Optimizing deprescribing of hospital-initiated opioids: a multi-level consensus study bridging evidence, expertise, and patient perspectives.

Hospital-initiated opioid analgesics that extend beyond discharge can lead to long-term use and adverse outcomes. Despite a growing understanding of opioid-related harms, there is a lack of published protocols for structured deprescribing of hospital-initiated opioids in Europe. Opioid stewardship initiatives, which encompass coordinated approaches to optimize prescribing practices and reduce long-term use, require deprescribing frameworks that extend beyond medication discontinuation to include patient communication and systematic care coordination. We aimed to develop an actionable opioid de

Shared decision-making and deprescribing to support anti-thrombotic therapy (dis)continuance for persons living with cancer in their last phase of life: A realist synthesis.

INTRODUCTION: Patients with cancer are at increased risk of thrombotic complications from both the disease and its treatments, with antithrombotic therapy (ATT) usually continued in the last phase of life where the benefit is less clear and there is high risk of harms. Physiological changes toward the end of life increase the risk that ATT will cause serious bleeding events, but discussion between clinicians and patients of ATT risks and benefits is sub-optimal. This realist synthesis explores shared decision-making (SDM) to: (a) provide insights into why prescribing continues in end-of-life c

Deprescribing tools and guidelines in chronic kidney disease: A scoping review.

BACKGROUND: Chronic kidney disease (CKD) is a major global health concern that is associated with multiple complications and comorbidities, leading to polypharmacy, inappropriate prescribing, and increased risk of adverse drug events. Deprescribing has emerged as an effective strategy to mitigate these consequences. Evidence-based guidelines are essential to support appropriate deprescribing practices in this population. A variety of deprescribing tools and guidelines are now widely available, but little is known about their utility in CKD setting. This study aimed to identify and characterize

Facilitators and Barriers Influencing Antipsychotic Medication Prescribing and Deprescribing Practices in Critically Ill Adult Patients: a Qualitative Study.

BACKGROUND: Antipsychotic medications do not alter the incidence or duration of delirium, but these medications are frequently prescribed and continued at transitions of care in critically ill patients when they may no longer be necessary or appropriate. OBJECTIVE: The purpose of this study was to identify and describe relevant domains and constructs that influence antipsychotic medication prescribing and deprescribing practices among physicians, nurses, and pharmacists that care for critically ill adult patients during and following critical illness. DESIGN: We conducted qualitative semi-st

Anticholinergic deprescribing interventions for reducing risk of cognitive decline or dementia in older adults with and without prior cognitive impairment.

BACKGROUND: Anticholinergics are medications that block the action of acetylcholine in the central or peripheral nervous system. Medications with anticholinergic properties are commonly prescribed to older adults. The cumulative anticholinergic effect of all the medications a person takes is referred to as the anticholinergic burden. A high anticholinergic burden may cause cognitive impairment in people who are otherwise cognitively healthy, or cause further cognitive decline in people with pre-existing cognitive problems. Reducing anticholinergic burden through deprescribing interventions may

Barriers and enablers for deprescribing benzodiazepine receptor agonists in older adults: a systematic review of qualitative and quantitative studies using the theoretical domains framework.

BACKGROUND: Many strategies aimed at deprescribing benzodiazepine receptor agonists (BZRA) in older adults have already been evaluated with various success rates. There is so far no consensus on which strategy components increase deprescribing the most. Yet, despite an unfavourable benefit-to-risk ratio, BZRA use among older adults remains high. We systematically reviewed barriers and enablers for BZRA deprescribing in older adults. METHODS: Two reviewers independently screened records identified from five electronic databases-Medline, Embase, PsycINFO, CINAHL and the Cochrane library-and pub

Successful deprescribing of unnecessary proton pump inhibitors in a primary care clinic.

OBJECTIVE: The objective of this study was to determine the rate of successful deprescribing of unnecessary proton pump inhibitors (PPIs) after implementation of a clinical pharmacist- managed program that included detailed tapering instructions, patient education, and follow-up. SETTING: The setting was a family medicine clinic in St. Paul, MN, in an underserved community. PRACTICE DESCRIPTION: Eligible patients were taking a long-term PPI for gastroesophageal reflux disease without esophagitis, or without a clear indication. Each morning, the clinical pharmacist generated a report in the e

Prolonged use of benzodiazepine in primary health care: evaluation of effectiveness, dependence and cognitive function.

BACKGROUND: Anxiety and insomnia are global health challenges often managed in Primary Health Care (PHC). Benzodiazepines (BZD) are commonly prescribed, but prolonged use increases risks such as cognitive impairment, dependence, and tolerance. This study assessed PHC users with prolonged BZD use for anxiety or insomnia, focusing on dependence, effectiveness, and cognitive function. RESEARCH DESIGN AND METHODS: A cross-sectional study was conducted with 144 prolonged BZD users in PHC. Data collection included sociodemographic and clinical questionnaires, alongside instruments assessing depende

Quality of Life Assessment in Patients Using Benzodiazepines during the COVID-19 Pandemic in a Community Pharmacy Using EuroQol 5D-3L.

