[Posterior radiocarpal fracture-dislocation (eight case reports)].

The Authors report a retrospective series of eight cases of posterior radiocarpal fracture-dislocation. Average age of patients was 32 years (23/54), with a male predominance (7 men and a woman). Injury was often due to a fall on the hand in extension from an elevated place (5/8). The wrist in back of fork is the characteristic deformity. Diagnosis was confirmed by AP and lateral views of the wrist. Radial styloïd fracture, posterior dislocation of the carpus and posterior rim avulsion were constant. Ulnar styloïd fracture was present in half of the cases and distal radio-ulnar dislocation in

Unilateral mandible fracture with bilateral TMJ dislocation.

Temporomandibular joint (TMJ) dislocation occurs when the condylar head slips forward causing the posterior articulating surface of the condyle to advance ahead of the articular eminence, possibly becoming entrapped. Following dislocation, the ligaments around the joint often stretch, causing severe muscle spasms and joint pain. There is no standard evaluation and treatment method for acute TMJ dislocation, but the most effective course is immediate reduction. This paper presents a 42-year-old woman who sustained a unilateral mandible fracture with bilateral TMJ dislocation in an automobile cr

Posterolateral dislocation of the ankle without fracture.

We report an unusual case of open posterolateral dislocation of the ankle without fracture following a motor vehicle accident. Most of previously reported cases are posteromedial dislocation. The anterolateral, posterolateral, and purely superior dislocations of the talus make up smaller percentage. The mechanism of this injury appears to be forced eversion of the foot when it is maximally plantar flexed and axially loaded. Our case was treated by manual reduction and percutaneous fixation and anatomical repair of the disrupted deltoid ligament at the time of initial débridement, also achieved

Ipsilateral sternoclavicular joint dislocation and clavicle fracture.

A case of an ipsilateral sternoclavicular (SC) joint dislocation and clavicle fracture is reported. Two hours postinjury an open reduction and internal fixation of the fractured clavicle was performed using a Kirschner wire plus a stainless steel cerclage wire. This was followed by a surgical repair of the SC joint capsule. The Kirschner wire and cerclage wires were removed 10 months later. The patient had complete recovery of shoulder function.

Talar fracture/dislocation in the adolescent patient.

The authors report an 18-year-old patient who sustained a two-part injury involving his right ankle. The result of this traumatic episode was a fracture/dislocation of the talus. An interesting aspect of this injury is that the body of the talus dislocated laterally, distal to the fibular malleolus. Closed reduction was attempted and subsequent open reduction with internal fixation was performed. The patient was followed for 9 months, during which time a series of radiographs was taken. Nine months postoperatively, Hawkins sign was negative indicative of avascular necrosis. In review of the li

Variant of Galeazzi fracture-dislocation in children.

Fractures of the radial shaft associated with disruption of the distal ulnar radial joint (Galeazzi fracture) account for a small percentage of forearm fractures in adults but occur even more rarely in children. An equivalent injury in children is a fracture of the radius with separation of the distal ulnar physis in the form of a Salter-Harris type II epiphyseal fracture. Although this lesion has been mentioned in the literature, to our knowledge it has not been described in detail. We report three cases of this fracture. All cases were treated by open reduction internal fixation.

Posterior radial head dislocation without ulnar fracture.

Posterior radial head dislocation without an ulnar fracture in an adult is a rare injury. The pathology is often unrecognized initially. The patient's resultant elbow motion becomes severely compromised. Prompt diagnosis and reduction of the dislocation yield excellent results. A review of the biomechanical studies of the elbow ligaments enables us to propose a pathophysiological model of this injury, and a case report and review of the literature are presented.

Posteromedial dislocation of the ankle without fracture.

We report the results of treatment in eight patients who had posteromedial dislocation of the ankle without fracture. The ages of seven of the eight patients ranged from twenty-three to thirty-six years. Four patients had increased generalized ligamentous laxity. One-half of the injuries were sustained during athletic competition. The mechanism of injury appeared to be forced inversion of the foot when it was maximally plantar flexed and axially loaded. Five of the eight patients had an open injury, and four of the five patients had an associated injury to the anterolateral neurovascular struc

Transverse fracture of the glenoid without dislocation.

