Drug Costs in Federally Sponsored Cancer Clinical Trials: A Systematic Review.

IMPORTANCE: Federally sponsored cancer clinical trials (FS-CCTs) address research questions often overlooked by industry (eg, pharmaceutical or biotechnology companies), such as treatment combinations, rare cancers, within-class comparisons, and de-escalation strategies. However, drug costs in these trials have not been comprehensively quantified. Federal agencies typically support trial infrastructure but not drugs, necessitating cosponsorship or in-kind donations from pharmaceutical companies. These arrangements often involve complex contractual processes that may delay trial initiation. Qua

Lessons learned: Infrastructure development and financial management for large, publicly funded, international trials.

BACKGROUND/AIMS: Randomized clinical trials are widely recognized as essential to address worldwide clinical and public health research questions. However, their size and duration can overwhelm available public and private resources. To remain competitive in international research settings, advocates and practitioners of clinical trials must implement practices that reduce their cost. We identify approaches and practices for large, publicly funded, international trials that reduce cost without compromising data integrity and recommend an approach to cost reporting that permits comparison of cl

Kenya: the development of private services and the role of the Kenya Veterinary Association.

Private veterinary practice has existed in Kenya for more than half a century. Between the early 1930s and the mid-1960s, provision of clinical and advisory services almost entirely involved servicing commercial ranches and dairy farms. The Department of Veterinary Services (VSD) was mainly responsible for providing regulatory services in these areas. Until the mid-1960s, public sector veterinary responsibilities were predominantly associated with the prevention of notifiable diseases outside the commercial farming areas. In a major agrarian reform programme initiated in 1954, Kenya initiated

Global Health Governance and the WHO Pandemic Agreement: A Scoping Review of Challenges and Analysis of Reforms.

BACKGROUND: The COVID-19 pandemic exposed persistent weaknesses in global health governance, particularly in preparedness, equity, and accountability. The WHO Pandemic Agreement, adopted in May 2025, aims to address these systemic gaps through a binding international framework. OBJECTIVE: To identify key challenges in global pandemic preparedness and health governance reported in the literature (2019-2024) through a systematic scoping review, and to evaluate how these challenges are addressed in the provisions of the WHO Pandemic Agreement via qualitative document analysis. METHODS: Using Jo

Financing Universal Healthcare Coverage in Nigeria Through the Basic Health Care Provision Fund: Challenges and Potential Solutions.

This study examines the Nigerian government's efforts to provide Universal Health Care (UHC) and reduce out-of-pocket spending through the Basic Health Care Provision Funds (BHCPF) scheme. The study gathered insights into the degree of UHC accomplishment through the implementation of the BHCPF using qualitative and quantitative secondary data analysis approaches, which combined a series of interviews with Statista data. The findings show that BHCPF implementation must be better aligned with the needs of healthcare recipients, particularly at the local government level, with poor infrastructure

The Nigerian Emergency Care System: Structural Challenges, Workforce Constraints, and Financing Gaps.

Emergency Medical Services (EMS) are a critical component of resilient health systems, enabling timely response to trauma, acute medical conditions, and obstetric emergencies. Despite Nigeria's substantial burden of preventable emergency-related morbidity and mortality, the development of a coherent and fully functional EMS system remains incomplete. Existing initiatives, including the National Emergency Medical Service and Ambulance System (NEMSAS), have demonstrated policy intent but continue to face persistent structural constraints. This narrative review synthesised peer-reviewed literatur

Who will pay for the malaria vaccines in Africa?

Malaria remains one of the most persistent infectious threats in sub-Saharan Africa, where the burden of disease and mortality are disproportionately concentrated among children under five years of age. The World Health Organization's endorsement of the RTS, S/AS01 vaccine in October 2021 and the R21/Matrix-M vaccine in October 2023 represents a major scientific advance; however, the central question confronting African policymakers, donors, and governments is no longer scientific but financial. In 2024, an estimated 282 million malaria cases and 610,000 deaths occurred worldwide, with the Afr
