Atrial septal defect with failure to thrive in infancy: hidden pulmonary vascular disease?

Atrial septal defects are usually asymptomatic, and are closed surgically or by a catheter implanted device in preschool age children. Rarely, they may cause symptoms in infancy, and management at this age is debated. We report our experience of six infants who underwent surgical closure, with variable outcomes. Five of these had significant extracardiac pathology. Our data suggest the need to exclude other causes of symptoms, both cardiac and non-cardiac. The question should be asked as to whether surgery is of benefit in these children, and particularly whether they may have underlying prima

Long-term hospitalization of failure-to-thrive infants: developmental outcome at three years.

The present study documented later developmental outcome in a group of 29 failure-to-thrive (FTT) infants who received extended hospitalization in infancy as an intervention for their growth failure. All infants were seen at approximately 3 years of age and were given standardized assessments of intellectual and physical development. A standard interview documented demographic variables, health problems, placements subsequent to hospitalization and additional psychological and medical treatment. Infants were divided into three groups dependent on medical and treatment factors. Means and percen

Design and implementation of a pediatric failure to thrive registry system in Iran.

BACKGROUND: Failure to thrive (FTT) refers to inadequate growth caused by an inability to acquire or use enough calories for growth. It refers to a failure to develop over time, which can lead to more serious problems in adulthood if left untreated. FTT is a significant global health concern, particularly in developing countries like Iran. Effective FTT management necessitates timely, inclusive, and precise data on children's FTT status. A pediatric FTT registry system is a useful tool for gathering and managing data on children with FTT. This study proposes a pediatric FTT registry system for…

Failure to Thrive, Hyponatremia, Hyperkalemia - Differential Diagnostic Reflections of a Rare Genetic Disease.

Pseudohypoaldosteronism type I is a rare genetic disease of mineralocorticoid resistance that typically manifests in neonatal age. The patients are diagnosed with failure to thrive, dehydration, polyuria, vomiting, hyperkalemia, hyponatremia as well as potential metabolic acidosis accompanied by elevated values for serum aldosterone and renin. The disease is subdivided into a systemic and a renal type. Considering the renal type symptoms disappear in the toddler age group. The systemic type can -apart from the kidneys - affect colon, lungs, salivary and sweat glands. In that case symptoms pers…

Pathological features and proposed diagnostic criteria of porcine periweaning failure-to-thrive syndrome.

Porcine periweaning failure-to-thrive syndrome (PFTS) is a clinical syndrome characterized by anorexia and progressive debilitation of newly weaned pigs. The objectives of the current case-control study were to describe the histopathologic features of PFTS in North America and test for selected pathogens in case and control pigs on 8 farms allegedly fulfilling the clinical definition of PFTS. Based on observations during farm visits, 5 farms fully met the case definition (PFTS farms), whereas 3 farms only partially fulfilled the definition (NON-PFTS farms). Necropsy and histopathologic examina

Ring 2 chromosome associated with failure to thrive, microcephaly and dysmorphic facial features.

We report here a child with a ring chromosome 2 [r(2)] associated with failure to thrive, microcephaly and dysmorphic features. The chromosomal aberration was defined by chromosome microarray analysis, revealing two small deletions of 2p25.3 (139 kb) and 2q37.3 (147 kb). We show the clinical phenotype of the patient, using a conventional approach and the molecular cytogenetics of a male with a history of prenatal intrauterine growth restriction (IUGR), failure to thrive, microcephaly and dysmorphic facial features. The phenotype is very similar to that reported in other clinical cases with rin…

Classic Bartter syndrome: a rare cause of failure to thrive in a child.

Bartter syndrome is a group of rare autosomal-recessive disorders caused by a defect in distal tubule transport of sodium and chloride. Blood gases and plasma electrolytes raise suspicion of this diagnosis and the definitive diagnosis is made by genetic study. Early treatment improves prognosis. The authors present the case of an 11-month-old child with early failure to thrive and severe regurgitation. Blood gases revealed hypochloraemic metabolic alkalosis, hyponatraemia and hypokalaemia. Blood pressure was normal and polyuria was documented. She began therapy with potassium chloride suppleme…

[Shwachman-diamond syndrome as cause of infantile eczema associated with failure to thrive].

