Machine learning models for predicting metabolic syndrome to support clinical decision-making in ART-treated adults living with HIV.

BACKGROUND: Metabolic syndrome (MetS) is an emerging complication among people living with HIV (PLHIV) receiving long-term antiretroviral therapy (ART), particularly with protease and integrase inhibitor regimens. Early identification of high-risk individuals remains challenging, and predictive tools are limited in African settings. This study evaluated nine machine learning (ML) algorithms for predicting MetS in ART-treated adults. METHODS: We analysed a retrospective cohort of 1,027 PLHIV without baseline MetS, followed for 144 weeks; 854 with complete data were included. Nine ML algorithms

Development and evaluation of an explainable machine learning selection pipeline for predicting neoadjuvant chemoradiotherapy response in locally advanced rectal cancer.

BACKGROUND: The selection of appropriate machine learning (ML) methods for clinical research remains challenging, particularly when both predictive performance and model explainability are required in small-sample datasets. Conventional approaches often rely on limited variables and expert-driven choices, whereas many ML models remain difficult to justify clinically. This study aimed to develop an explainable ML model selection pipeline and demonstrate its application in predicting neoadjuvant chemoradiotherapy (nCRT) response in locally advanced rectal cancer (LARC). METHODS: We proposed a s

A prospective cohort study develops and validates a machine learning model for predicting ecchymosis after total knee arthroplasty.

Total knee arthroplasty (TKA) is a standard procedure for end-stage knee diseases, yet perioperative bleeding and its complications, such as ecchymosis, remain significant challenges. Predicting ecchymosis can guide personalized perioperative blood management, enhancing patient recovery and reducing the socioeconomic burden. In this prospective study (June 2023-March 2024), 416 TKA patients were split into ecchymosis (n = 135) and non-ecchymosis (n = 281) groups. The first seven months' data (312 patients) were used for training, and the next three months' (104 patients) for validation. Featur

Explainable Artificial Intelligence Unravels the Possible Distinct Roles of VKORC1 and CYP2C9 in Predicting Warfarin Anticoagulation Control.

Background: Warfarin pharmacogenomics is critical due to its narrow therapeutic index and significant interpatient variability. While machine learning (ML) can predict anticoagulation control status (ACS), its "black-box" nature limits clinical translatability. Explainable Artificial Intelligence (XAI) addresses this by providing interpretable insights. This study applied ML and XAI to a warfarin pharmacogenomic dataset to predict poor ACS and explain model decisions. Methods: A post hoc analysis was conducted on a cross-sectional dataset of 232 patients receiving warfarin for ≥6 months. Data

Interpretable four-class machine learning prediction of initial I-131 therapy responses in differentiated thyroid cancer.

BACKGROUND: Differentiated thyroid carcinoma (DTC) responses to initial post-operative I-131 therapy are heterogeneous. Standard binary models combine incomplete responses into a 'non-excellent' category, obscuring transitional states (indeterminate response [IDR] and biochemical incomplete response [BIR]) and outcome-specific drivers. We developed and validated an interpretable four-class machine learning framework to separate these dynamic trajectories and support pre-therapeutic decisions. MATERIALS AND METHODS: Data from 948 DTC patients were partitioned into training (n = 663) and test (

Development of Predictive Models for Long-Term Endoscopic Response to Ustekinumab in Crohn's Disease Based on Plasma Proteomics.

PURPOSE: Ustekinumab (UST) is effective for Crohn's disease (CD), yet reliable biomarkers for predicting long-term response remain scarce. This study aimed to identify novel plasma proteomic biomarkers and develop predictive models for long-term endoscopic response to UST therapy in patients. METHODS: Baseline plasma inflammatory proteins were profiled using the Olink platform in 40 CD patients treated with UST (20 responders, 20 non-responders). Differentially expressed proteins (DEPs) were identified after adjusting for age, age at diagnosis, and SES-CD scores. Stable DEPs were selected via

Beta-Blockers and Abdominal Aortic Aneurysm Growth: A Systematic Review and Meta-Analysis.

