A technique for retrofitting a metal ceramic crown to an attachment-retained removable partial denture: a clinical report.

In dental applications, precision attachments have been used to retain removable partial dentures (RPDs) for several decades. Various types of extracoronal attachments are commonly used in combination with fixed partial dentures and RPDs to achieve retention and stability. Fracture of the framework, fracture of the roots or teeth, and irretrievable decrease of retention are common reasons for a failed attachment-retained RPD. Another complication of metal ceramic crowns with precision attachment is decementation of the crowns. When fixed components of the attachment-retained RPD fail, the trad

Sectional impression tray and sectional denture for a microstomia patient.

Microstomia presents a unique challenge to the patient. Patients with microstomia who must wear removable dental prostheses often face the difficulty of being unable to insert or remove the prosthesis because of the constricted opening of the oral cavity. A completely edentulous patient, who developed microstomia along with Raynaud's phenomenon induced by scleroderma, is presented. This clinical report describes a quick and easy method for fabrication of a sectional custom impression tray connected by press button and a sectional complete denture retained by magnets. A sectional denture that p

Chairside options for the treatment of complete denture problems associated with the atrophic (flat) mandibular ridge.

This article outlines a number of simple clinical steps which should enable general dental practitioners to diagnose and treat the majority of complete denture problems in patients with atrophic mandibular ridges. The guiding principle is the reduction of forces transmitted to the denture-bearing area via the lower denture. Methods of optimising the size of the denture bases and/or the occlusal tables are discussed and illustrated.

Prosthetic rehabilitation of an edentulous patient with an oronasal fistula.

The presence of an oronasal fistula presents a challenge to maxillary complete denture fabrication because leakage of air from the nasal cavity through the fistula prevents the formation of an adequate border seal. Although surgical repair or dental implants are possible solutions, these options are invasive and sometimes not feasible. This clinical report illustrates an alternative prosthetic solution by integrating a small retentive component into a maxillary complete denture.

Hyperzincemia from ingestion of denture adhesives.

The purpose of this article is to review the recent literature that documents the serious adverse systemic effects of prolonged, excessive zinc ingestion from the overuse of denture adhesives. This condition causes elevation of serum zinc levels that result in depression of serum copper. The low serum copper levels cause bone marrow depression and widespread sensory and motor neuropathies. Epidemiologic studies revealed the source of excessive zinc intake to be from overuse of denture adhesives. Denture patients must be advised of the risks of prolonged overuse of denture adhesives.

The performance of two-implant overdentures in the atrophic maxilla: a case series with 1-year follow-up.

PURPOSE: To assess the implant and prosthesis survival rates, the clinical, radiographical and patient-related outcome measures, and the masticatory performance of maxillary overdentures supported by two implants in patients with an atrophic maxilla. METHODS: In this case series, 15 consecutive patients who were eligible for maxillary implant overdenture therapy, but who had insufficient bone volume to place at least four implants and were unwilling to be treated with reconstructive surgery were asked to participate. After giving consent, participants received two implants in the maxilla unde

Halitosis manifestation and prevention means for patients with fixed teeth dentures.

The objective of this research is to analyse the causal relationship between construction of fixed bridge dentures and the intensity of halitosis manifestations, as well as to establish basic hygiene requirements for construction of fixed dentures which would completely exclude retention of food particles and avoid bad breath. 48 patients (36 men and 12 women), who use fixed dentures for 2-10 years, have been involved in this research. 26 patients wore fixed bridge dentures made of punched tooth crowns, the other 22 patients wore cast fixed dentures. The obtained measurements of halitosis magn

Abutment-Bar Structure Connection Geometry: An Important Design Parameter for Implant-Supported Bar-Retained Overdentures With Cantilever Extension.

When extended distally due to higher loading in the posterior region, implant-supported bar-retained overdentures with cantilever bar extension exhibit greater bending moments on the implants closest to the cantilever bar and increased stresses in the overdenture components. In this study, a new abutment-bar structure connection was introduced to minimize undesired bending moments and reduce the resulting stresses by increasing the rotational mobility of the bar structure on the abutments. Copings of the bar structure were modified to have 2 spherical surfaces, sharing the same center, located

Repair potential in periodontal disease: the interproximal channel.

Interproximal channeling defects associated with lingual denudation of the root are often encountered in patients with insidiously progressive chronic periodontitis. Several cases of such lesions are described and their potential for repair demonstrated. It is suggested that this type of defect is predictably amenable to successful therapy by exposure and curettage, and that restoration of lost periodontal structures can be anticipated within the interproximal portion of the lesion.

Oronasal communication caused by a denture with suction cup: a case report.

Suction cups, which provide high retention, are not being recommended anymore because of the destructive effect of the negative pressure on the palatal tissues. It is known that dentures with suction cups can cause perforations in the palate. In this case report, an oronasal communication caused by a denture with suction cup in a patient who had previously undergone pleomorphic adenoma excision of the palate is presented. Disadvantages of suction cups are emphasized.

Prosthodontic rehabilitation for edentulous patients with palatal defect: report of two cases.

Cancer resection is the most common cause of acquired palatal defects, whereas cleft palate is the main cause of congenital defects. Palatal defect can be repaired by reconstructive surgery and/or a dental prosthesis. We present prosthodontic rehabilitation of two maxillary edentulous patients, one with a surgically induced palatal defect and the other with congenital cleft palate. In case 1, an 86-year-old man underwent surgical removal of soft-palate squamous cell carcinoma. The acquired palatal defect was repaired by a maxillary complete denture with a posteriorly extended speech bulb. The

Conversion of a failing fixed implant prosthesis into an implant-retained overdenture: A clinical report.

Implants supporting a fixed prosthesis may fail leaving unfavorable implant distribution for a fixed prosthesis. This clinical report illustrates a treatment option for converting a failing fixed prosthesis into a mandibular complete overdenture by using a portion of the existing fixed implant prosthesis. The use of a resilient liner to retain a converted mandibular overdenture framework provided satisfactory results in the first year of clinical use.
