The measurement of bone-conduction thresholds is an integral part of audiologic evaluation. The relationship between bone-conduction and air-conduction thresholds is the differentiating diagnostic indicator between conductive and sensorineural hearing loss. At the same time, the influence of middle ear and inner ear structures upon the bone-conduction response has been well documented. We present two cases illustrating this influence and attempt to explain the clinical bone-conduction thresholds with operative findings.

Superior semicircular canal dehiscence syndrome is a clinical picture with sound and pressure-induced vertigo, autophony, hearing loss and a lowered bone conduction threshold. It is an important differential diagnosis to otosclerosis and patent eustachian tube. Diagnostic investigations include audiometric testing, nystagmus provocation, computed tomography and vestibular evoked myogenic potentials. Surgical repair of the dehiscence represents a curative therapeutic option. We describe the disease symptoms on the basis of a typical case.

Studies have been made on thresholds of perception of amplitude-modulated focused ultrasound (AMFU) during modulation by 125-8,000 Hz in 6 healthy subjects, 100 patients with otosclerosis before and after operation, and 13 subjects with other disturbances in hearing. Self-stimulation thresholds were determined in rats by means of a modification of the psychophysical method of limits. Reinforcement values were determined after the administration of d-amphetamine alone, naloxone alone, and naloxone administered concurrently with d-amphetamine. d-Amphetamine yielded dose-related decreases in the threshold (0.25--2.00 mg/kg IP), while naloxone alone (2.0--16 mg/kg IP) caused no consistent changes.
