Exercise-induced asthma in asthmatic children. Predisposing factors.

BACKGROUND: Exercise-induced bronchoconstriction (EIB) has a high prevalence in children with asthma, and this is a common problem, even in case of controlled asthma, because of the high levels of physical activity in the childhood. OBJECTIVES: The aim of our study was to identify factors associated with the development of EIB in children with controlled asthma. METHODS: We studied children evaluated for asthma. A personal and familiar history was collected from each patient to estimate asthma severity, precipitating factors, exercise ability, immunotherapy treatment and atopic familiar diso

Wheat allergy in patients with recurrent urticaria.

BACKGROUND: Clinical observation revealed that most of wheat-induced anaphylaxis (WIA)/wheat-dependent exercise-induced anaphylaxis (WDEIA) patients showed a history of recurrent urticaria. We aim to determine the association between recurrent urticaria and anaphylaxis in wheat allergy. METHODS: Population-based cohort study involved patients with WIA (n = 193, including WDEIA n = 104), recurrent urticaria (n = 177), non-wheat-related anaphylaxis (n = 584), atopic disease (excluding anaphylaxis, n = 221) and healthy control (n = 95) from 2009 to 2016. Detailed course of urticaria and anaphyla

The cow's milk allergy that wasn't: allergy to supplemental oral lactase enzyme.

BACKGROUND: Allergy to supplemental lactase is sparsely reported in the literature with only one prior case of anaphylaxis documented [2]. Reactions to this agent can occur following cow's milk ingestion which could lead to an erroneous diagnosis of cow's milk allergy in the absence of another explanation. CASE PRESENTATION: Our patient, a 48-year-old male with eczema, exercise-induced asthma and rhinoconjunctivitis, presented with four episodes of systemic reactions characterized by mucosal swelling and asthma symptoms following ice-cream exposure. It was later recognized that he had been ta

Identification of IgE-binding epitopes on gliadins for patients with food allergy to wheat.

BACKGROUND: Food allergy to wheat induces different symptoms as atopic eczema/dermatitis syndrome (AEDS), urticaria and more severe reactions as wheat-dependent exercise-induced anaphylaxis (WDEIA). Different gliadin classes are involved in this allergy but IgE-binding epitopes are known only on omega5-gliadins and for WDEIA cases. OBJECTIVES: The aim of the study was to identify IgE-binding epitopes on several gliadin classes and for several patients with different symptoms and ages. METHODS: Eleven sera were analysed by pepscan with overlapping synthetic peptides. RESULTS: Sera from five

IgE-mediated food allergy throughout life.

Food allergy (FA) has become an increasing problem throughout the world. Over the last 2 decades, the frequency of FA has increased in both children and adults. The prevalence differs according to the research methodology, age, and geographic regions, ranging between 2.0% and 10.0%. The most common form of FA is immunoglobulin E (IgE)-mediated FA. In this form, patients may present with life-threatening conditions, such as anaphylaxis, or milder conditions, such as urticaria, angioedema, sneezing, and nausea alone. The gold standard in the diagnosis of FA is oral provocation tests. Epidermal s

Identification of a Candidate IgE-Binding Maize Protein in a Patient With Corn-Dependent Exercise-Induced Anaphylaxis: A Case Report.

Corn-dependent exercise-induced anaphylaxis (food-dependent exercise-induced anaphylaxis [FDEIA]) is rare, and its causative allergens remain poorly defined. Here, we report a case of corn-dependent FDEIA and investigate candidate IgE-binding proteins using immunological and proteomic analyses. A 13-year-old boy with a history of buckwheat allergy developed recurrent anaphylaxis during exercise after ingesting corn-containing foods. An exercise challenge after ingesting 272 g of sweet corn reproduced the symptoms and confirmed the diagnosis. Corn-specific IgE was 3.72 UA/mL, and skin prick tes

Report of prenatal maternal demoralization and material hardship and infant rhinorrhea and watery eyes.

BACKGROUND: Previously, we found that reported infant rhinorrhea and watery eyes without a cold (RWWC) predicted school age exercise-induced wheezing, emergency department visits, and respiratory-related hospitalizations for asthma. These findings appeared independent of infant wheezing and allergy. Overall, we theorize that prenatal material hardship and psychosocial distress can induce infant dysregulation in the autonomic nervous system leading to infant RWWC and school age exercise-induced wheezing. OBJECTIVE: To test the hypotheses that indicators of prenatal stress and measures of mater

Prevalence of Asthma Symptoms in 13-14-Year-Old Adolescents in Karaj.

Asthma is the most common chronic inflammatory respiratory disorder in children. This study was designed to assess the prevalence of asthma in 13-14-year-old adolescents in Karaj, Alborz province in Iran, using the international study of asthma and allergies in childhood (ISAAC) questionnaire. Totally 950 adolescents attending 40 schools located in 4 regions of Karaj city were enrolled in the survey. The Persian version of the ISAAC questionnaire was filled by 13-14-year-old students. Multi-stage clustered random sampling was used to divide the city of Karaj into four educational districts. Ev

[Idiopathic anaphylaxis].

