An Elemental Diet Reduces Chylous Ascites of Patients Undergoing Pancreatoduodenectomy in the Setting of Early Enteral Feeding.

OBJECTIVES: The elemental diet (ED) is a formula to support nutritional status without increasing chylous burden. This study evaluates the efficacy of early ED feeding after pancreatoduodenectomy (PD). MATERIALS AND METHODS: A prospective phase II study of consecutive patients who underwent PD with early ED feeding was conducted. Patient backgrounds, surgical outcomes, and ED feeding tolerability were compared with a historical cohort of 74 PD patients with early enteral feeding of a low residue diet (LRD). RESULTS: The ED group comprised 104 patients. No patient in the ED group discontinued

Elemental diet preventative effects for adverse events during chemotherapy in patients with esophageal cancer - A systematic review and meta-analysis.

The effectiveness of an elemental diet (ED) for preventing adverse events (AEs) during chemotherapy for patients with esophageal cancer (EC) remains unclear. The aim of this meta-analysis was to comprehensively assess the efficacy of ED for preventing AE in EC patients during chemotherapy. Medline (via PubMed), Embase, the Cochrane Library, and Web of Science were searched to retrieve prospective and randomized studies published before April 12, 2023. The odds ratio (OR) of each AE was calculated using Review Manger 5.4.1. The risk of bias was assessed, and a random effect model-based meta-ana

Thickness of commercial blenderized formulas adversely affects successful delivery via enteral feeding pumps.

BACKGROUND: There has been a renewed focus on offering commercial formulas made with real-food ingredients because of their perceived health benefits, such as improved feeding tolerance and gut health. Children receiving enteral nutrition through these formulas often are fed via feeding pumps. Because these formulas vary in thickness, we aimed to explore the relationship between formula thickness and prescribed formula delivery via feeding pumps. We hypothesized that inaccurate volumes of commercial blenderized formula (CBF) are delivered via feeding pumps and that these inaccuracies are direc

Introduction of blended diet for enteral tube feeding in paediatrics: A case report.

Background: Advances in medical technology have resulted in changes to enteral tube feeding (ETF). Standard practice in the UK is to use commercial formula for ETF via the stomach or small intestine; however, blended diets are becoming increasingly popular. Emerging evidence shows benefits of a blended diet although evidence informing clinical guidance is sparse. Consequently, there is disparity in practice, and it remains unclear which patient groups may benefit most from a blended diet. Aim: To demonstrate the initiation of a blended diet in clinical practice. Methods: This case report featu

Practical approach to paediatric enteral nutrition: a comment by the ESPGHAN committee on nutrition.

Enteral nutrition support (ENS) involves both the delivery of nutrients via feeding tubes and the provision of specialised oral nutritional supplements. ENS is indicated in a patient with at least a partially functioning digestive tract when oral intake is inadequate or intake of normal food is inappropriate to meet the patients' needs. The aim of this comment by the Committee on Nutrition of the European Society for Pediatric Gastroenterology, Hepatology, and Nutrition is to provide a clinical practice guide to ENS, based on the available evidence and the clinical expertise of the authors. Omegaven for the treatment of parenteral nutrition associated liver disease: a case study.

A 17-month-old infant diagnosed with Short Gut Syndrome developed severe cholestasis and hepatopathy due to chronic parenteral nutrition (PN). In an effort to prevent multivisceral organ transplantation, he was started on Omegaven, an experimental alternative form of parenteral lipid. Within six months, his cholestasis had resolved and his hepatopathy had significantly improved. Omegaven uses Omega-3 rather than Omega-6 triglycerides as its primary fat source, improving fat absorption and modulating the systemic inflammatory response. It has also shown promise as a means of alleviating PN-indu

Elemental diet in the treatment of refractory coeliac disease.

A minority of patients with coeliac disease do not improve in spite of strictly adhering to gluten-free diet and are considered to have developed refractory disease. Such patients are usually treated with steroid and/or immunosuppressive agents but these are not without serious side effects. Herein, we present a patient with refractory coeliac disease in whom remarkable clinical and histological improvement was achieved on elemental diet. Elemental diet should be considered as a treatment option in such patients.

Diet and the irritable bowel syndrome.

Food intake plays a key role in triggering or perpetuating symptoms in patients with IBS. Evaluation of the impact of diet in the individual patient requires a precise dietary history and a 7-day prospective dietary analysis, which should include the quality and quantity of food consumed, chronologic sequence and nature of symptoms, and the frequency and consistency of bowel movements. The caloric density of the meal, total fat intake, the quantity and quality of lactose-containing foods, sorbitol, fructose, and the nature and quantity of soluble and insoluble fiber intake must be noted. Analysis of selenium content in commercial dietetic products.

Selenium (Se) is largely associated with the protein component of foods. The processing of elemental feeds and artificial formulations used in clinical dietetic practice can, therefore, result in significant losses of Se from these products and the Se status of patients consuming these feeds may be compromised. A selection of commercial formulas typically used for enteral feeding, and for patients with cystic fibrosis, phenylketonuria and maple syrup urine disease were analysed for Se micro-fluorimetrically. Low Se contents were found in all food supplements except for Peptisorbon (0.09 mg Se/

Dermatitis herpetiformis: consequences of elemental diet.

