Distal Femoral Stress Fractures Has High Operation Rates in Adolescent and Young Adult Athletes.

BACKGROUND: Although stress fractures of the distal femur are rare, symptoms can overlap with other peri-articular knee pathology, delaying diagnosis. Untreated stress fractures have the potential to progress into completed fractures with a higher likelihood for requiring surgery and longer recovery times in otherwise healthy adolescents and young adults. This case series represents the largest cohort of adolescent athletes with distal femoral stress fractures who presented with activity-related knee pain. METHODS: Patients treated nonoperatively and operatively for metaphyseal distal femur s

Bone reconstruction with modified Masquelet technique in open distal femoral fractures: a case series.

BACKGROUND: Large bone defects require complex treatment, multidisciplinary resources, and expert input, with surgical procedures ranging from reconstruction and salvage to amputation. The aim of this study was to provide the results of a case series of open comminuted intra-articular distal femoral fractures with significant bone loss that were managed by early fixation using anatomical plates and a modified Masquelet technique with the addition of surgical propylene mesh. METHODS: This retrospective study included all patients referred to our institution with OTA/AO C3 distal femur open fra

[Distal femoral fractures in geriatric patients : Does time to surgery affect revision and mortality?].

BACKGROUND: For proximal femoral fractures the time to surgery has been reported to influence the mortality rate. To date, detailed analyses in geriatric patients with distal femoral fractures are not available. MATERIAL AND METHODS: A monocentric study with retrospective data retrieved from an electronic database was performed. The study included distal femoral fractures with surgical treatment between 2006 and 2017 in patients aged 65 years and older. A total of ten variables were evaluated and two outcome measures were investigated: revision and mortality in relation to time of surgery wit

Total knee arthroplasty for distal femoral fractures in osteoporotic bone: a systematic literature review.

PURPOSE: Distal femoral fractures in the elderly are associated with high morbidity and mortality and their incidence is increasing with an ageing population. Management of these fractures has evolved over recent decades and there is now an accepted recognition of the important role that acute arthroplasty may have in treatment of these fractures. Our purpose was to systematically review the evidence available in the literature for arthroplasty as a treatment option for distal femoral fractures. METHODS: This systematic review was conducted in accordance with the PRISMA reporting guidelines.

Locking plate fixation of distal femoral fractures is a challenging technique: a retrospective review.

Distal femoral fractures have typically a bimodal occurrence: in young people due to a high-energy trauma and in older people related to a low-energy trauma. These fractures are associated to a very high morbidity and mortality in elderly. Distal femoral fractures might be treated with plates, intramedullary nails, external fixations, and prosthesis. However, difficulties in fracture healing and the rate of complications are important clinical issues. The purpose of this retrospective review was to present our experience in treatment of distal femoral fracture in a sample of older people in or

Distal femoral osteotomy for genu valgum deformity caused by malunited Hoffa fracture.

Hoffa fractures are unstable intra-articular fractures of the femoral condyle that occur in the coronal plane.Insufficient anatomical reduction and internal fixation may lead to non-union or malunion. A 39-year-old man was involved in a traffic accident while riding a motorcycle and was diagnosed with left Hoffa fracture and avulsion fracture of the femoral attachment of the medial collateral ligament. Open reduction and internal fixation were performed 5 days after injury. The patient experienced intermittent knee pain, joint contracture and deformity, and attended our hospital for further tr

Surgical management of complex intra-articular distal femoral and bicondylar Hoffa fracture.

Bicondylar Hoffa's fractures of distal femur are rare. We report the case of an 18-year-old male who sustained a closed complex fracture of both femoral condyles of the left femur on the sagittal plane (Hoffa fracture), combined with distal metaphyseal fracture. He was treated using parallel plates and interdigitating screws, with our technique based on an extrapolation of the principles of distal humerus fixation. The fracture united clinically and radiologically at 16 weeks. Twelve months postoperatively, the range of movement in the knee was 5°-115°. In this complex case, our technique prov

Interlocking intramedullary nailing for ipsilateral fractures of the femoral shaft and distal part of the femur.

A review of the data on 684 fractures of the femur that had been treated with intramedullary nailing led to the identification of twenty-three patients who had had a fracture of the shaft of the femur with an accompanying ipsilateral supracondylar fracture (twelve patients, group I) or a concomitant ipsilateral intercondylar fracture (eleven patients, group II). The group-I fractures had been treated with interlocking nailing without supplemental fixation. In group II, ten fractures were stabilized with interlocking nailing and supplemental screw fixation and one, with interlocking nailing and

Management of comminuted extra-articular distal femoral fracture: A finite element study.

