Burden of serious harms from diagnostic error in the USA.

BACKGROUND: Diagnostic errors cause substantial preventable harms worldwide, but rigorous estimates for total burden are lacking. We previously estimated diagnostic error and serious harm rates for key dangerous diseases in major disease categories and validated plausible ranges using clinical experts. OBJECTIVE: We sought to estimate the annual US burden of serious misdiagnosis-related harms (permanent morbidity, mortality) by combining prior results with rigorous estimates of disease incidence. METHODS: Cross-sectional analysis of US-based nationally representative observational data. We e…

Identification of factors associated with diagnostic error in primary care.

BACKGROUND: Missed, delayed or incorrect diagnoses are considered to be diagnostic errors. The aim of this paper is to describe the methodology of a study to analyse cognitive aspects of the process by which primary care (PC) physicians diagnose dyspnoea. It examines the possible links between the use of heuristics, suboptimal cognitive acts and diagnostic errors, using Reason's taxonomy of human error (slips, lapses, mistakes and violations). The influence of situational factors (professional experience, perceived overwork and fatigue) is also analysed. METHODS: Cohort study of new episodes …

[Scabies vs. histiocytosis X: a possible diagnostic error].

Scabies is a relatively common disease among Spanish children. The classic papulovesicular type is not usually a diagnostic problem. Therefore, the nodular type, much less frequent, is unknown for a lot of specialists (except dermatologist), and even it is mistaken for Histiocytosis or Lymphoma. Authors present a female infant, 6 months old, with nodular skin lesions whose first diagnosis was X-Histiocytosis. Regarding the big difference between the two processes (in respect of prognosis and treatment), this chance should be excluded, among others, when X-Histiocytosis is suspected in a child

Pulmonary Manifestations of Inborn Errors of Immunity: Diagnostic and Therapeutic Insights.

BACKGROUND: Inborn errors of immunity (IEIs) are a heterogeneous group of genetically determined disorders that lead to immune dysfunction, recurrent infections, and organ-specific complications. The lungs are among the most commonly affected organs, with both infectious and noninfectious manifestations that significantly contribute to morbidity and mortality. This study aimed to provide a comprehensive overview of pulmonary manifestations in IEI, with emphasis on pathophysiological mechanisms, diagnostic approaches, and therapeutic strategies. METHODS: A narrative review and synthesis of cur

Infantile Spasms in Inborn Errors of Metabolism: Diagnostic and Therapeutic Considerations.

Infantile epileptic spasms syndrome is an early-onset epileptic encephalopathy. Its association with inborn errors of metabolism is underrecognized, and the treatment can be complicated by the biochemical vulnerability. This report showed individualized treatment in 2 patients with inborn errors of metabolism. The first patient was found to have propionic acidemia in the setting of encephalopathy by third day of life. Infantile epileptic spasms syndrome was diagnosed at 10 months. Levetiracetam was started, and vigabatrin was introduced following liver transplant, which resulted in seizure fre

Autopsy-detected diagnostic errors over time in the intensive care unit.

We evaluate the evolution over time of discrepancies between clinical diagnoses and postmortem findings in critically ill patients and assess the factors associated with these discrepancies. We conducted a prospective study of all consecutive patients who underwent autopsy in a medical-surgical intensive care unit (ICU) between January 2008 and December 2015. Among 7655 patients admitted to our ICU, 671 (8.8%) died. Clinical autopsy was performed in 215 (32%) patients. Major missed diagnoses were noted in 38 patients (17.7%). Eighteen patients (8.4%) had class I discrepancies, and 20 patients

Diagnostic errors in abdominopelvic CT interpretation: characterization based on report addenda.

PURPOSE: The purpose of the article is to characterize the diagnostic errors in abdominopelvic CT interpretation through review of radiology report addenda. METHODS: We searched abdominopelvic CT reports for the word "addendum" over a nearly seven-year period. Addenda were reviewed to identify those reporting a diagnostic error. Cases were characterized by a spectrum of features. RESULTS: 709 addenda describing 785 diagnostic errors were identified, representing approximately 0.5% of searched reports. 84.1% were a new finding, 5.1% an upgrade in severity of an originally reported finding, 3.…

Diagnostic and therapeutic errors in cluster headache: a hospital-based study.