The administration of an Elemental Diet to 5 patients with dermatitis herpetiformis, requiring high doses of Dapsone (diaminodiphenylsulfone, DDS), showed a rapid and beneficial effect on the skin lesions within two weeks. This effect was not influenced by simultaneous gluten challenge in one patient. A possible explanation is a reduction in the amount of harmful immune complexes due to the elimination of proteins from the diet. Subsequent introduction of a more comprehensive diet led to an increase of the minimal effective dose of Dapsone. These results underline the importance of dietary inf

Twenty-three adult patients with protein-calorie malnutrition were referred for outpatient tube feeding. Initial nutritional support with hypercaloric supplemented diets did not prevent further catabolism and weight loss, as the mean protein intake was 35.2 g/24 hr and the mean calorie intake was 844 cal/24 hr. Outpatient tube feeding was then initiated in deliver a mean of 78.5 g protein/24 hr and 2248 cal/24 hr. Patients on tube feeding demonstrated an increase of serum albumin and total body weight compared to diet therapy alone. The patient and family members were instructed in the techniq

The role of technology in modern nutrition.

The three basic nutritional problems facing the world today are the over-intake of foods by individuals or population segments, nutrient deficiencies in the diet of population groups, and food shortages. In all three cases the food scientist has a role to play but it is stressed that close co-operation between the nutritionist, the agricultural scientist and the food technologist is essential. To alleviate the problem of food shortages, consideration should be given to unconventional food resources. The shortage of trained food scientists in South Africa detrimentally affects research and deve

Older adults and patients in need of nutritional support: review of current treatment options and factors influencing nutritional intake.

BACKGROUND & AIMS: Many older adults and patients do not achieve sufficient nutritional intake to support their minimal needs and are at risk of, or are suffering from, (protein-energy) malnutrition. Better understanding of current treatment options and factors determining nutritional intake, may help design new strategies to solve this multifactorial problem. METHODS: Medline, Science Citation Index, ScienceDirect and Google databases (until December 2008) were searched with the keywords malnutrition, elderly, older adults, food intake, energy density, variety, taste, satiety, and appetite.

Systematic review of safety and tolerability of a complex micronutrient formula used in mental health.

BACKGROUND: Theoretically, consumption of complex, multinutrient formulations of vitamins and minerals should be safe, as most preparations contain primarily the nutrients that have been in the human diet for millennia, and at safe levels as defined by the Dietary Reference Intakes. However, the safety profile of commercial formulae may differ from foods because of the amounts and combinations of nutrients they contain. As these complex formulae are being studied and used clinically with increasing frequency, there is a need for direct evaluation of safety and tolerability. METHODS: All known

Metabolic alkalosis due to the use of an oligoantigenic diet in infancy.

A 7-month-old boy on an oligoantigenic diet because of multiple food intolerances presented with anorexia, failure to gain weight and severe hypochloremic metabolic alkalosis with hyperreninemia. Clinical symptoms and biochemical abnormalities disappeared after adequate dietary supplementation with potassium and sodium chloride. This case emphasizes that minimal daily mineral requirements must be provided in infant diets, and highlights the risk of nutritional deficiencies inherent in the prolonged use of oligoantigenic diets not adequately supplemented.

Cow milk protein allergy during the first year of life: a 12 year experience at the children's hospital, Bangkok.

CMA should be suspected for patients aged less than one year who had persistent diarrhea and/or hematemesis with no enteric pathogen found. Confirmed diagnosis could be made by Goldman challenge test. Patients with confirmed CMA should be treated by changing the cow milk feeding to soy milk feeding. However, in our study, 17% of CMA patients were also allergic to soy protein. Thus the soy milk was replaced by the elemental formula for successful treatment of this group of patients. Beside persistent diarrhea, hematemesis, anemia and hypoalbuminemia were other possible findings among patients w

[Neonatal hyperbilirubinemia of inadequately breast-fed infants and the effect of formula supplementation].

One hundred and fifty full-term breast-fed infants were analysed for their oral intake in the first 6 days. Fifty infants with less than 330 ml/kg/6 days breast feeding were defined as inadequately breast-fed infants. Twenty inadequately breast-fed infants without formula supplementation had significantly lower total fluid intake, lower total calorie intake, more body weight loss and significantly higher rates of hyperbilirubinemia and requirement of phototherapy when compared with the control group of 100 infants with more than 330 ml/kg/6 days breast feeding. On the other hand, 30 inadequate

What Are We Actually Feeding ICU Patients? The Case for Studying Formula Composition.

Large randomized controlled trials testing the quantity and timing of enteral nutrition (EN) in critically ill patients have consistently failed to demonstrate benefit. We propose that these negative results may partly reflect an unexamined variable: the composition of EN formulas themselves. Most widely used formulas rely on corn-derived simple carbohydrates, isolated functional fibers, dairy-based proteins, and omega-6-rich vegetable oils, ingredients selected largely for manufacturing efficiency and shelf stability rather than clinical evidence. These components bear little resemblance to w
