Surgical treatment of perforated diverticulitis of the sigmoid colon.

A retrospective review of 1353 cases of acute perforated sigmoid diverticulitis treated surgically demonstrates that those operations that resect or exteriorize the perforated segment at the first operation are associated with a lower operative mortality rate than procedures that fail to remove the perforated segment at the initial operation. These results are true for both diffuse peritonitis and localized abscess.

Acute perforations of the sigmoid colon secondary to diverticulitis.

Diverticulitis is a complex disease and demands careful cooperation between physician and surgeons, because although it is a benign disease, the presence of complications makes it potentially lethal. For successful management, knowledge of the treatment in past decades should be integrated with current surgical technics. A retrospective review of forty-one patients with perforated diverticulitis revealed a significant decrease in morbidity and hospital stay for the group of patients undergoing the Hartmann procedure versus the group undergoing the classic three stage approach. In addition, the

Right colon, sigmoid colon, and transverse colon diverticulitis in the same patient: report of a case.

Although sigmoid colon diverticulitis is frequently seen, right colon and transverse colon diverticulitis remain rare forms of the disease. This case report examined the disease course of a 46-year-old female who first presented to our institution in 1990 with perforated right-sided diverticulitis. During the next 11 years, she developed sigmoid colon diverticulitis and then transverse colon diverticulitis. The right and sigmoid colon diverticulitis were treated with surgery and the transverse colon diverticulitis was managed conservatively. This is the first reported case of a single patient

Transverse colon diverticulitis mimicking acute appendicitis.

Diverticulitis in a solitary transverse colon diverticulum is uncommon, with only a handful of cases documented in the literature. There are various clinical manifestations of the disease, which make clinical and radiological diagnosis rather challenging. Herein, we present a case of a premenopausal female patient in her late 40s who presented to the emergency department, complaining of right lower quadrant abdominal pain, nausea, anorexia and fever. Following clinical, biochemical and radiological tests, the patient was prepared for surgical operation, with the presumed diagnosis of acute app

[Ultrasonography in acute diverticulitis of the colon: semiologic features].

To assess the ultrasonographic (US) patterns of acute colonic diverticulitis, we prospectively examined 21 patients. US was performed in all of them, CT in 13/21, contrast enema in 18/21 and endoscopy in 2/21 patients. US follow-up was carried out in 15/17 patients managed with conservative treatment. To analyze the US patterns of simple diverticula, we carried out an in vitro study of two surgical specimens with simple colonic diverticulosis. Wall thickening was seen in 21/21 cases, diverticula in 19/21, changes in pericolic fat in 11/21, local tenderness on gradual compression in 19/21 and c

Malignant fibrous histiocytoma of the colon associated with diverticulitis.

We report the second case of a malignant fibrous histiocytoma of the large bowel. The neoplasm constituted an incidental finding during the diagnostic work-up for diverticulitis of the sigmoid colon. There was a massive abdominal recurrence of the tumor four months after colectomy, and the patient died nine months after the initial operation. The literature on fibroblastic sarcomas of the colon is also reviewed.

Functional exteriorized colon for perforations due to diverticulitis.

Three patients had normal bowel function with the sigmoid colon exteriorized through an abdominal wound for 39, 42 and 72 days. Colostomy was thus avoided during the time the patient was recovering from peritonitis and inflammation in the involved segment of colon. Ready access was provided to the distal limb of the colon at second operation.

[Colon diverticulosis and complicating diverticulitis (author's transl)].

Dietary habits in the civilised countries of the West with foods low in bulk and cellulose are blamed as causes of the increase in colon diverticulosis and the complicating diverticulitis in recent times. Of all sections of the colon, the sigmoid is most frequently involved, older patients above the 5th decade being principally affected. While uncomplicated diverticulosis of the colon usually remains symptomless, complicated diverticulitis often shows very uncharacteristic symptoms. For prophylaxis and the treatment of slight complaints, conservative treatment with administration of bulky food

[Acute left-sided colonic diverticulitis: diagnosis and surgical indications after successful conservative therapy of first time acute diverticulitis].

PURPOSE: To compare the performances of computed tomography (CT) and water soluble contrast enema in the diagnosis and evaluation of the severity of acute left colonic diverticulitis, and to propose indications for elective colectomy after a first episode of diverticulitis successfully treated medically. PATIENTS AND METHODS: Prospective non-randomized study (1986-1997) of 542 patients (290 women and 252 men with a median age of 64 years) whose final diagnosis was confirmed either histologically or radiologically. 465 patients (86%) had a CT, 439 (81%) had a water soluble contrast enema, and

Parameters of Body Composition Predict Clinical Course in Acute Colonic Diverticulitis.

BACKGROUND: Acute colonic diverticulitis (ACD) is the most common complication of colonic diverticulosis. Body composition, that is, proportion, distribution and quality of muscle and adipose tissues may play a relevant role in ACD. Previously, only few reports with small number of patients analysed the prognostic role of body composition in ACD. Our purpose was to analyse associations between the occurrence of complications and parameters of body composition in patients with ACD in a large sample. METHODS: This retrospective study included 646 patients with ACD. The duration of hospital stay

Laparoscopic Lavage in Complicated Diverticulitis with Colonic Perforation, Always Be Closing?

Laparoscopic lavage is seen as an acceptable alternative to colonic resection in selected patients with acute diverticulitis with purulent peritonitis. There is no consensus on what surgical technique should be used when performing this procedure. This case series describes the disease course of 3 patients with acute diverticulitis with purulent peritonitis treated with laparoscopic lavage and direct suturing of a colonic perforation. All patients (38- and 71-year-old males and a 44-year-old female) were seen in the emergency department due to acute lower abdominal pain. Clinical examination a
