate variation in appearances of the amorphous calcification in the tendon and cortical erosions on plain radiographs. Isotope bone scans show focal increased activity and computed tomography (CT) clearly demonstrates erosion of cortical bone and calcification within the tendon insertion of gluteus maximus. Follow-up examinations in two cases demonstrated resolution of the changes. We believe this series demo

Ten knees in nine patients with large, symptomatic osteochondritis dissecans lesions of the knee were treated by cortical bone peg fixation. The follow-up time averaged 2.9 years (range, 0.9-6.6 years). The average age was 19.5 years (range, 15-33 years). The knees were evaluated and rated clinically and roentgenographically. Magnetic resonance (MR) imaging of nine knees was obtained at the follow-up examination to evaluate lesion and bone peg incorporation, evidence of lesion loosening, and the articular cartilage. Eight good to excellent, one fair, and one poor result were obtained clinicall

The purpose of this investigation was to measure the reduction in bone strength resulting from drill holes in diaphyseal bone and to compare this with finite element and theoretical predictions for stresses in a tubular structure. Fifty-two pairs of canine femora were tested to failure in four-point bending. One bone of each pair was used as the control; the other femora had holes of variable size drilled in the lateral cortex. At a ratio of drill hole diameter to bone diameter of 0.2, the bone retained only 62% of its expected strength. A linear regression between the area fraction (the ratio

Periosteal, intracortical, and endosteal resorptive changes were evaluated in both hands and the proximal radius in 161 chronic renal-failure patients by microradioscopy, radiographic morphometry, and photodensitometry. These changes were compared to abnormalities found in skeletal surveys. A combination of microradioscopy and morphometry of hand bones was sufficiently sensitive for early detection of bone loss in renal osteodystrophy in 61% of the patients. Intracortical resorption in metacarpals appeared to be a sign of more advanced renal osteodystrophy than subperiosteal resorption in phal
