Fluid therapy of acute brain edema in children.

The clinical experience with a new fluid therapy in children with acute brain edema complicated by infectious disease is reported. The clinical data of a retrospective group of 192 patients and a prospective study of 1,302 and 2,279 patients is summarized. One method of fluid therapy for children with acute brain edema is traditional; fluid intake is restricted to less than 1,200 mL/m(2) daily (60 mL/kg daily). Another method is the new fluid therapy regimen used in our prospective study, in which dehydration and fluid replenishment are individualized. On the first day the fluid intake of pati…

Selecting and managing fluid therapy. Colloids versus crystalloids.

The critical care nurse is responsible for monitoring the effects of parenteral therapy. If the nurse is to collaborate with the physician on the type of solution appropriate for the patient, it is imperative that the nurse understand how the solution will impact the patient. Only through comprehending the effects and properties of each solution can the nurse provide quality care and ensure desirable patient outcomes.

Fluid therapy for alimentary disease: origins and objectives.

Fluid therapy took nearly a century to gain acceptance in human medicine, despite excellent early work demonstrating its rationale and its effectiveness. Progress in veterinary fluid therapy lagged behind, partly because of skepticism and partly because of real practical difficulties. From the earliest to the most recent developments, human cholera has provided the impetus for much of the progress in fluid therapy. The challenge of treating such a severe diarrhoea in primitive surroundings with severe limits on cost and supervision has led to the emergence of oral fluid therapy as the prime te…

Fluid and Electrolyte Therapy in the Patient with Cardiac Disease.

Using a framework of understanding the pathophysiology of heart disease, this article discusses considerations for prescribing fluids in patients with cardiac disease as well as those with heart failure. It highlights the importance of recognizing electrolyte and volume abnormalities, which are common in congestive heart failure, and the need for goal-directed, individualized fluid therapy to maintain balance and prevent worsening of heart failure or renal impairment. The article covers fluid management strategies, including resuscitation, rehydration, maintenance, and considerations during co…

Fluid and Electrolyte Therapy in the Patient with Kidney Injury.

In small animals with kidney injuries, fluid therapy is the cornerstone of supportive care. Although fluids do not repair kidney damage, appropriate therapy can stabilize the patient, correct fluid, electrolyte, and acid-base imbalances, and provide vital support. The primary goals are to rescue the animal, optimize systemic perfusion and renal hemodynamics, and restore hydration without causing fluid overload. Isotonic crystalloids are the fluids typically administered first, but fluid therapy of the animal with kidney injury commonly must be individually adjusted to meet unpredictable and ch…

An international comparison of the cost of fluid resuscitation therapies.

OBJECTIVE: Fluid resuscitation is a ubiquitous intervention in the management of patients treated in the intensive care unit, which has implications for intensive care unit resourcing and budgets. Our objective was to calculate the relative cost of resuscitation fluids in several countries to inform future economic evaluations. METHODS: We collected site-level data regarding the availability and cost of fluids as part of an international survey. We normalised costs to net present values using purchasing power parities and published inflation figures. Costs were also adjusted for equi-effectiv

A physiological approach to fluid and electrolyte therapy in the horse.

In this article a physiological approach to fluid therapy is discussed, commencing with examination of fluid distribution in the normal horse. The functions of individual plasma electrolyte concentrations are considered and practical causes of acid-base disturbances discussed. When fluid administration is necessary, selection of the route for fluid administration as well as the type of fluid are important considerations and these must be adjusted to the needs of the individual case. Balanced polyionic solutions appear to be most suitable for general use because normal saline can produce hypoka…

Fluid Management: Pharmacologic and Renal Replacement Therapies.

OBJECTIVES: Focusing on critically ill children with cardiac disease, we will review common causes of fluid perturbations, clinical recognition, and strategies to minimize and treat fluid-related complications. DATA SOURCE: MEDLINE and PubMed. CONCLUSIONS: Meticulous fluid management is vital in critically ill children with cardiac disease. Fluid therapy is important to maintain adequate blood volume and perfusion pressure in order to support cardiac output, tissue perfusion, and oxygen delivery. However, fluid overload and acute kidney injury are common and are associated with increased mor…

Perioperative fluid management in pancreaticoduodenectomy in the era of goal-directed fluid therapy: a review of the literature.

BACKGROUND: Pancreaticoduodenectomy (PD) is the only curative option for localized pancreatic head and periampullary malignancies. However, … alance between restrictive fluid administration and physiologic demand. CONCLUSIONS: Goal-directed fluid therapy protocols offer a promising approach to PD fluid management by individualizing fluid delivery, minimizing complications, and improving perfusion metrics, resulting in a more effective restrictive fluid approach. Despite these advancements, heterogen …

Comparison of Goal-Directed Fluid Therapy and Conventional Fluid Therapy in Elective Major Abdominal Surgery: A Meta-Analysis of Randomized Controlled Trials.

Goal-directed fluid therapy (GDFT) has emerged as an important perioperative strategy aimed at optimizing hemodynamic status and improving surgical outcomes; however, evidence regarding its effectiveness in patients undergoing major abdominal surgery remains variable. This meta-analysis was conducted to evaluate the impact of intraoperative GDFT compared with conventional fluid therapy on postoperative morbidity and clinical outcomes in adult patients undergoing elective major abdominal surgery. Randomized controlled trials comparing GDFT with conventional intraoperative fluid therapy were ana…

[Research advances in fluid therapy for severe acute pancreatitis].

Severe acute pancreatitis (SAP) is a condition with high mortality, characterized by persistent (>48 hours) organ dysfunction. Fluid therapy serves as the cornerstone of acute-phase management in SAP, crucial for reducing morbidity and mortality. In recent years, significant research progress has been made in the treatment concepts, methods, and strategies of fluid management for SAP patients, contributing to improved patient outcomes. Based on an understanding of the pathophysiological characteristics of SAP, it is important to recognize the significance of individualized fluid management str…
