Bacteriology of necrotizing fasciitis.

Sixteen patients with necrotizing fasciitis were observed under clinical and laboratory conditions for collection, preservation, and culture that permitted optimal retrieval of anaerobes. The clinical observations of necrosis of fascia, subcutaneous fat and skin with thrombosis of the microvasculature, and absence of myonecrosis were clearly apparent in these patients. Two clear-cut groups of culture and gram stain results were found, suggesting that the clinical entity of necrotizing fasciitis can occur after infection by different infecting organisms. The cultivation of Streptococcus pyogene

Erysipelas and necrotizing fasciitis.

The clinical course of necrotizing fasciitis in 8 patients is compared with observations on 22 other patients with erysipelas. In necrotizing fasciitis the early erythematous areas turn into a dusky blue colour with later vesiculation and formation of bullae. An important finding is a non-pitting oedema extending outside the erythematous patches. The disease often progresses and involves further skin areas proximal to the initial ones. Gangrene tends to follow in multiple sites after the 1st week of illness. Group A streptococci in conjunction with widespread thrombosis and vascular necrosis o

Imaging of necrotizing fasciitis.

Necrotizing Fasciitis (NF) is a rare, but highly lethal, rapidly progressive necrotic infection of the soft-tissue fascia. More common in immunocompromised patients, NF typically affects the extremities, perineal area, genital area (Fournier gangrene), and less commonly the torso. Although classically a clinical diagnosis, imaging is a powerful adjunct to facilitate early diagnosis in equivocal cases. The key features of NF of radiography, ultrasound, CT, and MRI are reviewed, as are the inherent limitations of NF diagnosis for each modality. With a high level of sensitivity, specificity, and

[Cervico-facial necrotizing fasciitis].

Cervical fasciitis is a life-threatening condition often due to a benign pharyngeal or dental infection, leading to extensive necrosis, spreading along the fascia of the neck and possibly to the mediastinum. It represents one of the most severe ENT emergencies which requires immediate treatment by a multidisciplinary team. Facing any banal infection of the head and neck, great attention should be paid to any functional or general symptoms and signs of severe sepsis. Patient should be urgently referred to a tertiary center where a CT scan should immediately be done, followed by surgical drainag

"Synchronous" multifocal necrotizing fasciitis.

BACKGROUND: Necrotizing fasciitis (NF) is an infection of the soft tissue, and is fatal if not promptly and aggressively treated. Although it is rare, it is not exceptional; nevertheless, its presentation may be misleading and may delay the diagnosis. We highlight the possible synchronous development of NF in multiple noncontiguous areas. CASE REPORT: A 44-year-old diabetic man with no history of trauma complained of nonspecific lower back pain, which he treated with analgesics and oral antibiotics. Erythema at the left arm appeared, and the general condition worsened. The patient was admitte

[Necrotizing fasciitis. 2011 update].

Necrotizing fasciitis belongs to a group of complicated soft tissue infections that can be even life threatening. Despite growing knowledge about its etiology, predictors, and the clinical progression, the mortality remains at a high level with 20%. A relevant reduction can be achieved only by an early diagnosis followed by consistent therapy. The clinical findings in about 75% of the cases are pain out of proportion, edema and tenderness, blisters, and erythema. It is elementary to differentiate a necrotizing or a non-necrotizing soft tissue infection early. In uncertain cases it can be neces

Mortality in necrotizing fasciitis.

OBJECTIVE: The objective of this study was to determine the mortality rate in patients presenting with Necrotizing Fasciitis. This prospective study was conducted at ward 26, JPMC Karachi over a period of two years from March 2001 to Feb 2003. METHODS: All patients above the age of 12 years diagnosed to be having Necrotizing Fasciitis and admitted through the Accident and emergency department were included in this study. After resuscitation, the patients underwent the emergency exploration and aggressive surgical debridement. Post-operatively, the patients were managed in isolated section of

Life-threatening necrotizing fasciitis.

Necrotizing fasciitis is characterized by a rapidly progressing necrosis of subcutaneous fat and fascia, which can be life-threatening without prompt recognition, surgical intervention, and immediate antibiotic therapy. Necrotizing fasciitis has been subdivided into type 1, or polymicrobial necrotizing fasciitis, and type 2, or group A streptococcal necrotizing fasciitis. In addition, synonyms, such as streptococcal gangrene and "flesh-eating bacteria syndrome," have been used in the literature.

[Necrotizing fasciitis after varicella].

Necrotizing fasciitis is a rare and severe infection characterised by extremely rapid progressive involvement of the superficial fascias and deep dermal layers of the skin, with resultant vasculitis and necrosis. The authors present three clinical cases of necrotizing fasciitis; all three patients previously had varicella rash, rapid progressive spreading erythema with severe pain and toxic shock syndrome. Two patients had positive cultures of b-haemolytic streptococcus. Early stage differential diagnosis with celulitis, aggressive antibiotic treatment and pediatric intensive care support are
