An analysis of 125 patients with a history or clinical evidence of cryptorchidism and testicular germinal tumor treated at our hospital from 1934 to 1975 is presented. Cryptorchidism was corrected ipsilaterally or contralaterally in 83 patients with intrascrotal testis cancer when they were from 4 to 42 years old, either spontaneously (21 patients), by orchiopexy (51 patients) or by hormonal therapy (11 patients). The chromosomes of undescended tests from 13 boys were analyzed. In 12 of these patients the peripheral blood chromosomes were examined and in 7 the chromosomes of normal skin cultures were examined. In addition to standard techniques the chromosomes also were analyzed by G and Q-banding methods. We were unable to detect any significant abnormalities of either chromosome number of structure in the undescended testes. In this case report, abdominal symptoms simulating acute appendicitis were due to recurrent torsion of an intra-abdominal testicle. Cryptorchidism frequently goes unnoted. Most undescended testicles are in the groin and easily palpable. If not, they can be absent, retroperitoneal or intra-abdominal. An intra-abdominal testicle is more likely to occur on the right than the left.

Intravenous urography of 78 cryptorchid boys revealed no clinically significant upper urinary tract anomalies. Two boys had a rotated kidney and 2 others a double renal pelvis. One boy had previously been operated upon because of hydronephrosis. There thus appears to be no reason for routine intravenous urography of cryptorchid boys. Forty-two per cent of the boys had spina bifida occulta in the lumbar and sacral spine. One case of asymptomatic congenital cardiac disease was discovered at routine chest X-ray.

A total of 1849 boys born to mothers resident in a defined area around Oxford were examined for cryptorchidism. Those born in hospital were examined at birth and again after three months if cryptorchid at the earlier examination. The incidence of cryptorchidism at three months adjusted to the birthweight distribution of England and Wales was 1.58%. By comparison, in a very similar study conducted around 1960 the incidence was 0.96%. Torsion of the testicle is a well-recognized urological emergency. Torsion of the cryptorchid testicle, however, is described in the urology literature, but not well recognized in the emergency department. We discuss a case of a torsed cryptorchid testicle and review the pathophysiology, imaging modalities, and prognosis.

Prospective clinical studies have shown that the prevalence of cryptorchidism among boys with birth weight > or =2500 g has increased in UK from 2.7 to 4.1% between the 1950s and the 1980s and in Denmark from 1.8 to 8.4% between the 1950s and the 1990s. Development of normal germ cells during childhood is a continuous process which ends at puberty. Cryptorchid gonads have severe impairment of their germ cell development which is more pronounced the higher the gonads are situated. However, all newborns with intra-abdominal testes had a normal number of germ cells. This strongly supports the theory that cryptorchidism is a disease and not a malformation. As a consequence, adequate and early treatment of cryptorchid boys should be undertaken in order to preserve good chances of fertility.

A 6-year-old boy whose chief complaint was right undescended testis was referred to our clinic. He had mental retardation, prominent and widely spaced eyes, large and low-set ears, micrognathia, hyperpigmentation and enlarged genitalia. Therefore, he was diagnosed to have Leprechaunism. Leprechaunism is rare and few studies dealing with this disease have been reported in Japan. With a review of the literature, we briefly discuss about the clinical features of Leprechaunism.
