BACKGROUND AND PURPOSE: The crista galli is part of the ethmoid bone and, as such, it could be expected that aeration of the crista would come from ethmoid cells. After observing crista pneumatization from the frontal sinuses in several cases, we undertook this study to establish how often crista galli pneumatization came from the frontal sinuses rather than from the ethmoid complex.
MATERIALS AND METHODS: Two hundred consecutive CT scans of the paranasal sinuses were studied in adult patients to obtain the incidence of crista galli pneumatization and the cell of origin for this phenomenon. A…

We reviewed the computed tomography (CT) appearances, plain film radiographs and clinical findings of 17 patients who suffered fractures of the naso-ethmoidal complex. Six patients (35%) with diplopia were noted on CT to have lateral displacement of medial orbital wall fragments into the orbit (medial wall 'blow-in' fractures) leading to displacement of the globe or extra-ocular muscles. Medial displacement of ethmoid bone fragments (medial wall 'blow-out' fractures) with medial rectus entrapment was noted in a further two cases (11%) also. Multiple associated facial fractures were seen in all…

Ocular dystopia with inferomedial displacement of the globe is an unusual but well-recognized complication of bony orbital decompression. It is caused by displacement of the muscle cone and orbital connective tissue system into the maxillary and ethmoidal sinuses. We illustrate a surgical variation of bony orbital decompression leaving intact a strut of bone at the maxillary-ethmoid junction. This bony strut reduces inferomedial displacement of the muscle cone and provides a medial supporting "ledge" in cases requiring late orbital reconstruction.

Sinus mucoceles are nonmalignant cystic tumors lined by non-neoplastic epithelium, typically involving the frontal and ethmoid sinuses. Although it is common for these mucoceles to cause destruction of surrounding bone tissue due to their growth, cerebrospinal fluid leaks resulting from skull base penetration by an ethmoid sinus mucocele have rarely been reported. A 24-year-old male patient presented with right proptosis and right periorbital pain, who underwent bilateral endoscopic sinus surgery 12 years ago. Endoscopic sinus surgery was performed to treat the right ethmoid sinus mucocele, an

The role of nasal infection in the transmission of leprosy has been extensively studied. Leprosy can affect the paranasal sinuses due to mucosal continuity and bacillaemia. This prospective study was performed on 25 untreated patients with lepromatous leprosy. 5 mm contiguous axial and coronal CT sections of paranasal sinuses, on soft tissue and bone windows, were obtained in all patients. Each sinus was examined for mucosal thickening, soft tissue densities and bony outlines. Representative biopsies were taken from ethmoid sinus to confirm the radiological diagnosis in 12 patients with multip

Rosai-Dorfman disease is a very rare disease characterized by histiocytic accumulation in the head and neck region and lymph node enlargement. We report a rare pseudo-malignant paranasal extranodal Rosai-Dorfman disease. A 69-year-old-man presented nasal bleeding and nasal obstruction. Paranasal mass was detected in the left nasal cavity and computed tomography (CT) findings are the sphenoid sinus, maxillary sinus, and ethmoid sinus were involved with inconstant bone thickening, however, no bone destruction was detected. Magnetic resonance imaging scans show iso-intensity signal in T1-weighed

PURPOSE: To delineate the relationship between the optic nerves and the posterior paranasal sinuses using CT data.
METHODS: Direct coronal sinus CT scans of 150 consecutive patients with chronic inflammatory sinus disease were reviewed by two radiologists. Axial oblique reconstructions along the course of the optic nerve were obtained for the first 100 patients. The direct relationship between the optic nerve and the posterior ethmoid and sphenoidal sinuses was recorded, as were identations into the sinus wall, course of the nerve through the sinus region, pneumatization of the anterior clino

