Effect of decreased gallbladder stimulation on enterohepatic cycling and kinetics of bile acids.

The effect of diet on the rate of enterohepatic recycling and bile acid kinetics was studied in groups of human subjects by reducing dietary … diet, but their synthesis rate was not significantly changed. It is concluded that (1) significant enterohepatic circulation of bile acids occurs even in the absence of dietary stimuli for gallbladder contraction; and (2) diet may significantly affect bile acid pool size and fractional turnover, while bile acid synthesis remains essentially unchanged.

Rationale for therapy with ursodeoxycholic acid in patients with cholestatic liver disease.

Ursodeoxycholic acid is increasingly used in the treatment of a variety of cholestatic diseases such as primary biliary cirrhosis. The present article summarizes current understanding of the mechanism of action of ursodeoxycholic acid in these diseases. Apart from a direct hepatoprotective effect, ursodeoxycholic acid also influences the enterohepatic circulation with decreased ileal reabsorption and increased faecal excretion of potentially hepatotoxic endogenous bile salts. In addition, a cholehepatic shunt mechanism and immune modulating effects may also be important.

Chronic bile duct cannulation of rhesus macaques (Macaca mulatta) without causing biliary fistulas.

Two surgical procedures were used for establishing chronic bile duct cannulations in rhesus macaques (Macaca mulatta) while maintaining an intact enterohepatic circulation for use in metabolism studies. One procedure resulted in the formation of biliary fistulas in all of the animals, whereas the other procedure allowed successful maintenance of the macaques without fistulation for up to 8 months after surgery. The possible importance of pressure against bile outflow in the development of the fistulas was discussed.

High-Fat Diet-Induced Decreased Circulating Bile Acids Contribute to Obesity Associated with Gut Microbiota in Mice.

The altered circulating bile acids (BAs) modulate gut microbiota, energy metabolism and various physiological functions. BA profiles in liver, serum, ileum and feces of HFD-fed mice were analyzed with normal chow diet (NCD)-fed mice after 16-week feeding. Furthermore, gut microbiota was analyzed and its correlation analysis with BA was performed. The result showed that long-term HFD feeding significantly decreased hepatic and serum BA levels, mainly attributed to the inhibition of hepatic BA synthesis and the reduced reabsorption efficiency of BAs in enterohepatic circulation. It also signific…

Partial External Biliary Diversion for Severe Diarrhea After Liver Transplant in Patients with Progressive Familial Intrahepatic Cholestasis Type 1.

Progressive familial intrahepatic cholestasis is a heterogeneous group of autosomal recessive disorders, and liver transplant is the only curative treatment. A biliary diversion operation for disruption of enterohepatic circulation in patients with progressive familial intrahepatic cholestasis type 1 without cirrhosis is another option. We present a pediatric patient with progressive familial intrahepatic cholestasis type 1 who underwent liver transplant due to end-stage liver disease. After transplant, diarrhea and growth retardation complications resolved after partial external biliary diver…

Explanation for potency of repeated oral doses of morphine?

Morphine given by injection is the standard by which all other strong analgesics are measured, but when given orally in single doses it is a poor analgesic. With repeated oral administration it becomes very effective and it may be that on repeated dosage active metabolites, particularly morphine-6-glucuronide, account for much of the analgesic activity. Enterohepatic circulation may also contribute to the maintenance of blood and tissue levels of morphine and its metabolites in chronic use.

Roles of the Hepatic Endocannabinoid and Apelin Systems in the Pathogenesis of Liver Fibrosis.

Hepatic fibrosis is the consequence of an unresolved wound healing process in response to chronic liver injury and involves multiple cell types and molecular mechanisms. The hepatic endocannabinoid and apelin systems are two signalling pathways with a substantial role in the liver fibrosis pathophysiology-both are upregulated in patients with advanced liver disease. Endogenous cannabinoids are lipid-signalling molecules derived from arachidonic acid involved in the pathogenesis of cardiovascular dysfunction, portal hypertension, liver fibrosis, and other processes associated with hepatic disea

Multi-omics insights into the physiological mechanisms of bile acid accumulation in the gallbladder in brumation-like snakes.