BACKGROUND: Cluster headache (CH) is a severe, disabling form of headache. Even though CH has a typical clinical picture it seems that its diagnosis is often missed or delayed in clinical practice. CH patients may thus face: misdiagnosis, unnecessary investigations and delays in accessing adequate treatment. This study was conducted to investigate the occurrence of diagnostic and therapeutic errors with a view to improving the clinical and instrumental work-up in affected patients. METHODS: Our study comprised 144 episodic CH patients: 116 from Italy and 28 from Eastern European countries (Mo

[Diagnostic errors and mortality in those suffering in road traffic accidents].

The paper deals with the analysis of diagnostic errors and lethality causes among the victims of traffic accidents. It was found that 40.75% of cases died from craniocerebral injuries, 32.5%-from traumatic shock and blood loss. Complete coincidence of clinical and medicolegal diagnosis was in 47.05% of cases, and complete discrepancy in 8.13%. The greatest number of errors are concerned with the diagnosis and treatment of chest and abdomen injuries. The least number of errors were made in big multiprofile clinics with well organized reanimation service. Some recommendations on the improvement …

Cluster Headache: Economic and Psychological Burdens of Diagnostic Errors and Delays.

Cluster headache (CH) is a devastating primary headache disorder and is widely recognized as one of the most severe pain conditions encountered in clinical practice. The goal of this narrative review is to examine the challenges contributing to the delayed recognition and diagnosis of CH. We divided the cost of CH into direct and indirect costs. We found that direct costs include healthcare services, such as emergency department, primary care, and specialist visits, diagnostic tests, pharmacy services, oxygen therapy, and neuromodulation devices, and increased healthcare utilization, including

Diagnostic errors in abdominal pain: a secondary analysis of case reports.

OBJECTIVES: Diagnostic errors remain common in abdominal pain, a frequent yet challenging presentation. We sought to compare diagnostic error profiles between abdominal and non-abdominal pain and to identify abdominal pain-specific, potentially modifiable targets in physical examination, diagnostic testing, and differential diagnosis. METHODS: We conducted a retrospective secondary analysis of a systematically coded database of diagnostic error case reports. Adult pain cases were categorized as abdominal or non-abdominal after excluding pediatric and non-pain cases. Errors were classified usi…

Epidemiology of diagnostic errors in pediatric emergency departments using electronic triggers.

OBJECTIVES: We applied three electronic triggers to study frequency and contributory factors of missed opportunities for improving diagnosis … sing electronic triggers with selective record review is an effective process to screen for harmful diagnostic errors in EDs: detailed review of 5% of charts revealed MOIDs in half, of which half were harmful to the patient. With further refining, triggers can be used as effective patient safety tools to monitor diagnostic quality.

Nationwide questionnaire survey on diagnostic errors in pediatric outpatient acute care.

BACKGROUND: Diagnostic errors are harmful and occur at unacceptably high rates. However, data regarding diagnostic errors in pediatric populations, particularly in acute outpatient care settings, remain insufficient. This study aimed to investigate the frequency of diagnostic errors, contributing factors, common symptoms, initial diagnoses, and final diagnoses in pediatric outpatient acute care in Japan and clarify the challenges that should be prioritized for preventing such errors. METHODS: A secondary analysis of the Nationwide Survey on Diagnostic Errors in Pediatric Outpatient Acute Care…

Identifying diagnostic errors in the emergency department using trigger-based strategies.

IMPORTANCE: Diagnostic errors represent a major patient safety concern, with the potential to significantly impact patient outcomes. To address this, various trigger-based strategies have been developed to identify diagnostic errors, aiming to enhance clinical decision-making and improve patient safety. OBJECTIVE: To evaluate the performance of three pre-established triggers (T) in the emergency department (ED) setting and assess their effectiveness in detecting diagnostic errors. DESIGN: Consecutive cohort, retrospective observational design. SETTING: Academic ED with 80 000 annual visits.…

Diagnostic errors related to acute abdominal pain in the emergency department.

OBJECTIVE: Diagnostic errors in the emergency department (ED) are harmful and costly. We reviewed a selected high-risk cohort of patients presenting to the ED with abdominal pain to evaluate for possible diagnostic errors and associated process breakdowns. DESIGN: We conducted a retrospective chart review of ED patients >18 years at an urban academic hospital. A computerised 'trigger' algorithm identified patients possibly at high risk for diagnostic errors to facilitate selective record reviews. The trigger determined patients to be at high risk because they: (1) presented to the ED with abd…