Frontal and/or maxillary sinusitis frequently originates with pathologic processes of the ethmoid sinuses. This clinical association is explained by the close anatomical relationship between the frontal and maxillary sinuses and the ethmoid sinus, since developmental trajectories place the ethmoid in a strategic central position within the nasal complex. The advent of optical endoscopes has permitted improved visualization of these spaces, leading to a renaissance in intranasal sinus surgery. Advancing patient care has consequently driven the need for the proper and accurate anatomical descrip

PURPOSE: The osteologic anatomy of the orbit is still a field of intense research, particularly as far as vascular channels are concerned. Among them, ethmoidal foraminas (EFs) are certainly those that have more clinical importance and indeed have been deeply investigated. Unfortunately, the vast production of articles, far from clarifying their anatomy, generated a certain degree of confusion.
METHODS: A search on Pubmed and Scopus databases updated up to December 31, 2023, has been carried out with the keyword "ethmoidal foramen" yielding a list of 357 items. With a careful screening proces

Seven skulls of newborns and of infants aged 3 weeks to 12 months were studied using computed tomography. Size of the maxillary sinuses was measured. Between 3rd and 12th week of life the ethmoidal sinus formed small spaces in ethmoidal labyrinth. Maxillary sinus could be noted at the level of lower orbital margin. In newborns, it formed a shallow indentation in the lateral wall of the nasal cavity. The anteroposterior dimension of the sinus was greater than the two remaining dimensions. Beginning from the 3rd month of life, number of ethmoidal cells increased, maxillary sinuses enlarged and e

We report an aggressive cemento-ossifying fibroma of the ethmoidal sinus that initially manifested as proptosis and was misdiagnosed as meningioma after a frozen section was tested. Fibroosseous lesions containing cementum are considered to be of periodontal membrane origin and are most common in the mandible and maxilla. With the exception of the gigantiform cementoma, they are usually small, innocuous lesions following a benign course and are easily removed surgically. On rare occasions, the cemento-ossifying fibroma attains a large size, behaves aggressively, destroys bone, and requires rad

Cone beam computed tomography has become a widely used imaging technique for dental indications. Depending on the selected size of the field of view, anatomical structures outside the dentomaxillary complex become visible. As a consequence, the clinician must be able to interpret also those anatomical regions. In this article, the anatomy of the nasoethmoidal region is presented based on a literature review. The nose is characterized by the nasal septum and the superior, middle, and inferior conchae. The turbinates may be pneumatized (concha bullosa), mainly the middle concha. The ethmoid bone…

A supra-ethmoidal approach was used to treat cerebrospinal fluid rhinorrhea secondary to anterior fractures of the base of the skull. After an internal orbital incision, the anterior wall of the frontal sinus is removed and the posterior wall of this sinus trephined in the internal region of its horizontal portion in order to expose the meninges covering the roof of the first ethmoidal space. Dissection is continued within the space towards the anterior extremity of the apophysis of the crista galli against which the fibrous band issuing from the anterior ethmoidal opening is sectioned. The Goodale-Montgomery osteoplastic obliterative frontal sinusotomy has been the ultimate resource in America for the most difficult and intractable cases of frontal sinusitis. This operation respects the integrity of the frontal floor and is based upon a concept with which the author disagrees: that the frontal sinus may be treated as an entity quite separate form the ethmoid. Macbeth, who does not obliterate, avails himself of the excellent transfrontal approach to the ethmosphenoids, but breaches the frontal floor in so doing. American surgeons have not followed his lead. The pros and cons

OBJECTIVE: The anterior ethmoidal artery (AEA) is an important structure to identify during endoscopic sinus surgery. Although identification on imaging is easily taught, a consistent endoscopic landmark for the AEA, independent of anatomic ethmoid cell variation, is lacking, leaving many surgeons unclear about the exact location without dependence on navigation. Here, we describe a consistent endoscopic landmark, regardless of anatomical ethmoid variation.
METHODS: We prospectively enrolled adult patients undergoing endoscopic surgery involving frontal and ethmoid sinuses in this observation
