A 72-year-old woman underwent surgery for a distal radius fracture with lower jaw fracture under general anesthesia. Preoperative laboratory data showed hypokalemia (3.1 mEq · l(-1)), hypertension, and leg edema. These adverse effects in terms of immunoallergic reactions include interstitial pneumonia, liver injury, allergic cystitis, and drug eruption. In this manuscript the chemical and pharmacokinetic evaluation of a doping case associated with the use of an Ephedra-labelled dietary supplement is reported. The urine of a Dutch professional cyclist was found to be positive for norpseudoephedrine at a doping control. An inquiry revealed that the cyclist had consumed a liquid herbal food supplement with Ephedra as one of its 15 declared ingredients. Various botanical extracts are used as adjuncts in treating obesity. A recent review concerning botanical products used to suppress appetite has included a discussion of Citrus aurantium and p-synephrine. The authors have incorrectly equated the actions of C. aurantium with ephedra in terms of adrenergic receptor binding, pharmacological effects and safety. Current research literature is reviewed which clarifies these issues.

The Food and Drug Administration has banned the sale of ephedrine-based weight-loss products because of their association with many cardiovascular adverse effects. Bitter orange is now being used as a stimulant in "ephedra-free" weight-loss supplements but was recently implicated in adverse cardiovascular sequelae. To our knowledge, this report describes the first case of variant angina associated with bitter orange in a dietary supplement.

Ephedra containing products (ECPs), which most often contain additional sources of caffeine alkaloids, may be an under-recognized cause of hypertension. ECPs, especially when used in combination or at higher than recommended doses, can cause life-threatening cardiovascular and neurological complications. We present a case of hypertensive encephalopathy with new onset generalized tonic-clonic seizure secondary to concomitant use of two OTC supplements containing a mixture of ephedrine and caffeine alkaloids.
