Chronic pancreatitis: how does surgery step in?

Chronic pancreatitis (CP) is a progressive inflammatory disorder of the pancreas characterized by irreversible morphological changes that lead to chronic abdominal pain, exocrine and endocrine insufficiency, and a significant reduction in quality of life. Historically, surgical intervention was considered a last-line option, reserved for patients in whom conservative or minimally invasive therapies had failed. However, growing evidence suggests that early surgical management may provide superior outcomes in terms of pain control and quality of life. The surgical management of CP encompasses a

Surgical treatment in chronic pancreatitis timing and type of procedure.

Pain relief and improvement in the quality of life are of paramount importance for any intervention in chronic pancreatitis. In several trial good results have been published after different drainage procedures and resections. An optimal surgical intervention should manage mainly the intractable pain, resolve the complications of the adjacent organs and achieve the drainage of the main pancreatic duct. An optimal procedure should guarantee a low relapse rate, preserve a maximum of endocrine and exocrine function, and most importantly, restore quality of life. Thus an ideal operation should rep

Surgical treatment of endocrine exophthalmos by removal of orbital fat: clinical experience.

PURPOSE: To report a series of 73 patients with endocrine exophthalmos treated by removal of orbital fat using the transpalpebral approach during the period 1989 to 1999. METHODS: The operation was performed according to the technique described by Olivari. Aesthetic analysis was done preoperatively and postoperatively (more than 6 months after surgery). The number of complications was also observed. RESULTS: The average volume of resected fat was approximately 7.6 mL per orbit. No major complication was observed. In 9 patients with epiphora, all improved. One patient developed postoperative

Endocrine surgery during COVID-19 pandemic: do we need an update of indications in Italy?

The ongoing spread of the coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) poses a significant threat to global health. As the coronavirus outbreak began spreading, hospitals were forced to relocate resources to treat the growing number of COVID-19 patients. As a consequence, doctors across the country canceled tens of thousands of nonurgent surgeries. However, recognizing that the COVID-19 situation may be highly variable and fluid in different communities across the country, elective surgery could be still allowed in some centers for

Scintigraphic techniques in primary hyperparathyroidism: from pre-operative localisation to intra-operative imaging.

INTRODUCTION: Primary hyperparathyroidism (PHPT) is an increasingly diagnosed disease worldwide. In most cases, PHPT is related to the presence of a solitary parathyroid adenoma (PA). Fifty percent or more of newly diagnosed PHPT patients are asymptomatic, and there is debate among endocrinologists and endocrine surgeons about whether or not such patients should be treated. LOCALIZATION: Usually, in a PHPT patient with a solitary PA that is well localised pre-operatively, a parathyroidectomy with limited or minimally invasive neck exploration is offered. The diffusion of minimally invasive ne

Association between genotype and long-term risk of reintervention and distant metastases in patients with multiple endocrine neoplasia type 1-associated duodenopancreatic neuroendocrine tumors.

BACKGROUND: Duodenal and pancreatic neuroendocrine tumors are the leading cause of mortality in multiple endocrine neoplasia type 1, due to distant metastasis. Although surgery is the cornerstone of management, patients may require reintervention for disease progression or hormone-related morbidity. The clinical and genetic determinants of reintervention and long-term oncologic outcomes after index duodenal and pancreatic neuroendocrine tumor surgery in multiple endocrine neoplasia type 1 remain unclear. METHODS: We retrospectively analyzed patients with genetically confirmed multiple endocri

Metabolic and bariatric surgery in MAFLD: a meta-analysis of endocrine outcomes, fibrosis remission, and postoperative complications.

Metabolic dysfunction-associated fatty liver disease (MAFLD) is increasingly common among candidates for metabolic and bariatric surgery, yet clinical thresholds defining when surgery is beneficial or contraindicated remain unclear. This systematic review and meta-analysis synthesized evidence from 29 studies including 71,904 patients with biopsy-confirmed or elastography-confirmed MAFLD undergoing sleeve gastrectomy, Roux-en-Y gastric bypass, or other metabolic procedures. Bariatric surgery was associated with marked hepatic and endocrine improvement, with pooled remission rates of 70% for st

Pancreas transplantation.

Since the introduction of pancreas transplantation more than 40 years ago, efforts to develop more minimally invasive techniques for endocrine replacement therapy have been in progress, yet this surgical procedure still remains the treatment of choice for diabetic patients with end-stage renal failure. Many improvements have been made in the surgical techniques and immunosuppressive regimens, both of which have contributed to an increasing number of indications for pancreas transplantation. This operation can be justified on the basis that patients replace daily injections of insulin with an i

Surgical Approaches to Chronic Pancreatitis: Indications and Techniques.

