References on the anatomical course of the neurovascular bundles of the penis in epispadias are rare. We studied the anatomy of the neurovascular bundles in 5 patients undergoing primary epispadias repair and 13 adolescents undergoing correction of erectile deformities. In all primary cases the neurovascular bundles were truly lateral as they ran along the distal and middle portions of the corporeal bodies, and became anterolateral only proximally. The operative treatment was used in 46 children with total epispadia. In 37 patients operations were made by the Derzhavin method (in eight of them it was one-step plasty of the ureter and penis with local tissues), in 9 patients the operations were made after Savostitski--Young. Long-term results were observed in 38 patients. Good long-term results were obtained in 25 patients (22 following the Derzhavin operation, 3 following the Savostitski--Young operations). The Derzhavin operation is considered to give better results when performed at the age of 6-7 years.

INTRODUCTION AND IMPORTANCE: Urethral duplication is a rare congenital urinary tract anomaly. Its association with male epispadias is extremely rare. Due to various anatomical types, patients with urethral duplication can have a wide range of clinical presentations. The term exstrophy-epispadias complex refers to a group of midline defects ranging from epispadias to cloacal exstrophy. Bladder exstrophy is the most frequent malformation of this spectrum and it can present as a classical or a variant form. We report a case of a hybrid bladder exstrophy variant having some characteristics of both a duplicate bladder exstrophy and a superior vesical fistula.

An improved surgical approach to glanuloplasty in male epispadias repair is reported. The 3 essential steps of this repair include 1) reverse meatal advancement and glanuloplasty, 2) excision of redundant glans wing tissue bilaterally and 3) creation of a glandular urethra with a 2-layer glans closure. In more than 200 cases this technique has proved successful in producing a cosmetically satisfactory glans with a low rate of fistula formation (less than 5%).

The outcome of 15 children and young adults who underwent reconstruction of continent urinary reservoirs (CUR) from January 1987 to 1990 is presented. A surgical technique for the repair of complete epispadias, with emphasis on correction of the urethropenile deformity and attempt to restore normal anatomic relations has been presented. The epispadiac orifice is dissected and followed proximally to beyond the bladder neck, then brought out perineally between the bifurcated crura. The penis is denuded from its skin, then embedded in the scrotum, except for the glans penis which protrudes from the midscrotum.