Hibernation/brumation represents an important physiological adaptation for animals to cope with seasonal environmental changes. Field observ … oncentrations in serum and intestinal contents were markedly reduced, indicating suppression of the enterohepatic circulation and consequent accumulation of bile acids in the gallbladder. Hepatic transcriptomic analysis revealed significant downregulation of bile acid synthesis and regulatory genes in brumation-like snakes. In contrast, the alternative synthesis pathway ge …

The effect of pancreatic and biliary depletion on the in vivo pharmacokinetics of digoxin in pigs.

Several transporter systems in the liver and intestine are known to change their expression and function during cholestatic disease states. The objective of the present in vivo study was to investigate the effect of biliary depletion, as a method to mimic cholestasis, on the bioavailability and disposition of digoxin in biliary and pancreatic duct cannulated pigs. The study was divided in two parts. In the first part, a solution of 10 microg/kg digoxin was administered intravenously to the cannulated pigs with intact enterohepatic circulation (Control) and during depletion of the bile and panc…

The choleretic action of cholecystokinin and cholecystokinin octapeptide in dogs.

Mongrel dogs were prepared by cholecystectomy, ligation of the lesser pancreatic duct, and insertion of gastric and duodenal cannulas. The common bile duct was cannulated through the duodenal fistula for bile collection while the enterohepatic circulation of bile salts was maintained by intravenous infusion of sodium taurocholate. Cholecystokinin (CCK) and cholecystokinin octapeptide (CCK-OP) caused increased bile flow and had no influence on the biliary clearance of erythritol. The results indicate that the predominant mechanism of CCK and CCK-OP choleresis is due to ductular stimulation.

Enterogastric and enterohepatic recycling of dendrobine in rats: a novel disposition mechanism responsible for pharmacokinetic multiple peaks.

Dendrobine, a neuroprotective alkaloid, exhibited complex pharmacokinetics characterized by multiple peaks and continuous concentration plateaus in rats, particularly for dendrobine and its major metabolite N-demethylated dendrobine (N-deM dendrobine), but not for dendrobine N-oxide. This study elucidated the disposition mechanism underlying the multiple peaks phenomena through systematic investigation of tissue distribution, excretion, enteral reabsorption, and intestinal biotransformation. Key findings revealed that glucuronide N-deM dendrobine and glucuronide N-deM dendrobine N-oxide were m

Hyperbilirubinemia connected with parenteral administration of higher amounts of fluids in premature infants.

During a study on the influence of different amounts of fluid intake on water and electrolyte metabolism in the first 3 days of life, a high incidence of hyperbilirubinemia was observed in infants receiving a large water load (150 ml/kg/24 h) intravenously. The amount of meconium excreted during the 3-day period in newborns with total parenteral alimentation was significantly lower than in controls. Hyperbilirubinemia is considered to be due to the enterohepatic circulation of bilirubin present in the retained meconium. The role of slight hemolysis and insufficient stimulation of choleresis is…

Oral manifestations of oxalosis secondary to ileojejunal intestinal bypass.

Oxalosis is a rare condition that may be either hereditary or acquired. It may be secondary to a number of primary causes, which include renal failure, oxalate poisoning, malabsorption syndromes, and in this case, ileojejunal bypass surgery. Systemic oxalate overload following such bypass surgery results from a defect in the enterohepatic circulation and from the loss of calcium and bile salts in the feces. The oxalate is then absorbed into the circulation, and the supersaturated solution precipitates in the systemic tissues. Reported cases of oral involvement are rare. This article presents a…

Bileome: The bile acid metabolome of rat.

Bile acids (BA) play a vital physiological role in vivo. They are not only detergent of dietary lipids and nutrients, but also important hormones or nutrient signaling molecules in metabolic regulation process. Recent studies have also shown BA involvement in various cancers and diseases such as Parkinson's and Alzheimer's and liver diseases. However, majority of the reported literature about BA is restricted to enterohepatic circulation. Hitherto, there has been no comprehensive study of the BA profile in all the major tissue and biofluids in rat has been reported. In this first bileomics stu…

Feminization of chronic alcoholic men: a formulation.

A review of the factors historically thought to contribute to the feminization of men with Laennec's cirrhosis is presented. Objective scientific data is presented both in support of and in rejection of such factors when available. Recent hypotheses about the significance of an altered estrogen to androgen (E/T) ratio as being important in the pathogenesis of feminization also are discussed. Finally, a hypothesis which incorporates the findings of hypogonadism and cirrhosis with portal-systemic shunting is presented as a pathogenic mechanism for the feminization of men with alcohol-induced Lae