Users of benzodiazepines (BZDs) should have their quality of life monitored to minimize the risks associated with long-term treatments. The aim of this study is to use the EuroQol 5D-3L to analyze the quality of life of 127 patients under treatment with BZDs during the COVID-19 pandemic. The results show that lorazepam comprises 25.49% of all dispensing requests, and that the mean duration of BZDs treatments is four years (range: 0.3-25). When rating their general health status, BZDs users reported 59.29 points out of 100. Thirty-two percent of patients reported mobility problems; 16.5% report

Benzodiazepine and Z drug cessation in elderly patients: A qualitative study on the perception of healthcare providers and the place of advanced practice nurses.

Sedative-use disorder can occur in elderly individuals, but remains a taboo subject. France is the second top-consuming country of benzodiazepines (BZDs) and Z drugs in Europe, with 38% of women >80 years old using these drugs. Despite the recommendations of the French National Authority for Health (HAS) to general practitioners, deprescription remains rare. The aim of this study was to understand the perception of healthcare providers towards BZD or Z drug withdrawal within a psychogeriatric unit and to provide insights from advanced practice nurses (APRNs) on this topic. Eight healthcare pro

Cardiovascular and non-cardiovascular concerns with proton pump inhibitors: Are they safe?

The introduction of proton pump inhibitors (PPIs) has been a cornerstone in the treatment of acid-related disorders, such as gastroesophageal reflux and peptic ulcer, and their use has increased rapidly during the last decades. Being highly lipophilic drugs, they may potentially affect several pathophysiological pathways involved in cardiovascular and kidney morbidity, immune response and infections, absorption of selected nutrients, bone metabolism and cognitive function. Clinical epidemiology evidence coming from well-designed analyses of observational data consistently reported that long-te

Practical aspects of managing multimorbidity in older adults with cancer.

PURPOSE OF REVIEW: Managing multimorbidity in older adults with cancer is a central, complex challenge in modern oncology. Historically, this population was underrepresented in clinical trials, leaving clinicians without practical guidance. This review synthesizes recent evidence that moves beyond simply documenting frailty to deploying targeted, evidence-based interventions to improve supportive and palliative care. RECENT FINDINGS: The literature supports a practical 2-step approach to assessment, using screening tools like the Geriatric-8 to trigger a full Comprehensive Geriatric Assessmen

Pharmacological principles for safe benzodiazepine and Z-drug dose reduction.

Many guidelines recommend deprescribing benzodiazepines and z-drugs in most long-term users. Due to physical dependence in those using medication as prescribed (distinct from addiction), discontinuation can be difficult and is often unsuccessful, as withdrawal symptoms can be severe and long-lasting, especially after long-term use. There is a lack of clarity on how to taper patients in a tolerable manner that minimizes discomfort. Current guidelines vary with some suggesting linear reductions (e.g. 1 mg every 1-4 weeks) and some proportionate reductions (e.g. 5%-10% of the most recent dose per

Impact of balneotherapy on drug consumption: a systematic review and an economic evaluation of drug deprescribing.

Some previous studies have suggested that balneotherapy could be associated with a decrease in drug consumption. No study has investigated pharmacoeconomic consequences in terms of monetary savings. The present study was performed to evaluate potential savings (according to societal perspective) associated with decreased drug consumption resulting from spa treatments using randomized clinical trials (RCT) identified after a systematic review. Among 48 RCT, 8 trials investigating rheumatological diseases (low back or shoulder pain, osteoarthritis, ankylosing spondylitis, fibromyalgia), were inc

Clinical practice guideline for deprescribing opioid analgesics: summary of recommendations.

INTRODUCTION: Long term opioids are commonly prescribed to manage pain. Dose reduction or discontinuation (deprescribing) can be challenging, even when the potential harms of continuation outweigh the perceived benefits. The Evidence-based clinical practice guideline for deprescribing opioid analgesics was developed using robust guideline development processes and Grading of Recommendations, Assessment, Development and Evaluation (GRADE) methodology, and contains deprescribing recommendations for adults prescribed opioids for pain. MAIN RECOMMENDATIONS: Eleven recommendations provide advice a

Deprescribing psychotropic medicines for behaviours that challenge in people with intellectual disabilities: a systematic review.

BACKGROUND: Clear evidence of overprescribing of psychotropic medicines to manage behaviours that challenges in people with intellectual disabilities has led to national programmes within the U.K. such as NHS England's STOMP to address this. The focus of the intervention in our review was deprescribing of psychotropic medicines in children and adults with intellectual disabilities. Mental health symptomatology and quality of life were main outcomes. METHODS: We reviewed the evidence using databases Medline, Embase, PsycINFO, Web of Science, CINAHL and Open Grey with an initial cut-off date of