In case of extra-articular fractures, internal fixation is usually performed using implants such as Intramedullary nails (IMN), medial plates (MP), and lateral plates (LP). Recently, an innovative "sandwich" technique has been introduced for treating acetabular defects. The efficacy of such a technique to treat comminuted extra-articular distal femoral fractures has never been investigated. Therefore, the current study aims to compare the biomechanical performances of three implants, that is, medial plate, lateral plate, and IM nails, with and without the "sandwich" bone grafts. A CAD model of

Anatomical analysis of different helical plate designs for distal femoral fracture fixation.

PURPOSE: Helical plates potentially avoid the medial neurovascular structures of the thigh. Two implant designs for additional medial 90° helical plate in double plate constructs for geriatric patients and 180° helical plate for single plating in young patients are potential alternatives to widely used standard straight plates. AIMS: (1) assess the distances to adjacent anatomical structures being at risk when applying medial 90° and 180° helical plates with MIPO technique to the femur, (2) compare these distances with medial straight plates, and (3) correlate measurements performed during su

Complex distal femoral fracture treated with interfragmentary screws and medial buttress plating.

A woman in her 70s suffered a motor vehicle accident, resulting in an intraarticular, distal femoral fracture with a unique fracture pattern. A 3D-printed model was obtained using the preoperative CT scan for surgical planning. Anatomic reduction was achieved using a medial parapatellar approach and the fracture was fixed with a medial buttress plate and interfragmentary screws. After 18 months, the patient presented a fair Knee Society Score of 62.

The use of 'blocking' screws for the 'closed' reduction of difficult proximal and distal femoral fractures.

Most meta-diaphyseal femoral fractures that are treated with intramedullary nailing can be reduced satisfactorily by skeletal traction without 'opening' the fracture site and therefore, complications such as nonunion, infection and wound healing problems are reduced. In cases where adequate fracture reduction cannot be achieved by skeletal traction, 'reduction aids' have been used during the operative procedure in order to avoid the exposure of the fracture site. The 'blocking' screw, as a reduction tool, was proposed initially for the 'difficult' metaphyseal fractures of the tibia. Subsequently

Trends in hip and distal femoral fracture rates in Italy from 2007 to 2017.

Osteoporosis-related fractures are a growing public health concern worldwide due to high societal and economic burden. The study aims to assess trends in incidence rates of hip and distal femoral fractures and in the use of anti-osteoporosis drugs in Italy between 2007 and 2017. Patients with hip and distal femoral fractures (ICD-9-CM codes 820.x and 821.x) were identified in the Italian National Hospital Discharge Database while anti-osteoporosis medication data were retrieved from the National Observatory on the Use of Medicines Database. A joinpoint regression analysis was performed to iden

The use of knee mega-prosthesis for the management of distal femoral fractures: A systematic review.

INTRODUCTION: Distal femur fractures (DFFs) are unusual and difficult to deal, especially in elderly patients. A consensus about a gold-standard treatment has not been reached yet. Available options include both conservative and surgical management. In elderly patients a prosthetic replacement could be a valid treatment option. Literature is lacking about the use of mega-prosthesis in this type of fractures. The purpose of the present systematic review is to examine which fracture, both acute and chronic, involving distal femur should be treated by using a mega-prosthesis. MATERIALS AND METHO

The treatment of fractures of the femoral shaft with the Brooker-Wills distal locking intramedullary nail.

Ninety-two fractures of the femoral shaft in eighty-five patients were followed after treatment with the Brooker-Wills distal locking intramedullary nail. Twenty-one (22.8 per cent) of the fractures were open and seventy-one (77.2 per cent) were closed; 84.8 per cent of the fractures were comminuted. All but one fracture united, in a mean time of 4.4 months. Four fractures healed with mild angulation, and three had shortening of more than one but less than two centimeters. Significant rotatory deformities did not occur. No patient required postoperative traction or external stabilization, and

The Role of Distal Femoral Bypass in Limb Salvage Following High-Energy Femoral Fractures With Arterial Disruption: A Systematic Review.

High-energy femoral fractures complicated by arterial disruption are associated with a significant risk of limb loss due to rapid distal ischemia. This systematic review evaluated the role of distal femoral bypass in limb salvage in such injuries. A literature search was conducted in PubMed, Embase, Scopus, and the Cochrane Library following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. Five studies encompassing 309 patients were included, reporting vascular repair with autologous reversed saphenous vein grafts in the context of complex femora
