The authors studied 1,342 cases of adenocarcinoma of the stomach identified by a population-based cancer registry in Los Angeles County, California. The cases were males aged 20-64 years first diagnosed between 1972 and 1982. To determine whether exposure to occupational dust increased the risk of developing stomach cancer, occupational titles were rated for the likelihood of exposure to various kinds of dust. Changes in respiratory function (FEV1) and polymorphonuclear neutrophils (PMN) on nasal epithelium were studied in 68 students and 39 cotton mill workers in an experimental cardroom. The exposure was characterised by the vertical elutriator dust and endotoxin levels. A dose related decrease was found for FEV1 which was more pronounced in smoking workers. Biochemical alterations in guinea pig lungs caused by hematite dust were followed at 150 days after intratracheal administration of the dust. One hundred and thirty-two patients known or suspected of allergy to house dust were skin-tested with pure extracts of four varieties of dust mites. The results show that both Dermatophagoides culinae and D. pteronyssinus probably played important parts in sensitizing susceptible people, as did Glycyphagus destructor and G. domesticus in some cases. In some patients these results enabled successful desensitization against the dust to be carried out, and for corticosteroid therapy to be stopped. Aqueous extracts prepared from dust collected in the card-rooms of several cotton mills have been prepared and found to contain activity which contracts the smooth muscle of guinea-pig ileum, guinea-pig trachea, rat stomach strip, and rat duodenum. The extracts contained an unknown contractor substance which was dialysable, resistant to boiling for one hour, and not destroyed by the action of proteolytic enzymes.
