Prenatal Monitoring of Perinatal Pregnant Women and Fetus Based on a Smart Electronic Fetal Monitoring System.

Источник пометил публикацию как отозванную

The aim of the study is to study the prenatal monitoring of perinatal pregnant women based on a smart electronic fetal monitoring system. Through the comparative analysis of 230 pregnant women in maternal and child health care hospital who received fetal heart monitoring during the perinatal period and those who did not receive fetal heart monitoring during the perinatal period, cases of fetal distress, neonatal asphyxia, and cesarean section were observed in both groups. Results show that the incidences of fetal complications and cesarean sections in the experimental group were 16.36% and 36.…

Safety of Internal Electronic Fetal Heart Rate Monitoring During Labor.

OBJECTIVE: To investigate the safety and efficacy of internal electronic fetal heart rate (FHR) monitoring during labor. METHODS: This was a retrospective case-control study, which was an analysis of monitoring FHR with a fetal scalp electrode or a Doppler probe (94 pregnant women per group). In the internal monitoring group, when the opening of the uterine orifice was ≥3 cm, the fetal scalp electrode was placed after natural or artificial rupture of the membrane. FHR was simultaneously monitored using a Doppler probe. In the external monitoring group, continuous FHR monitoring was performed

Continuous Fetal Monitoring During Electroconvulsive Therapy: A Prospective Observation Study.

OBJECTIVE: The use of electroconvulsive therapy in pregnancy has been limited by concerns about its effects on fetal well-being, despite lim … t rate tracing. RESULTS: Five subjects underwent 44 electroconvulsive therapy sessions. Continuous fetal monitoring was performed on 34 of the sessions. Transient fetal heart rate decelerations occurred in 4 sessions, all self-resolved and none required intervention. CONCLUSION: This case series is the first to report the results of continuous FHR monitoring dur …

Simultaneous monitoring of separate fetal magnetocardiographic signals in twin pregnancy.

Fetal magnetocardiography (fMCG) allows the non-invasive recording of fetal cardiac electrical activity with increasing efficacy as gestation progresses. Many reports on the successful extraction of reliable fetal magnetocardiographic traces in singleton pregnancies exist in the literature, whereas there is only one report on the reconstruction of averaged fetal cardiac signals obtained in a twin pregnancy with the use of a double sensor array system. In this paper, we aimed at assessing the effectiveness of an ICA-based procedure to reconstruct the time course of fetal cardiac signals recorde

Gardnerella vaginalis-infected scalp hematoma associated with electronic fetal monitoring.

Infections in the neonate caused by the vaginal commensal Gardnerella vaginalis are rare and mostly consist of bacteremia. A 4130-g term neonate developed an infection of a scalp hematoma with G. vaginalis. The infection developed in association with electronic fetal monitoring. The only clinical sign was an increase in size of the hematoma.

[Fetal brain blood flow velocity monitoring in high risk pregnancies].

OBJECTIVES: Blood velocity in the fetal vein of Galen and straight sinus is normally even and without fluctuation. Transverse sinus blood velocity pattern shows a characteristic triphasic shape. The aim of this study was to establish whether blood flow velocity pulsations in the Galen vein, straight sinus and transverse sinus velocimetry in high-risk pregnancies are related to an adverse outcome. MATERIALS AND METHODS: The Galen vein, straight sinus and transverse sinus were located by color Doppler ultrasound in 110 pregnancies complicated by pregnancy-induced hypertension and intra-uterine

Uncertain value of electronic fetal monitoring in predicting cerebral palsy.

BACKGROUND: Electronic monitoring of the fetal heart rate is commonly performed, in part to detect hypoxia during delivery that may result in brain injury. It is not know whether specific abnormalities on electronic fetal monitoring are related to the risk of cerebral palsy. METHODS: Among 155,636 children born from 1983 through 1985 in four California counties, we identified singleton infants with birth weights of at least 2500 g who survived to three years of age and had moderate or severe cerebral palsy. The children with cerebral palsy were compared with randomly selected control children…

[Biophysical profile in prolonged pregnancy. Another alternative to fetal monitoring].

The value of the biophysical profile scoring to predict accurate perinatal outcome in prolonged pregnancy, was assessed. 60 patients with the diagnosis of prolonged pregnancy were included in this prospective clinical trial. A fetal biophysical profile score, described by Manning and modified by Johnson, was recorded in all these patients. There were 40 cases (66%) with normal profile scoring 3 of which had a feature considered as perinatal morbidity (specificity 94.8%). In contrast, from 20 cases with abnormal profile scores, 18 (90%) had abnormal perinatal findings (sensitivity 85.7%). The f

Electronic monitoring evidence of fetal distress in high-risk pregnancies.

