Delayed Presentation of Popliteal Artery Injury Following Tibia and Fibula Fractures: A Case Report.

Vascular damage linked to tibia and fibula fractures is rare; however, it poses a significant risk to limb viability. Disruption of the popliteal artery commonly happens at the time of injury; however, delayed arterial rupture may occur due to a migrated bone fragment, fixation hardware, or rupture of a pseudoaneurysm. We present the case of a 36-year-old man who developed acute, profuse hemorrhage six weeks after open reduction and internal fixation of a distal tibia fracture. The bleeding was caused by a laceration at the popliteal artery bifurcation resulting from a comminuted proximal fibu

Long-term outcome in operatively and non-operatively treated isolated type B fibula fractures.

PURPOSE: Type B fibula fractures are the most common type of ankle fractures. Generally, surgical repair is advised for unstable fractures and non-operative treatment for stable fractures. However, evidence on long-term functional outcome of both treatment regimens is lacking. Aim of this study is to compare the long-term outcome in function and pain between patients with an isolated type B fibula fracture treated non-operatively and surgically. MATERIAL & METHODS: In this retrospective cohort study, all consecutive patients aged between 18 and 75 years, treated non-operatively or surgically …

Fixation of osteoporotic distal fibula fractures: A biomechanical comparison of locking versus conventional plates.

The increased use of locking plates to treat difficult fracture scenarios has been advocated in patients that have reduced bone mineral density. One of these difficult fracture patterns, fixation of the distal fragment of a distal fibula fracture, may depend on unicortical and cancellous bony purchase. This study investigated the construct stiffness afforded by using locking and conventional plating schema in a cadaveric model. Overall, the data indicate that a locking plate construct with two distal unicortical screws was mechanically equivalent to standard plating with 3 distal screws. In ad

Salvage technique for treatment of periplafond tibial fractures: the modified fibula-pro-tibia procedure.

We describe a technique of fixation for treatment of distal tibia periplafond fractures and nonunions that uses a modification of the principle of the fibula-pro-tibia procedure (fusing the tibia and fibula together to create a one-bone lower leg). The fibula is plated, and the screws are brought across to the medial tibial cortex. The procedure is accomplished with or without a tibial buttress plate and always includes iliac crest bone grafting of the nonunion site and synostosis. We have used this technique in five patients with satisfaction.

A Tillaux Fracture and Concurrent Nondisplaced Salter-Harris Type III Fracture of the Distal Fibula: A Case Report.

The Salter-Harris classification system categorizes pediatric fractures in relation to the physis. A Salter-Harris type III fracture occurs from the physis extending to the epiphysis. Tillaux fractures are a type of Salter-Harris type III fracture that occurs due to incomplete fusion of the growth plate and includes the anterolateral tibial epiphysis. This specific fracture is unique to adolescents due to the anterior tibiofibular ligament's strength in relation to the growth plate, causing avulsion of the tibial fragment. The settings for a Tillaux fracture and a Salter-Harris type III fractu

Surgeon-Administered Ultrasound-Guided Regional Anesthesia in Fixation of Distal Fibula Fracture.

Distal fibula fracture is a common injury of the lower limb. Significantly displaced distal fibula fracture is treated with surgical fixation under general, spinal, or regional anesthesia. We present a case of displaced distal fibula fracture with both the regional anesthesia and operation performed by the same attending orthopedic surgeon. The patient underwent successful ultrasound-guided regional anesthesia as well as open reduction and internal fixation. This case report highlights the technical detail for ultrasound-guided regional anesthesia and surgical fixation by surgeon.

Delayed pathologic tibial fracture with chronic osteomyelitis after fibula free flap.

The reported donor site morbidity of the fibula free flap (FFF) is low; however, several uncommon complications have been reported with tibia fracture rarely being reported. We present a case of a pathological tibial fracture in the setting of chronic osteomyelitis after FFF. A 54-year-old female presented with a benign fibro-osseous lesion of the right mandible and was treated with mandibulectomy and reconstructed with a left FFF. Approximately 1 year following surgery, the patient presented to the emergency department. Imaging showed a pathological fracture of the distal third of the tibial

Stress fracture of proximal tibia after proximal fibula osteotomy: A case report.

