The paper presents a good cosmetic and functional result of surgical treatment of distal interphalangeal joint dislocation of the annular finger in a 14 year old boy. The mechanism and pathology of this rare lesion of the hand are discussed. Factors affecting final outcome are: preserved joint cartilage, careful surgical approach and proper post-op treatment.

In the years 1981 to 1987, 50 patients had 59 replacements of the finger joints. Of these, 41 patients with 49 joint operations were reviewed, with follow-up ranging from two to eight years (average 32 months). About 80% of these patients were satisfied that silastic implant arthroplasty had relieved their pain and stiffness and improved function of the finger and hand. They were satisfied that this procedure was preferable to amputation or fusion. Success or failure was assessed not only on the range of movement of the joint being replaced, but on relief of pain, correction of deformity, stab…

The results of treatment of severe injuries to the proximal interphalangeal joint are unsatisfactory. The methods of joint reconstruction are discussed, including fusion, implant arthroplasty, perichondrial grafting and vascularized joint transfer. A patient is presented with a severe crush injury to the dorsum of the index finger with loss of skin and extensor tendon and proximal interphalangeal joint disruption. Immediate reconstruction of the finger is described using a composite free flap of skin, extensor tendon and proximal interphalangeal joint from the second toe. Follow-up at two year

Distal interphalangeal joint arthrodesis is a frequent surgical operation performed to treat severe arthritis. Nevertheless, the angle selected when fusing the joint is arbitrarily chosen without any quantified data concerning its mechanical effects, thus preventing the optimal choice for the patient. In the current study, we realized an experiment and developed a numerical model to investigate the effect of fusion angle on the biomechanics of adjacent non-operated joints. Six participants performed a pinch grip task while arthrodesis was simulated with a metal splint. Kinematic and force data

The best treatment for mallet fingers is still a matter of debate. Numerous splints with different designs to keep the distal interphalangeal (DIP) joint in extension have been described in literature. The outcomes of splint treatment are generally good with occasional reports of minor skin complications. Percutaneous Kirschner-wire pinning of the DIP joint for closed tendinous mallet finger represents a alternative treatment modality that reliably immobilises the joint and does not need much patient compliance or use of an external splint. We report a rare but devastating complication of perc

When severe proximal interphalangeal (PIP) joint flexion contracture is induced in the little finger by Dupuytren's disease, it interferes with activities of daily living. To extend the little finger, open fasciectomy is selected as a general treatment method. However, postoperative complications have been frequently reported. To solve these problems, finger shortening was undertaken. In this study, we treated two cases of Dupuytren's disease manifesting severe PIP joint flexion contracture of the little finger with finger shortening by proximodistal interphalangeal (PDIP) fusion in which the

BACKGROUND: Children with cerebral palsy (CP) exhibit impaired selective motor control (SMC) that contributes to poor hand function, but current clinical assessments lack the sensitivity to detect finger- and joint-specific deficits to guide rehabilitation strategies. This study aimed to determine the internal consistency and validity of an objective, instrumented assessment of selective finger control (individuation) in children with CP, and to examine its relationship to clinical measures of upper limb function. METHODS: A custom-designed device recorded three-dimensional isometric forces co

INTRODUCTION: Trigger finger is a common and functionally limiting disorder. Finger immobilization using an orthotic device is one of the conservative treatment options for treating this condition. The most common orthosis previously described for trigger finger is metacarpophalangeal joint immobilization. There are limited studies describing the effectiveness of proximal interphalangeal joint orthosis for treatment of trigger finger.
METHODS: This study was a single group pretest-posttest design. Adult patients with single digit idiopathic trigger finger were recruited and asked to wear a fu

An approach to finger motion capture that places fewer restrictions on the usage environment and actions of the user is an important research topic in biomechanics and human-computer interaction. We proposed a system that electrically detects finger motion from the associated deformation of the wrist and estimates the finger joint angles using multiple regression models. A wrist-mounted sensing device with 16 electrodes detects deformation of the wrist from changes in electrical contact resistance at the skin. In this study, we experimentally investigated the accuracy of finger joint angle est…

Mostly mallet finger can be treated conservatively. In cases of fracture of the distal phalanx open reduction and fixation is recommended. We favour the well-established cerclage, yet perform it in a more comfortable way with the use of cannulated drilling. This technique was applied on 13 patients even with lesions several weeks old. The evaluation 5-16 months postoperatively based on radiological, clinical and subjective criteria. We present the technique and the results followed by the discussion of the advantages over conservative and other operative managements.

This is a case of right ring and little finger carpometacarpal joint dislocation managed with conservative care following early presentation and accurate identification of the injury. It is a rare injury that can be easily missed with the need for operative intervention if there is a delayed diagnosis. The dislocation was reduced and retained with an ulnar gutter splint using plaster of Paris extending from the mid-forearm to the metacarpophalangeal joints. Caution and repeated radiological assessment are advised to ensure that the reduction has been maintained. Follow-up at one week and four

Currently Kaplan's criteria on examination are used as the gold standard diagnostic tool for complex dorsal dislocations of the index finger Metacarpophalangeal joint. Our case report reveals a situation when the diagnosis was obscure and when conventional X-rays were not beneficial. With the use of CT imaging we were able to confirm the diagnosis of complex dorsal dislocation of the index finger Metacarpophalangeal joint and also reveal multiple other injuries. We feel that CT imaging should be performed on every individual who has been diagnosed with this injury or where the diagnosis cannot

BACKGROUND: In the operative treatment of Dupuytren's disease, in certain cases proximal interphalangeal joint flexion contracture remains after fasciectomy due to shrinkage, shortening, and/or adhesion of the periarticular structures.
OBJECTIVES: How can a residual flexion contracture of the proximal interphalangeal joint after partial fasciectomy in Dupuytren's disease be treated surgically and what follow-up results can be expected?
METHODS: Description of anatomy, indication, surgical technique of arthrolysis of the proximal interphalangeal joint, postoperative treatment, and critical an

INTRODUCTION: Joint reconstruction following trauma at the proximal interphalangeal or metacarpophalangeal levels remain a difficult problem in hand surgery. Function of the injured finger depends on the mobility and stability at these joints. In fingers amputated at these levels with joint destruction, the hand surgeon can perform either an arthrodesis or a replacement arthroplasty (either as an emergency or as a secondary procedure). Arthrodesis will give a stable pain-free joint, but at the cost of sacrificing mobility and even cosmesis.
CLINICAL PICTURE: We report a case of traumatic ampu
