Protecting affiliation during pandemic crises: a capabilities approach for public health ethical deliberation.

During the COVID-19 pandemic, non-pharmaceutical interventions that affected people's ability to access and maintain social relationships, which philosopher Martha Nussbaum calls "affiliation", were implemented. However, the WHO has reported that no measures to actively protect social relationships have been put in place. Furthermore, existing pandemic ethical frameworks do not explicitly specify affiliation protection as a relevant ethical consideration. Subsequently, the main aim of this work is to justify why pandemic public health ethical deliberation should aim at protecting people's capa

Treating Psychosis Today: A Lacanian Take.

This paper examines the principal ideas from Jacques Lacan's psychoanalytic theory of psychosis. According to Lacan's theory in the 1950s, the central organizing element of symbolically organized mental life, the Name-of-the-Father, is missing in psychosis. That theory changes with later conceptual developments in Lacan's work that focus on the incompleteness of symbolic functioning. This connects with how, in his works from the late 1960s and the 1970s, Lacan embraces the idea of a fundamental non-rapport and symbolic non-existence at the basis of mental life. In a second step, the paper expl

Ethical Challenges and Considerations in Dysphagia Management: A Scoping Review.

BACKGROUND: Speech and language therapists (SLTs) working in dysphagia care regularly navigate complex ethical dilemmas involving clinical risk, patient autonomy and cultural considerations. While ethical principles are well-recognized in the field, consolidated evidence mapping how these principles and ethical reasoning components have been represented in the literature over time remains limited. This review offers a comprehensive synthesis of ethical challenges in dysphagia management across decades and contexts, uniquely structured using Rest's Four-Component Model of ethical behaviour. ME

Adult persons with type 2 diabetes and family engagement in diabetes self-management education and support: what works, for whom, under what circumstances and how? - a realist review protocol.

INTRODUCTION: Diabetes self-management education and support (DSME/S) programmes must demonstrate effective engagement of family members to achieve diabetes self-management outcomes and well-being of the whole family. These are complex interventions and their effectiveness is context dependent. We present initial programme theories of DSME/S and aim to answer: In what contexts and through which mechanisms do DSME/S interventions involving family caregivers impact diabetes self-management outcomes? METHODS AND ANALYSIS: The review will follow the Realist and Meta-narrative Evidence Syntheses:

"Reconciling"- conceptualising the grieving process of family members involved in assisted dying: a grounded formal theory.

BACKGROUND: The death of a loved one invariably triggers a complex constellation of profound emotional, psychological, and physical grief reactions. While theoretical frameworks for understanding the grieving process are well-established, the advent of assisted dying presents a novel and under-examined context for grief and bereavement. Thus, this study sought to generate a theory explaining the grieving process of family members involved in assisted dying. METHODS: A grounded formal theory approach was employed. Extensive systematic searches were undertaken across various four databases for

Patient refusal of treatment in obstetrics.

During the later half of the previous century, Clinical Ethics evolved from the iconoclastic and murky realm of philosophical dissertation into a practical and useful discipline to be applied to clinical dilemmas occurring in everyday practice. We have explored a brief exposition of prevailing ethical theories that are most commonly recognized. These include Consequentialism and Principlism, and we have applied them to actual cases as they might occur in clinical practice. It is our hope that this will help us all to facilitate decisions with which we will be comfortable and which will be comp

Ethics of nursing in the digital age: perceptions and challenges among Korean nursing students.

BACKGROUND: The advancement of digital technologies has brought transformative changes across the healthcare sector, and nursing is no exception. However, existing research has largely overlooked the ethical challenges nursing students face in real-world clinical settings, including dilemmas arising from the integration of artificial intelligence, patient privacy, and digital communication. This study explored nursing students' perceptions and challenges regarding nursing ethics in the digital age, focusing on the ethical dilemmas they face and their strategies for resolution. METHODS: We con

Ethics at the Centre of Global and Local Challenges: Thoughts on the Future of Business Ethics.

To commemorate 40 years since the founding of the Journal of Business Ethics, the editors in chief of the journal have invited the editors to provide commentaries on the future of business ethics. This essay comprises a selection of commentaries aimed at creating dialogue around the theme Ethics at the centre of global and local challenges. For much of the history of the Journal of Business Ethics, ethics was seen within the academy as a peripheral aspect of business. However, in recent years, the stakes have risen dramatically, with global and local worlds destabilized by financial crisis, cl

Square pegs in round holes: the dilemma of conjoined twins and individual rights.

The judgment in the English Court of Appeal case of Re A (Conjoined Twins: Surgical Separation) highlights forcefully the highly individualistic and abstract assumptions that commonly shape the deployment of rights discourse in liberal legal adjudication. Forced by the all-or-nothing nature of this discourse into a dilemma between perceiving of the twins as separate right-bearers or perceiving of the stronger twin, Jodie, as the singular right-bearer and of Mary, her weaker sibling, as a non-legal entity, the court chose the former option. Perceiving of the twins as distinct and equal legal pe

A Brief History of Biomedical Research Ethics in Iran: Conflict of Paradigms.

