OBJECTIVE: To investigate a method of quantification of the endolymphatic hydrops in experimental animal model.
METHODS: Thirty guinea pigs were divided into three groups at random. In control group, there were ten guinea pigs without operation on both ears. Endolymphatic hydrops was induced by endolymphatic sac obliteration through extradural posterior cranial fossa approach in right ear, including 4-week postoperative group(n = 10) and 8-week postoperative group(n = 10). The area of scala vestibuli (SV) and scala media(SM) of each turn on both cochlear midmodiolar sections was measured, res

We undertook a study to assess whether patients who had both spontaneous vertigo and headache would respond to treatment for endolymphatic hydrops rather than treatment for migraine. We also attempted to discover if there were any individual characteristics that might predict which patients with an equivocal history and symptoms would be more likely to have either of the two conditions as opposed to the other. All patients were initially treated for endolymphatic hydrops with sodium restriction and increased water intake. Response to treatment was assessed by three tools: the Dizziness Handica

In order to study the characteristics of pediatric patients (< or = 15 years at onset) with endolymphatic hydrops disorders, we selected 5 patients with Ménière's disease (MD) and 3 patients with delayed endolymphatic hydrops (DEH) from 7418 patients who had visited the Neurotological Clinic of the Toyama Medical and Pharmaceutical University Hospital between 1979 and 1999. Clinical features were characterized as follows: (i) pediatric MD patients were rare, comprising approximately 1.5% of all the 337 MD patients in the sample; (ii) a fluctuating hearing loss, strong positive response in glyc

Low-frequency masking is a new method for the diagnosis of endolymphatic hydrops. A short acoustic stimulus and a low-frequency masker tone are applied to the same ear in an adjustable phase relationship. We recorded phase-dependent masked thresholds from normal-hearing subjects, and patients with Ménière's disease and sensory hearing loss without vertigo. In normal hearing, there is a mean maximal difference in masking (modulation depth) of 28 dB between the phase delays of 0 degree and 270 degrees. In patients with sensory hearing loss without vertigo, modulation depth is reduced due to recr

The clinical entity of delayed endolymphatic hydrops was first defined by Schuknecht in 1978. It constitutes the development of symptoms consistent with endolymphatic hydrops either ipsilateral or contralateral to an ear with a profound hearing loss. We report our experience with this condition amongst 394 patients with Meniére's syndrome followed prospectively. Of 24 patients with the ipsilateral condition, nearly half responded to medical treatment. Thirteen patients underwent vestibular nerve section and the outcome, as assessed by a disability grading system, was very satisfactory. The pro

Delayed Endolymphatic Hydrops (DEH) is a clinical entity which is characterized by an early phase with a profound sensorineural hearing loss in one ear. After a prolonged period of time (from 1 to 68 years) a late phase of the disease appears with different otologic symptoms. The ipsilateral type of endolymphatic hydrops appears in the deaf ear with consequent episodic vertigo. The contralateral type develops symptoms of endolymphatic hydrop in the previously normal ear, with the onset of fluctuating hearing loss and/or episodic vertigo. This paper is a review of 12 cases of DEH (7 ipsilateral

A quantitative morphologic study of the cellular changes in human Reissner's membrane (RM) was performed under light microscopy on 30 normal temporal bones from 15 subjects, and on 20 temporal bones from 10 subjects with endolymphatic hydrops. The cellular density of the epithelial and mesothelial layers of normal RMs decreased with increasing age. In endolymphatic hydrops, cellular proliferation and decreased cellular density of a distended RM were observed, and these changes occurred more remarkably in epithelial cells. Decreased cellular density in RM was correlated to the degree of endolym

Our previous work has shown that auditory brainstem responses (ABRs) can be used to measure the basilar membrane travelling wave velocity (TWV). Based upon the hypothesis that increased pressure in the scala media will increase the stiffness of the basilar membrane and, hence, increase the speed of the travelling wave. ABRs have been used to provide a means of objectively testing for endolymphatic hydrops. Ten normally hearing subjects and 20 Meniere's patients were tested. The data for the Meniere's group show that the TWV was within normal limits at all frequencies tested, except 5.7 kHz. At

Delayed endolymphatic hydrops is a disease entity that can be differentiated from Meniere's disease. This condition was first reported simultaneously by Nadol, et al. and Wolfson and Leiberman, with further clarification by Schuknecht. The disease is characterized by a profound sensorineural hearing loss in one ear, with the onset of episodic vertigo from that ear after a prolonged period of time (ipsilateral delayed endolymphatic hydrops) or the development of fluctuating hearing loss and/or episodic vertigo in the opposite ear after a prolonged period of time (contralateral delayed endolymph

Gentamycin or kanamycin have no effect on endolymph formation in experimental induced endolymphatic hydrops. The increase of endolymphatic volume after obliteration of endolymphatic duct and sac is not influenced by aminoglycosides as well as the decrease of DC-potential and the increase of sodium-activity in endolymphatic hydrops. The way of action of intratympanal injected gentamycin seems to be in a partial chemical induced labyrinthectomy.

This paper discusses an analysis of two groups of patients who underwent a tack operation for incapacitating recurrent attacks of vertigo due to idiopathic endolymphatic hydrops. Group I consisted of the first 140 patients operated on by one of the authors (D.T.R.C.). All were followed up for at least 5 years after the operation. However, between the 63rd and 166th postoperative months, 12 patients died of unrelated causes, and an additional 26 patients were lost to follow-up between the 70th and 119th postoperative months. The analysis was based on the condition of these patients up to the ti

: The relationship between Menière's disease and endolymphatic hydrops is ambiguous. On the one hand, the existence of cases of endolymphatic hydrops lacking the classic symptoms of Menière's disease has prompted the assertion that endolymphatic hydrops alone is insufficient to cause symptoms and drives the hypothesis that endolymphatic hydrops is a mere epiphenomenon. Yet, on the other hand, there is considerable evidence suggesting a relationship between the mechanical pressure effects of endolymphatic hydrops and resultant disordered auditory physiology and symptomatology. A critical apprai

Tinnitus is one of the symptoms of Meniere's disease. The relationship between a clinical presentation of subjective tinnitus or ear fullness and endolymphatic hydrops (EH) has not yet been explored. We studied 15 patients with symptoms of tinnitus as their major complaint, with or without hearing loss, who were evaluated using magnetic resonance imaging (MRI). The mean age of the subjects was 59 years (range 35-79 years). Nine were women and six were men. Patients were divided into two groups based on whether they had fluctuating or stable tinnitus. These groups were subdivided in the presenc

PURPOSE OF REVIEW: Meniere's disease is characterized by the triad of fluctuating hearing loss, episodic vertigo, and tinnitus and by endolymphatic hydrops found on post-mortem examination. The cause of Meniere's disease remains unclear. Numerous factors play a role in the development of hydrops and in the pathogenesis of related cochleovestibular dysfunction. This review highlights recent advances in the understanding of the pathophysiology of symptom development in Meniere's disease by detailing the role of genetics, autoimmunity, endolymphatic fluid homeostasis, excitotoxicity, oxidative st

To investigate the role of pathology of the stria vascularis in endolymphatic hydrops, the vascularity and atrophy of the stria were examined by histological observation in guinea pig temporal bones affected by experimental endolymphatic hydrops. Blood cell sludge and vasoconstriction accompanied by strial atrophy were observed in the coiled arteriole and capillary of the stria vascularis of the ear with endolymphatic hydrops after exposure to noise, suggesting the development of inner ear circulatory disturbance. Furthermore, these findings were observed around the upper turn of the cochlea.
