OBJECTIVE: To evaluate all individual cases of dual twin demise following laser surgery for twin-twin transfusion syndrome (TTTS).
METHOD: This is an analysis of all monochorionic diamniotic twin gestations with TTTS complicated by dual demise following laser surgery from 2006 to 2019. Cases were reviewed by (1) a fetal surgeon researcher and (2) a panel of independent experienced maternal-fetal medicine specialists to code an etiology of demise for the donor and recipient, and to assess for possible preventability.
RESULTS: Of 753 twins that underwent laser surgery for TTTS, 52 (6.9%) had p

Twin-to-twin transfusion syndrome (TTTS) is characterized by an unbalanced blood transfer through placental abnormal vascular connections. Prenatal ultrasound (US) is the imaging technique to monitor monochorionic pregnancies and diagnose TTTS. Fetoscopic laser photocoagulation is an elective treatment to coagulate placental communications between both twins. To locate the anomalous connections ahead of surgery, preoperative planning is crucial. In this context, we propose a novel multi-task stacked generative adversarial framework to jointly learn synthetic fetal US generation, multi-class se

To evaluate the relative risk of placental abruption in monochorionic (MC) twin pregnancies complicated with twin-to-twin transfusion syndrome (TTTS) and treated with endoscopic laser coagulation of placental vessels (ELCPV). A retrospective analysis from January 2004 and December 2015 of 373 TTTS pregnancies, treated with selective ELCPV until January 2012 (287 cases), after which the Solomon technique was introduced (86 cases), compared with 243 normal MC pregnancies. A significant improvement in perinatal survival was observed after the introduction of the Solomon technique when compared to

Multiple gestation is associated with an increased risk for adverse pregnancy outcome. Monochorionic twins are at risk for complications specific to these pregnancies, such as twin-twin transfusion syndrome (TTTS) or twin reverse arterial perfusion (TRAP) sequence. In this article we give an overview on prenatal diagnosis, treatment and outcome of twin pregnancies complicated by TTTS and TRAP sequence.

We report a case of reversal of twin-to-twin transfusion syndrome (Stage IV TTTS) at 24 weeks of gestation after fetoscopic laser photocoagulation (FLP) for communicating vessels. The pathology of reversal of TTTS found that the 'initial' donor developed polyhydramnios whereas the 'initial' recipient developed oligohydramnios. With careful follow-up, the hydrops fetalis of the 'initial' recipient improved at 31 weeks of gestation. Because of the onset of labour pain at 34 weeks of gestation, two live and healthy babies were delivered by emergency caesarean section.

OBJECTIVE: The aim of this article is to investigate the prevalence, clinical presentation, prenatal management, and prognosis of mirror syndrome associated with twin-twin transfusion syndrome (TTTS) treated by amnioreduction or selective fetocide.
METHOD: A retrospective study of twin pregnancies with TTTS was conducted. The prevalence of mirror syndrome, defined as severe maternal edema related to fetal hydrops and placental edema, was calculated for TTTS, and data on clinical characteristics, treatment, and outcomes of the patients were reviewed.
RESULTS: We observed mirror syndrome in 4.

OBJECTIVE: This study was undertaken to determine whether laser thermocoagulation for twin-twin transfusion syndrome (TTTS) causes increased cell-free fetal DNA levels in maternal plasma, potentially as a result of placental injury.
STUDY DESIGN: We enrolled 34 patients with twin pregnancies complicated by severe TTTS who underwent fetoscopic selective laser ablation of placental vascular anastomoses. Blood samples were drawn before and sequentially after the procedure. Fetal DNA in maternal plasma was quantified by polymerase chain reaction amplification of a Y-chromosome sequence.
RESULTS:

OBJECTIVE: To determine whether there is a gender discrepancy in severe twin-twin transfusion syndrome.
METHODS: All cases of twin-twin transfusion syndrome evaluated between 1989 and 1996 were reviewed retrospectively. The following sonographic criteria were used: a single placenta, a thin membrane, the same gender, a combination of polyhydramnios-oligohydramnios, a stuck twin, and an estimated weight discordance exceeding 20%. At least five of six sonographic criteria were required for inclusion in the study. Only severe cases, which were defined as early onset (before 30 weeks' gestation),

In four twin pregnancies presenting with acute 2nd-trimester polyhydramnios due to presumed twin-twin transfusion syndrome, the fetal hemoglobin concentration was measured in samples obtained by cordocentesis. In no instance did the twin pair difference in the hemoglobin concentration exceed 2.7 g/dl.

Twin-twin transfusion syndrome (intrauterine parabiotic syndrome) is a potentially serious complication of monochorionic twinning. The frequency of fetal and neonatal death and perinatal morbidity is significantly higher than that in twin pregnancies uncomplicated by the syndrome. The authors present sonographic findings in ten cases of twin-twin transfusion syndrome proved by means of pathologic examination of the placenta and/or laboratory findings. The cases all involved diamniotic twins and covered a broad range of severity. Discordant fetal size and differing amniotic fluid volume in the

INTRODUCTION: The management of selective intrauterine demise in monochorionic pregnancies has high rates of adverse outcomes in the surviving co-twin. One of the complications might be severe anemia. The aim of this systematic review was to evaluate the role of intrauterine transfusion in case of selective intrauterine fetal death in monochorionic pregnancies with the surviving co-twin diagnosed with severe anemia.
METHODS: We carried out literature search from PubMed, Google Scholar, and Scopus. Cases with ultrasound signs of severe fetal anemia in the surviving co-twin and treated through

INTRODUCTION: Fetoscopy is a minimally invasive technique widely used for the treatment of twin-to-twin transfusion syndrome (TTTS), offering significant perinatal benefits. However, maternal complications of fetoscopy are underreported and incompletely characterized.
CASE PRESENTATION: We present the case of a 19-year-old primigravida with a monochorionic diamniotic pregnancy, diagnosed with Quintero stage II TTTS at 18 weeks of gestation, managed with fetoscopy and laser ablation. The clinical course remained stable until 30+3 weeks, 12 weeks after the fetoscopic procedure, when fetal demis

Twin Reversed Arterial Perfusion (TRAP) sequence is an infrequent complication of monochorionic pregnancies, characterized by retrograde perfusion of an acardiac twin via arterio-arterial and veno-venous placental anastomoses, leading to severe malformations in the acardiac twin and hemodynamic strain on the pump twin. This narrative review explores the pathophysiology, diagnostic criteria, and prognostic factors influencing management decisions, including acardiac-to-pump twin weight ratio, Doppler abnormalities, and early signs of cardiac dysfunction. Expectant management remains a viable op

Since a 2004 randomized trial established fetoscopy as the gold standard for the treatment for twin-to-twin transfusion syndrome (TTTS), advancements in surgical technique and expert training have considerably improved survival rates to 89% for at least one fetus and 65% for both fetuses. However, many challenges remain. This review provides an update on the current principles, issues, and debates. The Solomon technique has helped improve neonatal outcomes by reducing the risks of postoperative twin anemia-polycythemia sequence and TTTS recurrence. Nevertheless, fetoscopic laser photocoagulati

Twin-to-twin transfusion syndrome (TTTS) is a complex prenatal condition in which monochorionic twins experience an imbalance in blood flow due to abnormal vascular connections in the shared placenta. Fetoscopic laser photocoagulation is the first-line treatment for TTTS, aimed at coagulating these abnormal connections. However, the procedure is complicated by a limited field of view, occlusions, poor-quality endoscopic images, and distortions caused by artifacts. To optimize the visualization of placental vessels during surgical procedures, we propose Hybrid-MedNet, a novel hybrid CNN-transfo
