PURPOSE: We present a patient-specific model to estimate tumor location in the thorax during radiation therapy using chest surface displacement as the surrogate signal.
METHODS: Two types of data are used for model construction: Four-dimensional computed tomography (4D-CT) images of the patient and the displacement of two points on the patient's skin on the thoracic area. Principal component analysis is used to fit the correspondence model. This model incorporates the recorded surrogate signals during radiation delivery as input and delivers the 3D trajectory of the tumor as output. We evalua…

PURPOSE: Using intra-fractional cone-beam CT (CBCT) to evaluate the amplitude changes and baseline shifts of respiratory motion in liver stereotactic body radiation therapy (SBRT).
METHODS: The amplitude changes and baseline shifts of respiratory motion for 24 liver patients were evaluated by the four-dimensional (4D) CT, inter- and intra-fractional CBCT. The difference of the average liver position errors among all treatment fractions and the 4D CT representthe baseline shifts. According to the baseline shifts, the ITV to PTV margin was recalculated and the plan was re-designed to compare th

PURPOSE: Average intensity projection (AIP) is an image generation method obtained by averaging the images of each phase of 4D-CT. This study aimed to evaluate tumor respiratory motion and, using the union of contours from all 4D-CT phases (4DTotal) as the reference, to compare contour agreement among contours derived from 3D-CT, maximum intensity projection (MIP), and AIP images. In addition, dose metrics in treatment planning were evaluated to explore the dosimetric impact of contour differences among the image sets.
METHODS: Eight patients who underwent lung stereotactic body radiotherapy

Recent works in medical image registration have proposed the use of Implicit Neural Representations, demonstrating performance that rivals state-of-the-art learning-based methods. However, these implicit representations need to be optimized for each new image pair, which is a stochastic process that may fail to converge to a global minimum. To improve robustness, we propose a deformable registration method using pairs of cycle-consistent Implicit Neural Representations: each implicit representation is linked to a second implicit representation that estimates the opposite transformation, causin

Current respiratory 4DCT imaging for high-dose rate thoracic radiotherapy treatments are negatively affected by the complex interaction of cardiac and respiratory motion. We propose an imaging method to reduce artifacts caused by thoracic motion, CArdiac and REspiratory adaptive CT (CARE-CT), that monitors respiratory motion and ECG signals in real-time, triggering CT acquisition during combined cardiac and respiratory bins. Using a digital phantom, conventional 4DCT and CARE-CT acquisitions for nineteen patient-measured physiological traces were simulated. Ten respiratory bins were acquired f

Minimally invasive parathyroidectomy (MIP) is the standard of care for primary hyperparathyroidism (PHPT). Four dimensional computed tomography(4DCT) and F-18 Fluorocholine positron emission tomography/computed tomography (FCH PET/CT) localize adenomas accurately to perform MIP. We aimed to conduct a systematic review and metanalysis to evaluate the diagnostic performance of 4DCT and FCH PET/CT scan for quadrant wise localisation in PHPT patients and to do head-to-head comparison between these two modalities. BACKGROUND: Deformable image registration (DIR) is a crucial tool in radiotherapy for analyzing anatomical changes and motion patterns. Current DIR implementations rely on discrete volumetric motion representation, which often leads to compromised accuracy and uncertainty when handling significant anatomical changes and sliding boundaries. This limitation affects the reliability of subsequent contour propagation and dose accumulation procedures, particularly in regions with complex anatomical interfaces such as the lung-chest wall boundary.
PURPOSE: Given that organ motion is inherently a con

Lung injuries lead to heterogeneous ventilation behavior in the lung parenchyma, and conventional methods used to assess lung health, such as spirometry, fall short of providing regional information about lung function. Dynamic medical imaging and image registration offer a powerful tool for estimating the kinematic behavior of lung parenchyma in vivo. However, the difficulty of validating lung deformation estimated by image registration has curbed widespread adoption in the clinic. In-silico images, reconstructed from finite element (FE) simulations, provide a method to verify the results est