Idiopathic anaphylaxis (IA) has been described as a clinical entity by North America authors. The symptoms are brisk and relapsing episodes of angioedema, urticaria, bronchospasm, digestive symptoms, cardio-vascular collapse. Adults, females more than males, are mainly concerned. The seriousness is linked to laryngeal angioedema or to hypotension. Nevertheless lethality is rare. The first step of the procedure is aimed at the detection of several rare pathologies mimicking IA: the paroxystic capillary leak syndrome induced by a monoclonal gammapathy or by a release of cytokines, a carcinoid sy

Clinical and laboratory features, and quality of life assessment in wheat dependent exercise-induced anaphylaxis patients from central China.

Wheat dependent exercise-induced anaphylaxis (WDEIA) is a rare but potentially severe food allergy caused by the combination of wheat ingestion and physical exercise. The impact of WDEIA on quality of life (QOL) is unclear. This study characterized the clinical and laboratory features and investigated the QOL in WDEIA patients from Central China. Twenty-eight WDEIA patients were analyzed, and QOL was measured by validated Chinese version Food Allergy Quality of Life Questionnaire-Adult Form (FAQLQ-AF) and Food Allergy Independent Measure (FAIM) after obtaining the diagnosis. The results showed

Effect of Air Quality on the Mucosal Immune Molecules in Outdoor Male Athletes.

Elevated concentrations of particulate matter 2.5 (PM2.5) can harm the human respiratory system. Athletes training outdoors in polluted environments may face an increased risk of exposure. Few studies have reported on the effects of air quality on the mucosal immunity of athletes. In this study, we recruited 32 male athletes and employed a randomized crossover design. Each participant provided 2 ml of saliva sample before and after their regular 2-h exercise sessions on days with and without air pollution. The immune molecules in saliva, including immunoglobulin A, lysozyme, α-amylase, and nit

[Current position of the role of allergic and non-allergic food hypersensitivity in urticaria].

Foods are usually the first suspect as the cause of urticaria. However, a causal relationship is found only in special subtypes of urticaria. IgE-mediated food allergy should be clearly separated from non-allergic hypersensitivity (pseudoallergic reactions). The former may play a role in acute urticaria, particularly in patients with atopic dermatitis. The responsible food proteins vary with age. IgE-mediated sensitization can apply to food-dependent exercise-induced urticaria/anaphylaxis but more often the combination of food intake (irrespective of which type) plus exercise results in sympto

Exercise-Induced Bronchoconstriction in Children with Controlled Asthma Without Prominent Exercise-Related Symptoms: Prevalence and Associated Factors.

Background/Objectives: Exercise-induced bronchoconstriction (EIB) may persist despite apparently adequate asthma control and may remain unrecognized in children without prominent exercise-related symptoms. This study aimed to determine the prevalence of objectively confirmed EIB in children with controlled asthma who did not report prominent exercise-related respiratory symptoms and to identify factors associated with EIB. Methods: This cross-sectional study included 67 children with controlled asthma who underwent standardized exercise challenge testing with serial spirometry. Demographic, en

First report of asthma prevalence in Afghanistan using international standardized methods.

No data on the prevalence of asthma in Afghanistan have been published before. In a school-based survey in 2010-2011 the wheezing section of the International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire was completed by a random sample of 1500 children aged 6-7 years and 1500 adolescents aged 13-14 years old. The prevalence of physician-diagnosed asthma was 12.5% in 6-7-year-olds and 17.3% in 13-14-year-olds (P = 0.002). The prevalence of wheeze in the last 12 months was similar in children and adolescents (19.2% and 21.7% respectively). The prevalence of ever wheezing, ni

Exercise and airway injury in athletes.

Olympic level athletes present an increased risk for asthma and allergy, especially those who take part in endurance sports, such as swimming or running, and in winter sports. Classical postulated mechanisms behind EIA include the osmotic, or airway-drying, hypothesis. Hyperventilation leads to evaporation of water and the airway surface liquid becomes hyperosmolar, providing a stimulus for water to move from any cell nearby, which results in the shrinkage of cells and the consequent release of inflammatory mediators that cause airway smooth muscle contraction. But the exercise-induced asthma/

Respiratory arrest in a male athlete after running through a wheat field.

A male athlete suffered complete respiratory arrest after collapsing at the entrance to an Accident and Emergency Department of a hospital. He was resuscitated and recovered fully after several hours. He had multiple wheals on both legs and had complained of severe breathlessness before collapsing. The athlete had run through a wheat field, upon finding his usual pathway overgrown, which had caused wheat pollen to be released. He had been a mild asthmatic for 30 years and had recalled some mild urticaria after exercise but had never experienced exercise-induced asthma, or hay fever. Subsequent

Exercise-induced bronchoconstriction among Malay schoolchildren.

OBJECTIVE: Exercise-induced bronchoconstriction (EIB) may be demonstrated in 60-70% of asthmatic children in temperate climates. In areas of high humidity it is postulated to be low. The aim of the study was to determine the prevalence of EIB in a population of schoolchildren with wheezing, living in the humid tropical climate of Kuala Lumpur, Malaysia. METHOD: We performed a cross-sectional study using the International Study of Asthma and Allergies in Childhood questionnaire to identify 7-12-year-old Malay children with asthma symptoms from a primary school in central Kuala Lumpur.
