Hypersensitivity reactions to biologic agents.

Biologic agents (BAs) are important therapeutic tools; their use has rapidly expanded and they are used in oncology, immunology, and inflammatory diseases. Their use may be limited, however, by adverse drug reactions. This article reviews the current literature on clinical presentation and pathogenic mechanisms of both acute and delayed reactions. In addition, procedures for management of BA-induced reactions, including preventive and diagnostic work-up, are provided. Lastly, this article summarizes the current knowledge of desensitization to several widely used monoclonal antibodies.

[Cypress pollen allergy].

Cypress belongs to the Cupressaceae family, which includes 140 species with non-deciduous foliage. The most important genera in allergic diseases are Cupressus sempervirens or Green cypress, Cupressus arizonica or Blue cypress, Juniperus oxycedrus, Juniperus communis and Thuya. Because J. oxycedrus pollinates in October, C. sempervirens in January and February, C. arizonica in February and March, J. communis in April, the symptomatic period is long-lasting. Because of global warming, the pollination period is tending to last longer and Cupressaceae species are becoming established further the

Antibiotic hypersensitivity reactions and approaches to desensitization.

Before initiating antibiotic therapy, drug hypersensitivity is an important consideration, and a common strategy is to avoid giving patients medications when a high likelihood of severe reactions exists. With an increase in antibiotic resistance and a decrease in novel antibiotics, there is greater pressure to consider antibiotics in patients with a history of adverse reactions. The major concerns include IgE-mediated, or type I, reactions, anaphylaxis, Stevens-Johnson syndrome, and toxic epidermal necrolysis. Some antibiotics with similar characteristics, such as cephalosporins and penicillin

[Dynamic immunologic reactivity of schizophrenics during psychopharmacotherapy as exemplified by virus-induced reactions of the blood lymphocytes].

Clinical immunological investigation of 149 schizophrenic patients, 89 healthy donors and 38 infected patients revealed the fact of high lymphocyte reactivity in schizophrenic patients inhabitants of the city of Tomsk and Tomsk Province to the viral antigens of herpes simplex and Far-Eastern encephalitis. The course of herpes-induced immune responses was largely associated with the course of psychopathological signs. The role of immunomodulating and desensitizing effects of psychotropic therapy is discussed.

[Reactions to drugs. A review of literature].

Adverse drug reactions are originated by immunological and non-immunological mechanisms. Estimated incidence is of 6.7%, and the incidence of mortal reactions is of 0.32%. In women, skin reaction incidence is 35% higher than in men, and it is 20 times higher than presenting contrast medium-related reactions. Allergic reaction is classified accordingly to Gell and Coombs criteria. It is unknown the pathogenic mechanism of the drug-induced fever. Predictable reaction depends on the dose and it is frequently related to the drug. Unpredictable reaction depends on the individual and pseudoallergic

Hypersensitivity to PEG-Asparaginase and Desensitization Strategies in Pediatric Acute Lymphoblastic Leukemia: A Narrative Review.

This narrative review synthesizes current evidence on incidence, the immunologic basis of hypersensitivity-including the recently clarified role of anti-PEG (rather than anti-protein) antibodies-genetic predisposition, differential diagnosis from asparaginase-associated hyperammonemia, therapeutic drug monitoring (TDM), and desensitization. Hypersensitivity to PEG-asparaginase is a frequent, clinically consequential complication of pediatric acute lymphoblastic leukemia therapy that can compromise treatment exposure and survival. Recombinant Erwinia asparaginase (crisantaspase) is now licensed

Immune system modulation in the highly sensitized transplant candidate.

Heart transplantation can be a lifesaving option for patients with end-stage heart failure. However, implanting recipients with an organ or tissue from a donor presents immunologic challenges. Sensitized recipients are at risk for hyperacute rejection because of the presence of preformed antibodies. Immune modulation with cyclophosphamide, plasmapheresis, and intravenous gamma globulin are methods used to desensitize transplant candidates with preformed anti-HLA antibodies to achieve successful transplantation.

Safety of Epicutaneous Immunotherapy in Peanut-Allergic Children: REALISE Randomized Clinical Trial Results.

BACKGROUND: Treatment options for peanut allergy are limited. In previous clinical trials, epicutaneous immunotherapy with a patch containing 250-μg peanut protein (Viaskin Peanut 250 μg [VP250]) was well tolerated and statistically superior to placebo in desensitizing peanut-allergic children. OBJECTIVE: To examine the safety of VP250 in children, using a study design approximating potential real-world use. METHODS: REAL LIfe Use and Safety of EPIT (REALISE) is a phase 3 multicenter study consisting of a 6-month, randomized, double-blind, placebo-controlled period followed by open-label act

Evaluation of the highly sensitized transplant recipient.