BACKGROUND: Shwachman-Diamond syndrome is an autosomal recessive multisystem disorder involving an insufficiency of the exocrine pancreas and haematological problems as main symptoms. Frequently, ichthyosiform skin lesions are described but are usually not the leading symptom of the disease. CASE REPORT: We report on a 6-months-old boy suffering from ichthyosiform and eczematous skin eruptions beginning at the age of 3 months which were accompanied by failure to thrive. Because of an atopic predisposition and a sensitisation to hen's egg the diagnosis atopic dermatitis with food allergy was e…

Anomalous congenital band: a rare cause of intestinal obstruction and failure to thrive.

Intestinal obstruction caused by an anomalous congenital band is very rare in adults and children. A 7-year-old boy was admitted with acute intestinal obstruction. His parents mentioned that the child always had mild abdominal distention and failure to thrive from his infancy. On his medical history, there were not any attacks of abdominal pain, fever and hospitalization. Laparotomy showed an ileal loop compressed by an anomalous band, which extended from the ileum to the sigmoid mesentery resembling a mesenteric remnant. The band was resected. Histologically, it was composed of loose connecti…

[Failure to thrive as a main symptom of intracranial tumors in early childhood].

In the course of only three years we studied three children, who presented for a long time with failure to thrive as the only symptom of an intracranial tumour. Up to 24 months elapsed from the first manifestation at the age of four, ten and eleven months to the final diagnosis. A suprasellar tumour was discovered once, an infratentorial one twice. In spite of lower caloric intake a more rapid weight gain was documented after neurosurgery. The intracranial tumour is an important part of differential diagnosis in infants and young children not thriving satisfactorily.

An infant presenting with failure to thrive and hyperkalaemia owing to transient pseudohypoaldosteronism: case report.

A 3-month-old boy presented with failure to thrive and a history of a prenatally detected unilateral hydroureteronephrosis which was confirmed after birth. His growth and developmental milestones had been normal during the first 2 months but in the third month his appetite was poor with reduced intake but no vomiting. At presentation, his temperature was normal, there was mild dehydration and there was weight loss (his weight had decreased by 270 g in the past month). Haemoglobin was 11.9 g/dL, total white cell count 20.2 × 109/L (7-15) [neutrophils 30% (39-75) and lymphocytes 61% (16-47)], pl…

Malignant hypertension secondary to renovascular disease during infancy--an unusual cause of failure to thrive.

An 11-month-old girl presented with a history of failure to thrive, vomiting, polydipsia, polyuria and visual inattention. She was found to have malignant hypertension due to unilateral renal artery stenosis. This was successfully treated with percutaneous transluminal balloon angioplasty. Nearly 10 years following this initial presentation, she remains normotensive on no anti-hypertensive medications.

Premature thelarche in an infant girl with failure to thrive related to dietary soy exposure.

A 7-month-old female infant presented with failure to thrive. She was breastfed till 3 months of age, thereafter switched to soy-based milk formula. There was no history to suggest excess energy losses, recurrent infections or chronic diarrhoea. Three months after switching to exclusive soy-based milk formula, parents noticed significant enlargement of both breasts. Clinical examination was unremarkable except for enlargement of both breasts. None of the other secondary sexual characteristics were present. Initial blood investigations showed hyponatraemic hypokalaemic hypochloraemic metabolic …

Familial autoinflammatory Behçet-like syndrome: A rare cause of failure to thrive in children.

Familial auto-inflammatory Behçet-like syndrome-1 is a rarely reported entity. The clinical manifestations of this condition mimic Behçet's disease in the form of recurrent oral ulcers and gastro-intestinal manifestations. The complete clinical spectrum is yet to be ascertained, as this is a genetic syndrome involving multiple mutations. Here, we report the case of a child with severe malnutrition and failure to thrive in infancy, who turned out to have the said genetic syndrome.
