Electrolyte disturbances are frequently encountered in pediatric patients, not only in those with critical illness. They can manifest as lethargy, seizures, vomiting and cardiac arrhythmias. In many cases, electrolyte abnormalities are secondary to other underlying conditions, therefore clinical signs and symptoms can be predominated by the primary disease and not by the electrolyte imbalance. Clinical and laboratory evaluation must consist of a detailed history including any drug treatment, evaluation of volume status, acid-base balance, electrolytes as well as plasma and urine osmolality. Be…

There are five problem schemas presented in this article that indicate potential contradictions in therapeutic goals: (1) shock and edema presenting upon premature birth; (2) the hyperosmolar state, problematic in patients less than 750 g birth weight; (3) the respiratory distress syndrome and respiratory failure, often complicated by patent ductus arteriosus; (4) bronchopulmonary dysplasia, resulting from prematurity and mechanical ventilation; and (5) late onset of hyponatremia, sometimes accompanied by growth failure in the recovering premature. These five problems considered together compr

It is well known that renal dysfunction is associated with several types of electrolyte disorders. On the other hand, renal manifestations have been attributed to electrolyte disorders. Hypokalemia is the most frequent electrolyte abnormality encountered in clinical practice. The main cause of hypokalemia is due to abuse of laxatives and diuretics or to anorexia nervosa. Hypercalcemia is another major electrolyte abnormality, associated with numerous renal manifestations. Renal tubules damages and chronic interstitial nephritis are characteristic pathological findings in prolonged electrolyte

Established essential hypertension is characterized by normal equilibrium between the intake and renal excretion of sodium. Urinary sodium excretion is interrelated with that of other ions, such as potassium and calcium, and that the response of blood pressure to salt ingestion can be conditioned by the simultaneous intake of varying levels of those ions. The authors address three aspects: the correlations between urinary excretion of sodium and calcium and sodium and potassium in a population of untreated essential hypertensive persons, the response of blood pressure during the escape induced

Therefore, when treating congestive heart failure, one must consider how to prevent depletion of electrolytes or how to replete potassium and magnesium in deficiency states.

An overview of the principles of the diagnosis and management of severe electrolyte and acid-base problems in the critically ill is presented, stressing a pathophysiologic approach to an understanding of the problem in the individual patient.

It is very rare for disturbances of water balance to occur as isolated phenomena. More commonly, mixed electrolyte abnormalities present. However, to permit the logical application of therapy an understanding of basic water balance in health and disease is essential.

Other hormonal control includes antidiuretic hormone producing dilution of extracellular electrolytes and augmented peripheral resistance. A recently identified natriuretic factor isolated from cardiac atria appears to be a potent diuretic with actions similar to that of frusemide (furosemide). Other electrolytes have received closer scrutiny. Chlor …

Drugs can interfere with the normal intake, elimination, regulation and total body distribution of electrolytes. These drug-induced abnormalities may be dramatic and life threatening, posing diagnostic and management problems to the physician who is not familiar with them. Serum potassium concentrations can be altered as potassium shifts between the tissue and plasma compartments secondary to drug actions. In addition, drugs have been shown to interfere with the normal physiological functioning of the kidney with respect to potassium homeostasis, as well as the renin-aldosterone axis. The regu…

Severe hypokalemia (2.6 mEq/l), hypomagnesemia (0.6 mg/dl), mild hypercalcemia (10.9 mg/dl), and secondary hyperaldosteronism developed in a patient receiving viomycin for pulmonary tuberculosis. Reversible renal wasting of both potassium (K+) and magnesium (Mg++) was documented. Viomycin administered to 40 rats resulted in severe damage to the proximal tubule and mild damage to the distal tubule. THe case report and experimental data suggest viomycin induces proximal tubule dysfunction that results in renal wasting of sodium, K+ and Mg++ and secondary hyperaldosteronism. Hypercalcemia, not pr

The author presents the results of parallel studies of potassium, sodium and calcium in the blood plasma and the 24-hour urine of 146 patients suffering from toxic goiter of different severity (mild course in 43 patients, of moderate severity in 59, and severe in 44). Investigations were carried out before and after the treatment with thyrotoxic preparation and in some of the patients (24) after strumectomy. Before the treatment patients with mild and moderately severe forms of the disease showed a tendency to increase of potassium and calcium and a fall of sodium level in the blood. Patients

Glomerular filtration rate and renal plasma flow may be normal, reduced or increased in cirrhosis. The mechanism of departures from normal is not known. Other renal functional changes in cirrhosis include avid sodium reabsorption, impaired concentrating and diluting abilities, and partial renal tubular acidosis. Fluid and electrolyte disorders are common. Sodium retention with edema and ascites should generally be treated conservatively because they tend to disappear as the liver heals and because forced diuresis has hazards. The indications for diuretics are (1) incipient or overt atelectasis