The oxytocin challenge test (OCT) has been shown in other studies to be valuable in evaluating high-risk pregnancies. The purpose of this study was to show the relationship of various disease states and clinical conditions with OCT results and fetal performance in labor. Of a group of normal patients, 4% had positive OCTs or late decelerations (LDs) in labor. The incidence of positive OCTs or LDs in labor in patients with diabetes mellitus (DM) class B-R was 23.2%; in DM class A, 27.6%; in intrauterine growth retardation (IUGR), 26.2%; in pregnancy-induced hypertension (PIH), 27.6%; in chronic

Two cases of continuous fetal heart rate monitoring in twins.

This report describes two cases of twin pregnancy in which continuous heart rate monitoring of both twins was successfully carried out during labour. In both cases monitoring permitted early diagnosis of fetal distress and both mothers were delivered by caesarean section. Continuous fetal heart rate monitoring permits diagnosis of fetal distress in the second twin much earlier than conventional methods.

A prospective cohort study of fetal heart rate monitoring: deceleration area is predictive of fetal acidemia.

BACKGROUND: Intrapartum electronic fetal monitoring is the most commonly used tool in obstetrics in the United States; however, which electronic fetal monitoring patterns predict acidemia remains unclear. OBJECTIVE: This study was designed to describe the frequency of patterns seen in labor using modern nomenclature, and to test the hypothesis that visually interpreted patterns are associated with acidemia and morbidities in term infants. We further identified patterns prior to delivery, alone or in combination, predictive of acidemia and neonatal morbidity. STUDY DESIGN: This was a prospect…

Continuous cardiotocography (CTG) as a form of electronic fetal monitoring (EFM) for fetal assessment during labour.

BACKGROUND: Cardiotocography (known also as electronic fetal monitoring), records changes in the fetal heart rate and their temporal relationship to uterine contractions. The aim is to identify babies who may be short of oxygen (hypoxic), so additional assessments of fetal well-being may be used, or the baby delivered by caesarean section or instrumental vaginal birth. OBJECTIVES: To evaluate the effectiveness of continuous cardiotocography during labour. SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Group Trials Register (31 December 2012) and reference lists of retrieve…

Current Innovative Methods of Fetal pH Monitoring-A Brief Review.

In this study, we explore the "why?", and "how?", monitoring the pH of the fetal scalp is used, and show its limitations. In addition, we review the development of new devices based on the modern physics and nanomaterials serving this topic. Most of the works we found in our search have focused on improving the prognostic of fetal heart rate monitoring, because it is the "golden standard" in determining fetal distress. Although the best-known screening method, it can only provide limited information about the actual status of the fetus. The best predictive assessment, with the highest reproduc

Home Monitoring for Fetal Heart Rhythm During Anti-Ro Pregnancies.

BACKGROUND: Fetal atrioventricular block (AVB) occurs in 2% to 4% of anti-Ro antibody-positive pregnancies and can develop in <24 h. Only rarely has standard fetal heart rate surveillance detected AVB in time for effective treatment. OBJECTIVES: Outcome of anti-Ro pregnancies was surveilled with twice-daily home fetal heart rate and rhythm monitoring (FHRM) and surveillance echocardiography. METHODS: Anti-Ro pregnant women were recruited from 16 international centers in a prospective observational study. Between 18 and 26 weeks' gestation, mothers checked FHRM twice daily with a commercially

Prenatal ultrasound monitoring of homozygous α0-thalassemia-induced fetal anemia.

A noninvasive approach by serial ultrasound examination at 12-15, 18, and 30 weeks of gestation can be used to exclude homozygous α0-thalassemia-induced fetal anemia. At 12-15 weeks of gestation, the predictive values for the fetal cardio-thoracic ratio were better than that for the placental thickness. At 16-20 weeks of gestation, measuring middle cerebral artery peak systolic velocity is associated with a low false-positive rate. However, the false-positive rate of this noninvasive approach can be about 3%, requiring an invasive test to confirm the diagnosis. A false-negative may result in a

Does ST analysis have a place in electronic fetal monitoring?

Intrapartum cardiotocography (CTG) has a high false-positive rate. This has contributed to the rapid increase in obstetric interventions without a strong improvement in perinatal outcomes. We explore the role of ST analysis (STAN®) as an adjunct to CTG in identifying fetal asphyxia during labor. We conclude that STAN® reduces the rate of fetal blood sampling and instrumental vaginal deliveries, and has the potential to reduce the number of Australian operative interventions without compromising neonatal outcome.

System errors in intrapartum electronic fetal monitoring: a case review.

Intrapartum electronic fetal monitoring (EFM) interpretation and management continue to be a common issue in litigation involving adverse outcomes in term pregnancies. This article uses a case study approach to illustrate system errors related to intrapartum EFM. Common system errors related to use of intrapartum EFM include knowledge deficits, communication failures, and fear of conflict. Strategies for reducing error and the promotion of a patient safety approach to risk management in EFM are discussed, with an emphasis on the importance of a true team approach to EFM education, interpretati…