The immune response against alloantigens involves the production of antibodies and development of T-cell immunity. Recipients sensitized to HLA antigens may have antibodies to almost all donors and may not be able to find a suitable kidney transplant donor. Strategies available to enable these patients to obtain a transplant are to give priority to highly sensitized patients, to perform therapy for antibody reduction or to transplant with existing antibodies and to intervene as needed with post-transplant treatment. While sensitization against HLA antigens is the most important cause of immuno

A high panel-reactive antibody rescue protocol for cross-match-positive live donor kidney transplants.

BACKGROUND: Alloimmunization can present a virtually insurmountable barrier to kidney transplantation. Past protocols to desensitize patients using plasmapheresis and cyclophosphamide have not been broadly applied because of the fear of complications, including high rates of immunologic failure. METHODS: Fifteen patients with a positive donor-recipient cross-match were desensitized with plasmapheresis to permit live donor (LD) transplantation under newer maintenance immunosuppressants. Pretransplant the patients received plasmapheresis three times weekly for a planned maximum of six treatment

[Desensitization to pirfenidone. Report of one case].

We report a 68-year-old woman with a history of idiopathic pulmonary fibrosis, who had immediate skin reactions associated with the use of pirfenidone in two opportunities. In the evaluation by immunology, an allergy to pirfenidone was diagnosed. Given the urgent need for the drug, a desensitization procedure was started. The dose of the medication was increased progressively, previous premedication with cetirizine, ranitidine and montelukast. After this procedure the patient was able to tolerate the treatment with pirfenidone without cutaneous reactions.

[B-cell pseudolymphoma caused by aluminium hydroxide following hyposensitization therapy].

Aluminium hydroxide is used as an adjuvant in vaccines. We describe the case of a patient who presented a persistent adverse local reaction to aluminium hydroxide due to hyposensitization therapy to dust mites. Multiple painful and pruriginous subcutaneous nodules were observed in both arms, along with hypertrichosis at the injection site. Histology revealed a pseudolymphomatous B cell reaction predominantly involving cells that were CD20 positive, did not express bcl-2, and did not display the t(14-18) translocation. The cells also exhibited polyclonal rearrangement of the immunoglobulin heav

Fighting food allergy: current approaches.

Food allergy is defined as an adverse immunologic reaction to allergens present in food and is associated with symptoms ranging from gastrointestinal discomfort to anaphylactic shock and death. The increase in prevalence and potential fatality of disease has led to increased efforts to find effective therapies and prophylaxis. While specific immunotherapy (SIT) is effective in desensitization against inhalant allergens, it is unadvised against food allergy because of the high risk of adverse side effects. A review of the recent literature shows that various approaches have been taken to develo

Desensitization in Iron Product Allergy.

Iron deficiency is the main cause of anemia in both sexes, with women being more commonly affected. Iron therapy is currently considered an effective and safe remedy to replenish the iron storages. Iron can be administrated both orally and intravenously. In particular, intravenous (IV) iron therapy is widely used when oral iron preparations are either not tolerated or ineffective. Indeed, IV iron improves iron stores more rapidly. Two main immunological responses have been described for iron hypersensitivity reactions (HSRs): IgE-mediated allergy and complement activation-related pseudo-allerg

[Specific features of the clinical course and treatment of chronic tonsillitis in children].

This author applied present-day methods--clinical, immunological, immunomorphological--to examine 2262 children with chronic tonsillitis and processed the data obtained statistically. The emergence and development of chronic tonsillitis were strongly stimulated by preceding cases of angina, acute respiratory pathologies, flu, and other infectious diseases. The children were treated using dioxidine, desensitizing drugs, vitamins, ultrasonic and tubus-quartz exposures as well as levamisole. The treatment was effective and caused no complications.

A new perspective on the regulation of food allergy: the tilt from a single component to a multi-component system.

The pathological mechanism of food allergy (FA) is mainly related to the abnormal activation of Immunoglobulin E (IgE)-mediated Th2 immune response. Although traditional methods can reduce immunogenicity by destroying the structure of allergens, they have limitations such as nutritional loss. A novel functional modulation system was constructed based on the two core pathways of allergen desensitization and active substance potentiation. On the one hand, through the establishment of coupling and protein modification systems, the precise masking of allergen epitopes is realized, and allergenicit