BACKGROUND: Aortic aneurysms are worrisome because of their predisposition to dissection and rupture. Beta-blockers are considered first-line therapy for aortic aneurysms. The following meta-analysis assesses if beta-blockers diminish aortic aneurysm growth. METHODS: A literature search was performed to collect information on clinical trials that have assessed aortic aneurysm growth between beta-blockers and placebo. The primary endpoint was aortic aneurysm growth rate per year. A forest plot with a random-effects model was used for analysis. RESULTS: Eight clinical trials were included in t

A random forest based computational model for predicting novel lncRNA-disease associations.

BACKGROUND: Accumulated evidence shows that the abnormal regulation of long non-coding RNA (lncRNA) is associated with various human diseases. Accurately identifying disease-associated lncRNAs is helpful to study the mechanism of lncRNAs in diseases and explore new therapies of diseases. Many lncRNA-disease association (LDA) prediction models have been implemented by integrating multiple kinds of data resources. However, most of the existing models ignore the interference of noisy and redundancy information among these data resources. RESULTS: To improve the ability of LDA prediction models,

[Characteristics of the response of mice with temporary immunodepression to the administration of live or inactivated vaccine against tick-borne encephalitis].

Mice with immunity defects caused by administration of cyclophosphane were inoculated with an inactivated vaccine or Langat virus (TP-21 strain) considered to be an approximate model of the vaccine against tick-borne encephalitis. In mice with depressed immune response, the asymptomatic infection caused by Langat virus turns into acute form and encephalitis with a fatal outcome develops in 86--100%. Pre-treatment with inactivated tick-borne encephalitis vaccine did not protect the animals with the immunity defect against neuroinfection induced by subsequent inoculation with Langat or tick-born

[Keloid paracoccidioidomycosis assessed with gallium].

This rare mycosis is caused by Paracoccidiodis loboii and is found mainly among indians of the Amazon forest. It is a cutaneous disease. No visceral affection was ever described, though generalized skin disease has been reported. One case of cheloid paracoccidioidomycosis was studied with gallium-67. The radiopharmaceutical has accumulated in the affected area and in a lymph node, although adenomegaly be rare. Degree of accumulation was discrete/moderate but the patient had been under irregular treatment for 10 years. As no laboratorial data are evaluable for this affection, gallium-67 may rep

Ligand-receptor interaction profiling as a predictive biomarker for anti-PD-1 therapy response in melanoma.

Cell-to-cell communication through ligand-receptor (LR) interactions can fundamentally shape the tumor microenvironment and immune responses, but the full spectrum of these interactions in anti-PD-1 therapy remains unexplored. We developed a predictive model for anti-PD-1 therapy responses incorporating 2,705 LR pairs across 121 melanoma samples. Using a random forest classifier, our model achieved robust accuracy in the training and test datasets as well as in two independent external validation cohorts. Feature importance analysis revealed nine key LR pairs with substantial predictive power,

Modifiable predictors of nonresponse to psychotherapies for late-life depression with executive dysfunction: a machine learning approach.

The study aimed to: (1) Identify distinct trajectories of change in depressive symptoms by mid-treatment during psychotherapy for late-life depression with executive dysfunction; (2) examine if nonresponse by mid-treatment predicted poor response at treatment end; and (3) identify baseline characteristics predicting an early nonresponse trajectory by mid-treatment. A sample of 221 adults 60 years and older with major depression and executive dysfunction were randomized to 12 weeks of either problem-solving therapy or supportive therapy. We used Latent Growth Mixture Models (LGMM) to detect sub

Topical negative pressure therapy: current concepts and practice.

Research into topical negative pressure therapy (TNPT) started in 1989 with Dr Louis Argenta and Prof Michael Morykwas of Wake Forest University School of Medicine in North Carolina, USA. In 1997, Morykwas and Argenta concluded that TNPT both enhanced granulation tissue formation and helped bacterial clearance, through the actions of negative pressure Armstrong and Lavery confirmed this in 2005, concluding that TNPT therapy was promoting the development of granulation tissue. Numerous other studies have proved that TNPT is beneficial for a myriad of other wounds including: sternotomy wounds, e