INTRODUCTION: Proximal fibula osteotomy (PFO) is a new method for treating medial compartment osteoarthritis of the knee, which is based on the theory of differential settlement (nonuniform settlement). This procedure has been widely recognized for its advantages of relative simplicity, low rate of postoperative complications, and low postoperative costs. Stress fracture of the proximal tibia after PFO has not been previously reported. CASE PRESENTATION: We report a 62-year-old woman with chronic rheumatoid arthritis (RA) underwent left PFO for chronic knee pain, who developed a stress fractu

Traumatic tibia and fibula fracture in a 36 weeks' pregnant patient: a case report.

BACKGROUND: Bone fracture management in third-trimester pregnant patients is rare and poorly discussed in the literature. In the case of fractures that require orthopedic surgery in near-term pregnant women, clinicians should decide between operating before or after the delivery, carefully evaluating the health of the mother and fetus. CASE REPORT: A pregnant 41-year-old woman at 36 weeks' gestation had a traumatic midshaft displaced tibia and fibula fracture. A multispecialty team approach resulted in nonoperative treatment until delivery. The sudden spontaneous premature rupture of membrane

Irreducible tibial pilon fracture caused by incarceration of the fibula in the tibial medullary canal.

Fractures can be irreducible for several reasons, including soft tissue or bone fragment interposition. We report an unusual fracture configuration of a comminuted tibial pilon fracture in which the distal fibular shaft fragment was occupying the medullary canal of the proximal tibial shaft fragment and inhibiting reduction and fixation. To the best of our knowledge, this has not been previously reported in a published study.

[Possible nerve injuries in ankle dislocations. Trimalleolar fracture including the proximal fibula].

We present a patient with an exceptional fracture type--a Maisonneuve fracture associated with a lateral ankle fracture--and the therapy of this injury. The Maisonneuve fracture consists of a proximal fibular fracture which is associated with failure of the deltoid ligament or medial malleolus and diastasis of the distal tibiofibular syndesmosis. Diverse nerves of the lower leg can be affected by severe trauma as well as by fracture-associated complications or an operation. Fracture fragments, an excessive distension caused by the trauma, or a fracture-associated hematoma can cause injury to t

Reconstruction of a pathologic fracture following osteomyelitis of the mandible using a fibula osteocutaneous flap.

The use of a fibula osteocutaneous flap is currently the mainstay of segmental mandibular reconstruction. This type of flap is used to treat tumors, trauma, or osteoradionecrosis of the mandible. However, a fibula osteocutaneous flap may also be a good option for reconstructing the mandible to preserve oropharyngeal function and facial appearance in cases of pathological fracture requiring extensive segmental bone resection. Chronic osteomyelitis is one of the various causes of subsequent pathologic mandibular fractures; however, it is rare, and there have been few reports using free flaps in

Distal fibula fracture fixation: Biomechanical evaluation of three different fixation implants.

BACKGROUND: The goal of this study was to evaluate the biomechanical performance of three distal fibula fracture fixation implants in a matched pair cadaveric fibula model: (1) a 5-hole compression plate with lag screw, (2) a 5-hole locking plate with lag screw, and (3) the 6-hole tabbed-plate with locking screws. METHODS: Three-dimensional motions between the proximal and distal fibular segments were measured under cyclic valgus bending, cyclic compressive axial loading, and cyclic torsional external-rotation loading. During loading, strains were measured on the surfaces of each fibula near

A biomechanical evaluation of locked plating for distal fibula fractures in an osteoporotic sawbone model.

BACKGROUND: Supination external rotation (SER) injuries are commonly fixed with a one third tubular neutralization plate. This study investigated if a combination locked plate with additional fixation options was biomechanically superior in osteoporotic bone and comminuted fracture models. METHODS: Using an osteoporotic and a comminuted Sawbones model, SER injuries were fixed with a lag screw for simple oblique fibula fractures, and either a one third tubular neutralization plate or a locking plate. Samples were tested in stiffness, peak torque, displacement at failure, and torsion fatigue. …
