Pediatric infectious endophthalmitis: a review.

Infectious endophthalmitis is a rare but severe complication of septecemia, intraocular surgeries, or penetrating eye trauma. The etiology, prognosis, and management of pediatric endophthalmitis resulting from exogenous and endogenous infections are reviewed. Open-globe trauma and glaucoma surgery are the most frequent causes of endophthalmitis in children, whereas endogenous infection is the least common cause. Streptococcus and Staphylococcus species are common bacterial agents in both posttraumatic and postoperative pediatric endophthalmitis, whereas Candida albicans is a commonly reported …

Bacterial endophthalmitis after suture removal.

We present 3 cases of endophthalmitis following suture removal after cataract surgery. In all cases, prophylactic antibiotics had been used. Treatment included vitreous tap and intravitreal antibiotic injection, with only 1 of the 3 patients regaining good visual acuity. Because povidone-iodine 5% is more effective at decreasing conjunctival bacterial counts than topical antibiotics, we recommend this method of conjunctival preparation before suture removal.

[Bacillary endophthalmitis. Four case reports].

PURPOSE: Bacillary endophthalmitis occurring after penetrating ocular trauma with an intraocular foreign body is always associated with poor visual outcome. Bacilli cause fulminant infection associated with tissue damage in the intraocular structures. CASE REPORTS: Our series consisted of four patients with penetrating ocular trauma and endophthalmitis caused by B. cereus or B. licheniformis. Three eyes ultimately developed phtisis. Only on eye recovered good vision (2.5/10 P4). DISCUSSION: Recommended early treatment includes topical, subconjunctival, parenteral antibiotics. A review of the…

Endophthalmitis following open-globe injuries.

Endophthalmitis following open-globe injuries is caused by a specific range of microorganisms, of which Bacillus sp. and coagulase-negative Staphylococcus are the most frequent. Risk factors include the presence of an intraocular foreign body, injury inflicted by organic material, delay in surgery, and the type of wound involved. Despite important advances in medical and surgical management, this type of endophthalmitis continues to pose a poor prognosis. In this sense, we consider prevention to be the best approach. We report our protocols for the prevention and treatment of endophthalmitis f…

Endophthalmitis following penetrating eye injuries.

Postinjury endophthalmitis is the eye infection with the worst prognosis. A retrospective 9-year study was made of penetrating eye injuries, with an analysis of the incidence of infection and its relation to the type of wound and the presence of intraocular foreign bodies. There were 403 cases of penetrating eye injury; of these, 233 affected the cornea and 170 involved the posterior pole. Intraocular foreign bodies were present in 40 cases. Endophthalmitis developed in 4.2% of cases (17/403), and was more common in patients with posterior pole involvement (7%) than in purely corneal trauma (2…

Group B streptococcal metastatic endophthalmitis.

Reports of invasive Group B Streptococcus infection in adults with underlying medical conditions have been increasing. Ocular infection with this organism is unusual. Metastatic endophthalmitis in adults caused by this organism has been reported rarely and has only been associated with endocarditis. We encountered two cases of Group B streptococcal metastatic endophthalmitis in adults who did not have endocarditis. These cases reflect the increasing incidence of invasive Group B Streptococcus infection with its varying manifestations. Additionally, they emphasize the importance of considering …

[Vitreous surgery for mycotic endophthalmitis].

We treated eight eyes of five patients with mycotic endophthalmitis. Intraocular inflammatory changes in one of these eyes improved only by antifungal therapy with 21-day administration of miconazole and fluconazole. The other seven eyes underwent vitrectomy, and the preoperative periods between the onset of ocular complications and the operation ranged from one and a half months to three months, during which time fungi were isolated only in one case. Dense vitreous opacities caused by the colonies of the fungi were found in the all vitrectomized eyes. Vitrectomy in the three cases showing no …