There are a number of surgical strategies for the treatment of chronic pancreatitis. The optimal intervention should provide effective pain relief, improve/maintain quality of life, preserve exocrine and endocrine function, and manage local complications. Pancreaticoduodenectomy was once the standard operation for patients with chronic pancreatitis; however, other procedures such as the duodenum-preserving pancreatic head resections and its variants have been introduced with good long-term results. Pancreatic duct drainage via a lateral pancreaticojejunostomy continues to be effective in ameli

An update on the status of nerve monitoring for thyroid/parathyroid surgery.

PURPOSE OF REVIEW: Recurrent laryngeal nerve (RLN) injury is one of the most common and serious complications associated with thyroid and parathyroid surgery. Although routine visual identification of the RLN is considered the current standard of care, the role of intraoperative neuromonitoring (IONM) of the RLN is more controversial. RECENT FINDINGS: Despite initial enthusiasm that IONM might substantially reduce the rate of RLN injury, most studies failed to show a significant difference in the rate of RLN injury when the use of IONM was compared with visualization of the RLN alone. However

Surgery for a gastroenteropancreatic neuroendocrine tumor (GEPNET) in multiple endocrine neoplasia type 1.

Multiple endocrine neoplasia type 1 (MEN1) is an autosomal dominantly inherited endocrine tumor syndrome characterized by tumor development in various endocrine organs such as the parathyroid, endocrine pancreas, anterior pituitary and adrenal cortex. The first extensive database for Asian patients with MEN1 was established by the MEN Consortium of Japan. Although the clinical features of Japanese patients with MEN1 are similar to those from Western countries, there are several characteristic differences between them. In particular, gastroenteropancreatic neuroendocrine tumor (GEPNET) is seen

[Pancreatic apudoma].

Endocrine tumors of the pancreas apparently develop from a premature stem-cell, possibly originating from the ectodermal neural crest. These tumors belong to the APUD-cell system, which is identical with the "Helle-Zellen-system". Following a description of the various forms of appearance there is a discussion of the different syndromes. The present-day differentiated diagnostic procedures are described. Finally, details and problems of treatment, especially regarding surgical intervention, are discussed. Cases from the author's own clinic are considered.

Genetic testing in endocrine surgery: Opportunities for precision surgery.

Recent innovations in molecular and genetic diagnostic techniques have led to rapid advances in genomic medicine and their application to the clinic. The identification and classification of various genetic associations, syndromes, and susceptibility genes in endocrine surgical disorders are increasingly relevant to patient care. Hereditary endocrine disorders represent a significant proportion of disease encountered by endocrine surgeons. Hence, genetic testing has emerged as an important adjunct for the diagnosis and management of patients with endocrine surgical disorders. This article summ

Cinacalcet as alternative treatment for primary hyperparathyroidism: achievements and prospects.

Primary hyperparathyroidism (pHPT), which most frequently occurs asymptomatically, is a common endocrine disease associated with increased morbidity and mortality. The newly introduced management guidelines as well as the recent availability of the first calcimimetic offer a highly promising therapeutic option for patients with pHPT. Cinacalcet, the first available calcimimetic, increases the sensitivity of the calcium-sensing receptor (CaR) to circulating serum calcium, thereby safely reducing serum calcium and PTH concentrations in patients with mild-to-moderate pHPT, intractable disease, an

Metastatic pheochromocytoma and paraganglioma: Management of endocrine manifestations, surgery and ablative procedures, and systemic therapies.

Metastatic pheochromocytomas and paragangliomas (MPPGs) are rare neuroendocrine tumors. Most patients present with advanced disease that is associated with manifestations of catecholamine release. Surgical resection of the primary tumor and ablative therapies of metastases-whenever possible-may improve clinical outcomes and, perhaps, lengthen the patient's overall survival. Significant steps in understanding the genetic alterations linked to MPPGs and scientific progress made on cancers that share a similar pathogenesis are leading to the recognition of potential systemic therapeutic options.

Perioperative considerations in diabetic patients.

Diabetes mellitus is an increasingly common disease that affects people of all ages, resulting in significant morbidity and mortality. Diabetic patients require more frequent hospitalization, have greater lengths of stay, and cost more to manage than non-diabetics. The major risk factors for diabetics undergoing surgery are the end-organ diseases associated with diabetes: cardiovascular disease, autonomic neuropathy and immune deficiency. Physicians should pay extra attention to preoperative and preprocedure evaluation and treatment of these diseases to ensure optimal perioperative management.