PURPOSE: Noninvasive cardiac radioablation is increasingly used for treatment of refractory ventricular tachycardia. Attempts to limit normal tissue exposure are important, including managing motion of the target. An interplay between cardiac and respiratory motion exists for cardiac radioablation, which has not been studied in depth. The objectives of this study were to estimate target motion during abdominal compression free breathing (ACFB) and respiratory gated (RG) deliveries and to investigate the quality of either implanted cardioverter defibrillator lead tip or the diaphragm as a gatin

Parkinson's disease (PD) is a progressive, incurable neuro-degenerative disease. Symptoms appear when approximately 70% of mid-brain dopaminergic neurons have died. Temporal analysis of the calculated area of the rima glottidis may give an indication of vocal impairment. In this paper, we present an automatic segmentation algorithm to segment the rima glottidis from 4D CT images using texture features and support vector machines (SVM). Automatic two dimensional region growing is then applied as a post processing step to segment the area accurately. The proposed segmentation algorithm resulted

We present a technique for continuous generation of volumetric images during SBRT using periodic kV imaging and an external respiratory surrogate signal to drive a patient-specific PCA motion model. Using the on-board imager, kV radiographs are acquired every 3 s and used to fit the parameters of a motion model so that it matches observed changes in internal patient anatomy. A multi-dimensional correlation model is established between the motion model parameters and the external surrogate position and velocity, enabling volumetric image reconstruction between kV imaging time points. We performed a prospective study to evaluate the values of dynamic four-dimensional computed tomography in assessing suspected chronic scapholunate instability. Forty patients were evaluated with radiographs, arthrography, and four-dimensional computed tomography. On plain radiographs and computed tomography, we found 16 patients with definite scapholunate instability, five with questionable scapholunate instability, and 19 with absence of scapholunate instability. We used four-dimensional computed tomography to evaluate the size of the scapholunate gap during radioulnar deviation. The mean an…

BACKGROUND: Respiratory signal detection is critical for 4-dimensional (4D) imaging. This study proposes and evaluates a novel phase sorting method using optical surface imaging (OSI), aiming to improve the precision of radiotherapy.
METHOD: Based on 4D Extended Cardiac-Torso (XCAT) digital phantom, OSI in point cloud format was generated from the body segmentation, and image projections were simulated using the geometries of Varian 4D kV cone-beam-CT (CBCT). Respiratory signals were extracted respectively from the segmented diaphragm image (reference method) and OSI respectively, where Gauss

BACKGROUND AND OBJECTIVE: Estimating the three-dimensional (3D) deformation of the lung is important for accurate dose delivery in radiotherapy and precise surgical guidance in lung surgery navigation. Additional 4D-CT information is often required to eliminate the effect of individual variations and obtain a more accurate estimation of lung deformation. However, this results in increased radiation dose. Therefore, we propose a novel method that estimates lung tissue deformation from depth maps and two CT phases per patient.
METHODS: The method models the 3D motion of each voxel as a linear d

Most of the people all over the world pass away from complications related to lung cancer every single day. It is a deadly form of the disease. To improve a person's chances of survival, an early diagnosis is a necessary prerequisite. In this regard, the existing methods of tumour detection, such as CT scans, are most commonly used to recognize infected regions. Despite this, there are certain obstacles presented by CT imaging, so this paper proposes a novel model which is a correlation-based model designed for analysis of lung cancer. When registering pictures of thoracic and abdominal organs

PURPOSE: The use of time-resolved four-dimensional computed tomography (4D-CT) in radiotherapy requires strict quality assurance to ensure the accuracy of motion management protocols. The aim of this work was to design and test a phantom capable of large amplitude motion for use in 4D-CT, with particular interest in small lesions typical for stereotactic body radiotherapy.
METHODS: The phantom of "see-saw" design is light weight, capable of including various sample materials and compatible with several surrogate marker signal acquisition systems. It is constructed of polymethylmethacrylate (P…