During the past two decades, Iran has experienced a noteworthy growth in its biomedical research sector. At the same time, ethical concerns and debates resulting from this burgeoning enterprise has led to increasing attention paid to biomedical ethics. In Iran, Biomedical research ethics and research oversight passed through major periods during the past decades, separated by a paradigm shift. Period 1, starting from the early 1970s, is characterized by research paternalism and complete reliance on researchers as virtuous and caring physicians. This approach was in concordance with the paterna

A tailored strategy for designing the Walk-Copenhagen (WALK-Cph) intervention to increase mobility in hospitalised older medical patients: a protocol for the qualitative part of the WALK-Cph project.

INTRODUCTION: Older medical patients (>65 years) represent 54% of the admissions to Danish medical and emergency departments. Acute admissions and bed-rest during hospitalisation are independent risk factors for death and dependency in older patients. Even short hospitalisations are associated with increased dependency in activities of daily living after discharge. Interventions that increase mobility during hospitalisation are therefore important. The purpose of this protocol is to describe the intervention design of the WALK-Copenhagen project, aimed at increasing 24 hours mobility in older

The repeating history of objections to the fortification of bread and alcohol: from iron filings to folic acid.

The fortification of staple foods has eliminated many deficiency diseases. Despite this, "tampering" with people's food always provokes opposition, much of it from health professionals. Opposition is often based on self-interest, tunnel vision and theory rather than research. A historical perspective of the patterns of objections to fortification and its outcomes may help resolve the anxieties and opposing ethical positions of advocates and opponents of fortification.

Caring for and connecting with homeless adolescents.

Adolescents from a variety of backgrounds are among the growing number of homeless Americans. Although they lack maturity and various skills, they manage to survive in dangerous and stressful environments. This article asserts that social policy that leads to services such as housing, education, and healthcare should be based on a philosophy, ethic, and theory of caring and connectedness, which have been shown to protect adolescents as they mature. The article includes brief descriptions of theories of caring, pathways to homelessness for adolescents, survival needs of homeless youth, and char

Inferring design: evidence of a preference for teleological explanations in patients with Alzheimer's disease.

Unlike educated adults, young children demonstrate a "promiscuous" tendency to explain objects and phenomena by reference to functions, endorsing what are called teleological explanations. This tendency becomes more selective as children acquire increasingly coherent beliefs about causal mechanisms, but it is unknown whether a widespread preference for teleology is ever truly outgrown. The study reported here investigated this question by examining explanatory judgments in patients with Alzheimer's disease (AD), whose dementia affects the rich causal beliefs adults typically consult in evaluat

Do affective episodes modulate moral judgment in individuals with bipolar disorder?

BACKGROUND: Bipolar disorder (BD) patients experience altered emotional states and deficits in social adaptation that may also be involved in deontological moral judgments in which participants have to choose whether to sacrifice one person in order to save the lives of a greater number. METHODS: In the present study we compared the utilitarian responses of BD patients in their different states (euthymia, mania, depression) and healthy controls to moral dilemmas with low (impersonal dilemma) and high (personal dilemma) emotional saliency. RESULTS: Our findings revealed an increased tendency

Struggling to Stay Engaged During Adversity: A Daily Investigation of Frontline Service Employees' Job Insecurity and the Moderating Role of Ethical Leader Behavior.

Drawing on conservation of resources theory, this study examined the moderating role of ethical leader behavior in the effects of daily perceived job insecurity on work outcomes the next day (i.e., work engagement and customer-directed helping) through occupational regret the next morning among frontline service employees working in adverse work situations (i.e., the coronavirus disease pandemic). Using experience sampling method, data were collected from 135 frontline service employees across five consecutive workdays. The results showed that daily perceived job insecurity had a negative indi

The Golden Rule and the potentiality principle: future persons and contingent interests.

Duties to future persons are central to numerous key ethical issues including contraception, abortion, genetic selection, treatment of the environment, and population control. Nevertheless, we still seem to be lacking Parfit's 'Theory X', a general theory of beneficence whose appropriateness extends to future generations. Starting from the Golden Rule (TGR), R.M. Hare purportedly derived counterintuitive duties to potential people and 'the potentiality principle'. However, I argue that Hare's derivation involves a hidden and unjustifiable extension from TGR, and show how the most plausible for

Physician-assisted dying: theory and reality.

...[T]here appears to be a conflation of physician-assisted suicide (the doctor makes the means of suicide available by, for example, writing a prescription for barbiturates) with active euthanasia (the doctor actively intervenes to kill the patient). I believe that these two entities are quite distinct in terms of several factors: they require very different roles for the physician, they involve distinct and disparate power relationships between physician and patient, and they would likely have a substantially different impact on the ethos of the medical profession. Thus, I would argue that i
