The impact of congenital cardiovascular malformations on the assessment and surgical management of infants with cleft lip and/or palate.

OBJECTIVE: The purpose of this study was to assess the cardiac evaluation of cleft lip and/or palate patients, characterize their cardiovascular malformations, and determine the impact of cardiovascular malformations on surgical management. DESIGN: A single-institution retrospective study of 329 consecutive cleft patients was performed. Cardiovascular malformations were categorized according to involvement of cardiac septa, vasculature, and valves. Their impact on the need for cardiac surgery, timing of cleft repair, need for subacute bacterial endocarditis (SBE) prophylaxis, and the perioper

A rare native mitral valve endocarditis successfully treated after surgical correction.

Mycobacterium abscessus and Kocuria species are rare causes of infections in humans. Endocarditis by these agents has been reported in only 11 cases. M. abscessus is a particularly resistant organism and treatment requires the association of antibiotics for a prolonged period of time. We report a case of native mitral valve bacterial endocarditis due to M. abscessus and Kocuria species in a 48-year-old man with a history of intravenous drug use. The case was complicated by a perforation of the posterior mitral valve leaflet, leading to surgical mitral valve replacement. Cultures from the blood

Angina micturition.

A case history of a 46-year-old Negro male was presented with rheumatic aortic insufficiency, angina pectoris, and possible subacute bacterial endocarditis. While hospitalized, the patient developed angina pectoris associated with micturition. This was not accompanied by a change in blood pressure, pulse, or increase in respiratory rate. The striking similarity to a description of a case approximately 300 years ago is pointed out and possible mechanisms of this association of angina and micturition are considered.

Infected cerebral hemorrhage by ruptured infectious anterior communicating artery giant aneurysm in a patient with T cell acute lymphoblastic leukemia. Case report.

Although the majority of infectious (bacterial) intracranial aneurysms occur in patients with subacute bacterial endocarditis, septic cerebral embolization and infectious intracranial aneurysm formation can also occur in immunocompromised patients. We report the case of ruptured bacterial aneurysm occurring in a 19 years old man immunocompromised by vigorously treated acute lymphoblastic leukemia. This case confirms very poor prognosis for infectious aneurysms in immunocompromised patient reported in literature, suggesting that a high index of suspicion needs for early diagnosis and that a con

Successful late non-surgical removal of intracardiac catheter fragment.

A patient was discovered to have a retained intracardiac catheter fragment more than two years after successful aortic valve replacement. During this time the patient had had extensive investigations and several hospital admissions for suspected subacute bacterial endocarditis, as well as for episodes of supraventricular tachycardia. The catheter fragment was successfully removed via a percutaneous catheter approach despite the very long interval that had elapsed since its detachment.

Endocarditis due to Salmonella typhi.

A case of endocarditis caused by Salmonella typhi is reported in a 24-year-old Egyptian woman known to have rheumatic heart disease. The diagnosis was confirmed by positive blood cultures and echocardiogram.

Viridans streptococcal (Streptococcus mitis) biosynthetic aortic prosthetic valve endocarditis (PVE) complicated by complete heart block and paravalvular abscess.

Prosthetic valve endocarditis (PVE) may be classified clinically as early (<60 days) or late (>60 days) post-valve replacement PVE. The pathogens of early versus late PVE differ in type and virulence. Early PVE pathogens are virulent, for example, Pseudomonas aeruginosa and Staphylococcus aureus. Late PVE pathogens resemble those of subacute bacterial endocarditis and are due to relatively avirulent and noninvasive organisms, for example, viridans streptococci. Viridans streptococci vary in their invasiveness and abscess potential. Myocardial abscess and complete heart block are rare complicat

Non-bacterial thrombotic endocarditis leading to the diagnosis of pancreatic adenocarcinoma.

Non-bacterial thrombotic endocarditis (NBTE) is a rare, non-infective endocarditis affecting cardiac structures, often associated with underlying conditions and is frequently overlooked or misdiagnosed. We present the case of a man in his 50s who initially presented with symptoms concerning for myocardial ischaemia, and laboratory and ECG findings raised suspicion for a subacute non-ST-elevation myocardial infarction. Coronary angiography showed coronary sclerosis without significant stenoses and a dilated left ventricle with reduced systolic function. Transthoracic echocardiography revealed ve

Experimental endocarditis in rabbits. 3. Significance of the proteolytic capacity of the infecting strains of Streptococcus faecalis.

Insertion of a polyethylene catheter into the left ventricle of the heart was used for regular establishment of sterile endocarditis, and bacterial endocarditis was established by injection of approximately 10(8) Streptococcus faecalis into the blood stream at the same time as removal of the catheter which had been in place for 3 days. 100 out of 102 rabbits died spontaneously of bacterial endocarditis. Evidence is produced that the host-parasite interaction is influenced by the proteolytic property of S. faecalis in this experimental model. Two distinct types of clinical course are described:

Migraine and the mitral valve prolapse syndrome.

We believe there is a significant association between migraine and the prolapse mitral valve syndrome. Propranolol is the drug of choice in these patients for the treatment and prevention of migraine. Increased platelet aggregability may be the common pathophysiologic mechanism as relates to emboli from the valve and possibly in strokes related to migraine. Recognition of the association of the two syndromes will result in appropriate subacute bacterial endocarditis prophylaxis for patients at risk as well as prevention of improper medication to those patients with migraine who are at risk for

Bacterial endocarditis at Blegdamshospitalet in Copenhagen 1944-1973.

The clinical pattern of 34 cases ob subacute bacterial endocarditis (SBE) and 46 cases of acute bacterial endocarditis (ABE) is outlined. In the SBE group the mortality was 9% and the incidence of major complications during the treatment period was 15% for cerebrovascular accidents, 9% for other systemic or pulmonary emboli and 9% for congestive heart failure indicating valvular damage. In 31 bacteriologically proven cases growth was obtained in 68% of all blood cultures, and in 94% of the cases at least one positive culture was among the first 5 ones drawn. In the ABE group the overall mortal

Infective endocarditis by Bartonella quintana masquerading as antineutrophil cytoplasmic antibody-associated small vessel vasculitis.

The Bartonella species have been recently recognized as important causative agents of culture-negative bacterial endocarditis. Antineutrophil cytoplasmic antibodies (ANCAs) have been associated with the spectrum of idiopathic small vessel vasculitis. However, a variety of infections can result in a false-positive ANCA test, and especially subacute bacterial endocarditis (SBE) with the presence of ANCAs occasionally mimics the clinical manifestations of an ANCA-associated vasculitis such as skin purpura and glomerulonephritis. In contrast, noninfectious endocardial involvement is known to be pa

Fever of unknown origin caused by late-onset rheumatoid arthritis.

Fever of unknown origin (FUO) is always a diagnostic challenge. The causes of FUO are legion and may be due to malignancy, infection, collagen vascular disease, and a variety of other unusual disorders. Currently, malignancies-followed by infectious etiologies-are the most common cause of FUO. We present an elderly female patient with an FUO who was thought to have subacute bacterial endocarditis because of an antecedent history of recent dental work. Subacute bacterial endocarditis was ruled out on the basis of negative cultures and negative transesophageal echocardiography. No evidence for a
