A Japanese infant had bilateral hypopigmented macules in a whorled or marble-cakelike configuration on her trunk and extremities. Her irides were bluish gray and her hair was dark brown. She had photophobia and macrocephaly with developmental delay. Analysis of peripheral lymphocytes and dermal fibroblasts disclosed a normal chromosomal pattern. A new finding was that the eumelanin content of the hypopigmented epidermis was decreased. We suggest that at least a portion of patients with this disorder have a defect in eumelanogenesis in hypopigmented skin.

A functioning epidermal melanin unit implies a melanocyte capable of transferring melanosomes to keratinocytes; this requires not only melanocytes with adequate dendrites but also "receptive" keratinocytes. Skin with incontinentia pigmenti was examined by electron microscopy. Premelanosomes were occasionally found within keratinocytes and deposits of extracellular granular material that came from vacuolar degeneration of keratinocytes adjacent to melanocytes.

By immunohistochemical methods, we detected CEA in the epidermis of psoriatic skin in 10 out of 20 cases with the plaque type of psoriasis vulgaris. The CEA detected in the psoriatic skin was limited in area to the intra-squamous cell cytoplasm, cell membranes, and intercellular space just beneath the markedly parakeratotic layer. Eighteen patients with lepromatous leprosy (LL) showed a significant reduction (P less than 0.001) of Langerhans cells (LC) irrespective of whether the biopsies were obtained from involved (398 +/- 186) or healthy skin (304 +/- 98). Aim: To identify DNA methylation and transcription biomarkers in the psoriatic epidermis. The interactions between lymphocytes, Langerhals cells and keratinocytes, are described by electron microscopy in cutaneous lesions of malignant epidermotropic lymphomas (mycosis fungoides and Sézary's syndrome). Various contacts are observed between the different cells. During the degenerating process of keratinocytes, Langerhans cells disappear and histiocytic cells are bound to necrosing epidermal cells. The immunological process of Pautrier microabscess formation is discussed by comparison with ultrastructural aspects of cutaneous lecions of G.V.H. reaction in man.
