Hurricane-Resilient Electronic Medical Records: A Distributed Architecture for Small Island Developing States.

Hurricane Dorian (2019, GLIDE TC-2019-000099-BHS) permanently destroyed decades of patient records in Grand Bahama, The Bahamas, exposing critical vulnerabilities in centralized health care information systems. This study proposes a distributed electronic medical record (EMR) architecture to maintain patient data availability during and after catastrophic hurricanes in small island states. The architecture connects The Bahamas' approximately 75 community health facilities through regional replication hubs, each maintaining a full local database replica with bidirectional synchronization, enabl

Electronic health records reveal variations in the use of blood units by hour and medical specialty.

BACKGROUND AND OBJECTIVES: Efficient blood supply chain requires accurate demand estimates. Blood demand is created by clinicians making transfusion decisions based on patient status. To better understand the use of blood units, we tracked their use hourly and across a large hospital organization. MATERIALS AND METHODS: We analysed blood use in adult patients over 2021-2022 at HUS Helsinki University Hospital, serving a population of 1.7 million and consuming a third of blood units used in Finland. We utilized electronic health records (EHR) to map transfusions to patient demographics, diagno

Feasibility of assessing symptoms associated with ovarian cancer using an electronic medical intake questionnaire.

OBJECTIVE: Symptoms occur in up to 75% of patients with early-stage ovarian cancer; as such, using a symptom inventory (SI) to stratify which patients should receive screening for ovarian cancer is an attractive approach to mitigate false-positives. We report on the feasibility and results of a prospective four-question SI that assessed the following symptoms: abdominal pain, bloating, early satiety, and urinary complaints. Frequency and duration of symptoms were not assessed. METHODS: A SI was added to the standard pre-appointment check-in process via the electronic medical record's (EMR) pa

Machine learning-based prediction of gastroesophageal junction cancer using electronic medical records.

Discriminating whether esophageal-related symptoms result from gastroesophageal junction cancer (GEJC) is challenging in clinical practice. This study aimed to develop and validate a tool to predict the likelihood of GEJC in patients with esophageal-related symptoms. The electronic medical record system was accessed to identify patients diagnosed with GEJC or gastroesophageal reflux disease (GERD) at our hospital between 2009 and 2023. Predictive variables included demographic characteristics, symptoms, and laboratory results. After propensity score matching, significant features of GEJC were

COVID-19 waves in an urban setting 2020-2022: an electronic medical record analysis.

BACKGROUND: Global and national surveillance efforts have tracked COVID-19 incidence and clinical outcomes, but few studies have compared comorbid conditions and clinical outcomes across each wave of the pandemic. We analyzed data from the COVID-19 registry of a large urban healthcare system to determine the associations between presenting comorbidities and clinical outcomes during the pandemic. METHODS: We analyzed registry data for all inpatients and outpatients with COVID-19 from March 2020 through September 2022 (N = 44,499). Clinical outcomes were death, hospitalization, and intensive ca

Predicting preterm birth using electronic medical records from multiple prenatal visits.

This study aimed to predict preterm birth in nulliparous women using machine learning and easily accessible variables from prenatal visits. Elastic net regularized logistic regression models were developed and evaluated using 5-fold cross-validation on data from 8,830 women in the Nulliparous Pregnancy Outcomes Study: New Mothers-to-Be (nuMoM2b) dataset at three prenatal visits: 6 0 - 13 6 , 16 0 - 21 6 , and 22 0 - 29 6 weeks of gestational age (GA). The models' performance, assessed using Area Under the Curve (AUC), sensitivity, specificity, and accuracy, consistently improved with the

Use of the electronic medical record to screen for high-risk geriatric patients in the emergency department.

Older adults with multimorbidities have the highest rate of emergency department (ED) usage. These patients are typically on numerous medications, may have underlying dementia, and often present with falls and delirium. Identifying these high-risk older adults for possible intervention is challenging in the ED setting since available screening methods are manual and resource-intensive. The objective is to study the electronic medical record (EMR) use for identifying high-risk older adults in ED. This feasibility study is conducted in an academic ED with 67 000 total and 24% geriatric (age ≥ 65

Antimicrobial usage surveys using electronic medical records in cattle practice in Gifu Prefecture.

As the majority of antimicrobial treatments for cattle in Japan are prescribed by veterinarians, medical record information can be useful in clarifying the amount and purpose of antimicrobial use. In this study, we examined their amount and purpose in cattle practices in Gifu Prefecture. In cattle, approximately 85% of the antimicrobials are used for the treatment of gastrointestinal (50.4%) and respiratory diseases (34.4%). The main antimicrobials were sulfonamides (27.1 kg, 49.2%), followed by amphenicols (11.9 kg, 21.7%). As for second-line antimicrobials for veterinary treatment, fluoroqui

Electronic medical record based tools: Not a panacea in the diagnosis of coin-shaped foreign bodies.

INTRODUCTION: Electronic medical record-based tools have been demonstrated to improve timeliness of x-ray order placement in patients presenting to the emergency department (ED) with coin-shaped foreign body ingestion. Similar efforts directed towards downstream processes are necessary to expedite diagnosis of an esophageal button battery. We predicted that improvement tools such as electronic medical record-based alerts and process standardization could be utilized to expedite x-ray completion. METHODS: Using Plan, Do, Study, Act methodology, iterative interventions were implemented. In July

Harnessing the epic electronic medical record to track indwelling ureteral stents in a pediatric population.

INTRODUCTION: Retention of indwelling ureteral stents due to loss to follow-up can result in significant harm to patients, often requiring multiple trips to the operating room. Despite widespread use of electronic medical records, there are few standardized options for urologists to track ureteral stents and no data on the rate of retained stents in a pediatric population. OBJECTIVE: This pilot quality improvement project aims to: 1) develop a simple process to track indwelling ureteral stents using the Epic electronic medical record and 2) determine the incidence of forgotten stents in a ped

Validation of algorithms for identifying outpatient infections in MS patients using electronic medical records.

Background Our multiple sclerosis (MS) stakeholder groups expressed concerns about whether MS disease-modifying therapies (DMTs) increase the risk of specific outpatient infections. Validated methods for identifying the risk of these selected outpatient infections in the general population either do not exist, exclude the clinically important possibility of recurrent infections, or are inaccurate, largely because existing studies relied primarily on International Classification of Diseases (ICD) codes to identify infectious outcomes. Additionally, no studies have validated methods among the MS

The relationship between stress and vitiligo: Evaluating perceived stress and electronic medical record data.

Vitiligo is a T-cell mediated skin disorder characterized by progressive loss of skin color. In individuals genetically predisposed to the disease, various triggers contribute to the initiation of vitiligo. Precipitating factors can stress the skin, leading to T-cell activation and recruitment. Though hereditary factors are implicated in the pathogenesis of vitiligo, it is unknown whether precipitating, stressful events play a role in vitiligo. To understand this, we utilized a validated perceived stress scale (PSS) to measure this parameter in vitiligo patients compared to persons without vit

The use of drug calendars for the diagnosis of cutaneous drug eruptions in the age of electronic medical records.

A morbilliform drug eruption is the most common condition leading to a dermatology consultation for a patient in the hospital. Timing is an important diagnostic tool since the onset of a skin rash usually takes place within days-to-weeks of the start of the implicated drug. A comprehensive, thorough, and reliable drug history by the clinician is essential. Therefore, to assist in the task of determining the causative medication of a new skin rash in a hospitalized patient, the creation of a drug calendar is recommended. The development of an electronic version of the drug calendar offers sever
