Amniotic Fluid Embolism.

Amniotic fluid embolism remains one of the most devastating conditions in obstetric practice with an incidence of approximately 1 in 40,000 deliveries and a reported mortality rate ranging from 20% to 60%. The pathophysiology involves an abnormal maternal response to fetal tissue exposure associated with breaches of the maternal-fetal physiologic barrier during parturition. This response and its subsequent injury involve activation of proinflammatory mediators similar to that seen with the classic systemic inflammatory response syndrome. Maternal treatment is primarily supportive, whereas prom…

Amniotic fluid embolism.

Amniotic fluid embolism (AFE) is one of the catastrophic complications of pregnancy in which amniotic fluid, fetal cells, hair, or other debris enters into the maternal pulmonary circulation, causing cardiovascular collapse. Etiology largely remains unknown, but may occur in healthy women during labour, during cesarean section, after abnormal vaginal delivery, or during the second trimester of pregnancy. It may also occur up to 48 hours post-delivery. It can also occur during abortion, after abdominal trauma, and during amnio-infusion. The pathophysiology of AFE is not completely understood. P…

Paradoxical embolism interrupted.

A 41 year-old African-American male presented with syncope preceded by shortness of breath at outside facility and transferred to us for management of extensive pulmonary embolism with unstable vital signs. Electrocardiogram showed sinus tachycardia with S1Q3T3 pattern. A transthoracic echocardiogram revealed a freely mobile strand like mass in the left atrium. A transoesophageal echocardiogram showed a very large freely mobile thrombus extending from a patent foramen ovale in to the left atrium. He underwent emergent surgery for the extraction of clot followed by thromboembolectomy from both …

Amniotic fluid embolism.

Amniotic fluid embolism (AFE) is an unpredictable and as-of-yet unpreventable complication of maternity. With its low incidence it is unlikely that any given practitioner will be confronted with a case of AFE. However, this rare occurrence carries a high probability of serious sequelae including cardiac arrest, ARDS, coagulopathy with massive hemorrhage, encephalopathy, seizures, and both maternal and infant mortality. In this review the current state of medical knowledge about AFE is outlined including its incidence, risk factors, diagnosis, pathophysiology, and clinical manifestations. Speci…

Pulmonary tumor embolism.

A 36-year-old woman with breast cancer was admitted with shortness of breath. A computed tomography angiogram was obtained and was negative for pulmonary embolism. She quickly developed hypoxemic respiratory failure and was transferred to the intensive care unit. She appeared to be improving when she suffered sudden cardiac death. Autopsy showed extensive lymphatic and vascular tumor emboli, which were the immediate cause of death. Tumor emboli are rarely diagnosed before death but seem to be more common than realized. It is important to recognize this entity, because there have been some case…

Perioperative pulmonary embolism.

Pulmonary thromboembolism (PTE) is a perioperative complication that requires prompt diagnosis and treatment to minimize mortality. Detection of deep vein thrombosis (DVT) suggests the presence of PTE. The clinical presentation of PTE is mainly hemodynamic and gas exchange abnormalities. Diagnostic tools include ventilation/perfusion scan, pulmonary angiography, spiral CT, and echocardiography. Therapeutic options include hemodynamic support with inotropics, anticoagulation, systemic thrombolysis, surgical embolectomy and an inferior vena cava filter. DVT prophylaxis should be considered in al…

Amniotic fluid embolism.

Amniotic fluid embolism is one of the most catastrophic complications of pregnancy. First described in 1941, the condition is exceedingly rare and the exact pathophysiology is still unknown. The etiology was thought to be embolic in nature, but more recent evidence suggests an immunologic basis. Common presenting symptoms include dyspnea, nonreassuring fetal status, hypotension, seizures, and disseminated intravascular coagulation. Early recognition of amniotic fluid embolism is critical to a successful outcome. However, despite intensive resuscitation, outcomes are frequently poor for both in…

Fat embolism syndrome.

OBJECTIVES: To assess the incidence and risk factors for fat embolism syndrome. MATERIALS AND METHODS: Data from the National Hospital Discharge Survey (NHDS) were analyzed using International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) codes. RESULTS: From 1979 through 2005 among 928,324,000 patients discharged from short-stay hospitals in the United States, 41,000 (0.004%) had fat embolism syndrome. Among 21,538,000 patients with an isolated fracture of the femur (any site), tibia, fibula, pelvis, ribs, humerus, radius, or ulna, 25,000 (0.12%) developed fat…

[Spinal fibrocartilaginous embolism].

Fibrocartilaginous embolism (FCE) of the spinal cord is a rare cause of ischemic myelopathy. We describe a 13-year-old patient with probable post-traumatic FCE. History of disease, clinical presentation, diagnostic workup, and disease course are outlined. Diagnosing FCE in the living often seems to be merely conjectural. Therefore, vertebral edema adjacent to the level of the spinal lesion is a possibly valuable diagnostic hint of spinal ischemia. Furthermore, exclusion of systemic embolism and other etiologies of myelopathy is necessary to corroborate the diagnosis of FCE. The postulated path…

[Paradoxical brain embolism].

Although paradoxical brain embolism result in the right-to-left shunt (RLS) is very important etiology of ischemic stroke especially in young adults, the detailed characteristics of them in Japan remain unknown. In collected clinical data for 7,245 acute stroke patients, patent foramen ovale (PFO) were more frequent in the young patients group (n = 1,584) than in the non-young patients group (n = 5,661) (0.7% vs. 1.2%, p = 0. 07). The frequency of PFO was rare than that of previously reported. Suspected reason is that the limited patients were underwent transesophageal echocardiography (TEE). …

Amniotic fluid embolism.

Amniotic fluid embolism (AFE) (also known as anaphylactoid syndrome of pregnancy)is a catastrophic condition that occurs during pregnancy or shortly after delivery. It is found throughout the world in developed and undeveloped countries and occurs at an incidence of between 1 in 80000 live births. In the United States, AFE occurs in 1 in 20000 to 80000 deliveries.

[Amniotic fluid embolism].

Amniotic fluid embolism is a rare obstetric emergency in which amniotic fluid, fetal cells, hair, or other debris enters the maternal circulation. Amniotic fluid embolism is an incompletely understood obstetric complication unique to pregnancy presenting with the acute onset of hypoxia, hypotension and severe coagulopathy. Up to 86% of the cases are fatal, maternal death occurs within the first hour of symptom onset in 50% of the patients. It cannot be predicted nor prevented. The epidemiology of amniotic fluid embolism, frequency, pathophysiology, clinical presentation, histologic findings, d…