This is a case report of a 15-year-old boy with an unusual fracture of the glenoid. The mechanism of injury was a fall on the partially extended, abducted arm. The fall produced a fracture with displacement of the inferior half of the glenoid. The technique of open reduction and exposure used differed from the standard approach in that the long head of the triceps was sectioned at its glenoid origin to facilitate the reduction. At one-year follow-up, the patient demonstrated a full range of motion and excellent triceps strength.

Transcuneiform fracture--cuboid dislocation of the midfoot.

Complete dislocation of the tarsal cuboid is presented along with regional anatomic considerations. Thorough literature search revealed only three other reported cases of dislocation of the tarsal cuboid. The operative procedure described encompasses an interesting method of reduction of this dislocation. It should be emphasized that A-P, lateral, and oblique X-rays should be obtained for injuries of the mid and forefoot. The rarity of such a dislocation is attributed to the structural integrity of the supporting ligamentous elements.

Surgical treatment of acute cervical fracture-dislocation.

The authors describe their experience in the surgical treatment of acute cervical fracture-dislocation by reporting the results obtained after the observation and follow-up examination of 53 cases. The surgical procedure adopted, after realignment of bone segments was obtained, depended on type of fracture. Posterior fixation with wire or wire and acrylic was performed in 31 cases. The anterior route was chosen in 19 cases, performing a ventral subtotal resection of the vertebral body with the use of acrylic. Three patients were treated by both procedures (anterior plus posterior approach). Th

[Treatment outcome in de Quervain's dislocation fracture].

In De Quervains dislocation fracture pseudarthroses of the scaphoid bone following closed reduction are very common, 59% being reported in a former publication. Exact adaptation of the fragments after reduction of the perilunar dislocation is essential for healing of the fracture. In cases where this is not possible by conservative means, open reduction and screwing of the scaphoid fracture is necessary for better result with a rate of pseudarthroses of only 16%.

Management of Finger Joint Dislocation and Fracture-Dislocations in Athletes.

Although finger joint dislocations are generally thought of as benign by many athletes and assumed to be a sprain, these injuries represent a spectrum that includes disabling fracture-dislocations. Failure to recognize certain dislocations or fracture-dislocations may result in permanent deformity and loss of motion. Simple dislocations are frequently amenable to early return to play with protection; however, more complex injuries may require specialized splinting or surgery. Delay in diagnosis of unstable proximal interphalangeal fracture-dislocations may require reconstruction or fusion. Ear

Use of external fixation for perilunate dislocations and fracture dislocations.

The purpose of this study was to review clinical and radiographic outcomes of perilunate dislocations and fracture dislocations treated with external fixation and K-wire fixation. Twenty patients (18 males and two females) with a mean age of 38 years (range 18-59) who had an acute dorsal perilunate dislocation or fracture dislocation were treated with the use of wrist external fixator and K-wires. The injuries included 12 perilunate dislocations, seven trans-scaphoid perilunate fracture dislocations, and one trans-styloid perilunate fracture dislocation. The median time from trauma to operatio…

[Perilunate dislocation and dislocation fractures--results of surgical management].

We report 22 patients who sustained a perilunate dislocation (PD) with or without a fracture of the scaphoid. The carpal ligaments were explored and reattached in all cases. The fractured scaphoid in perilunate fracture dislocations (PFD) was stabilised by a Herbert screw in 13 cases and in one case with a Matti-Russe bone graft. After an average of 5 years the clinical score of Green and O'Brien showed good results for both PD and PFD. Patients with PFD had less pain and better grip than the patients with PD. The sagittal range of motion (average between 70 degrees and 99 degrees) and the sub…

Unilateral facet dislocations and fracture-dislocations of the cervical spine.

We treated 36 patients with unilateral facet dislocations or fracture-dislocations of the cervical spine at the Mayo Clinic between 1975 and 1986. Adequate records were available for 34: ten patients were treated by open reduction and posterior fusion, and 24 by nonoperative management. Of these, 19 had halo traction followed by halo-thoracic immobilisation, four had a simple cervicothoracic orthosis, and one received no active treatment. Anatomical reduction was achieved more frequently in the operative group (60% compared with 25%). Nonoperative treatment was more likely to result in cervica
