Intramedullary nail fixation of non-traditional fractures: Clavicle, forearm, fibula.

Locked intramedullary fixation is a well-established technique for managing long-bone fractures. While intramedullary nail fixation of diaphyseal fractures in the femur, tibia, and humerus is well established, the same is not true for other fractures. Surgical fixations of clavicle, forearm and ankle are traditionally treated with plate and screw fixation. In some cases, fixation with an intramedullary device is possible, and may be advantageous. However, there is however a concern regarding a lack of rotational stability and fracture shortening. While new generation of locked intramedullary d

Forearm diaphyseal fractures in children: intramedullary Kirschner's wire fixation treatment.

INTRODUCTION: Diaphyseal fractures of the forearm comprise 6%-10% of all fractures in children. The treatment depends on the age and type of displacement, and conservative and surgical management with fixation of intramedullary nails, among other techniques, is valid. The aim is to show the radiological and functional outcomes, and complications of intramedullary fixation with Kirschner nails in children. MATERIALS AND METHODS: A retrospective descriptive case series of patients treated with intramedullary fixation of forearm fractures. The radiological and functional results, and complicatio

A Novel Intramedullary Locked Fixation Device for Treatment of Clavicle Shaft Fractures.

INTRODUCTION: An intramedullary nail (the Sonoma CRx) with a unique flexible anatomical design and locking system is a viable alternative for treatment of displaced and shortened clavicle shaft fractures. STEP 1 POSITIONING: Place the patient in the beach-chair position and drape the arm free so that you can obtain an exaggerated anteroposterior and an axial fluoroscopic view of the clavicle by manipulating the position of the arm. STEP 2 SKIN INCISION: The surgical technique necessitates opening the fracture site in order to implant the device. STEP 3 PREPARING THE MEDIAL FRAGMENT: The pre

Closed intramedullary fixation for metacarpal fractures using J-shaped nail.

The purpose of this study is to present the results of a new intramedullary fixation technique for metacarpal fractures. A J-shaped nail, which is a curved 2.0 mm diameter Kirschner wire sharply bent at the proximal end, was inserted from the dorsal aspect of the metacarpal base. Twenty-one metacarpal fractures (five transverse shaft fractures and 16 neck fractures) in 19 hands of 18 patients with were operated by this technique. All fractures had successful unions. Clinical and radiographic results were excellent in all ten hands that had excluded concomitant injuries. There was no mechanical

Metacarpal and proximal phalangeal fractures--fixation with multiple intramedullary Kirschner wires.

The use of intramedullary wires for fixation of fractures of the metacarpal and proximal phalanx in our hospital was reviewed. Twenty-six patients with 26 metacarpal fractures and four proximal phalangeal fractures were treated using this technique from 1993 to 1998. After a minimal follow-up of nine months, all the patients were assessed clinically and radiologically. All fractures proceeded to bony union at an average of 5.7 weeks (range four to eight weeks). With this simple and minimally invasive technique, most of these patients required very short hospital stay and were able to start mob

Screw Length Associated With Fracture Gapping of Fifth Metatarsal Base Fracture With Intramedullary Screw Fixation: A Cadaveric Study.

Intramedullary screw fixation is a well-established surgical treatment for fifth metatarsal Jones fractures, due to its minimally invasive nature, and potential early return to activity. Due to the curvature of the fifth metatarsal, optimal length of the screw is needed to prevent gapping at the fracture site. The placement of a straight screw induces straightening of a naturally curved bone. The purpose of this study was to aid surgeons in determining an appropriate screw length for intramedullary fixation of a fifth metatarsal Jones fracture in order to prevent fracture gapping. A transverse

Segmental femoral fracture management using intramedullary nailing combined with plate fixation: a case report.

INTRODUCTION: Segmental femoral fractures are challenging, with a high risk of non-union, requiring tailored treatments such as intramedullary nailing (IMN) and plate fixation. Although this combined approach is promising for tibial fractures, its effectiveness in segmental femoral fractures remains under evaluation. This study aims to evaluate the success rates of IMN and plate fixation in treating these challenging injuries. PRESENTATION OF CASE: A 52-year-old man presented with a deformed, shortened left thigh, moderate tenderness (Visual Analogue Scale score of 4), and limited hip and kne

Metastatic adenocarcinoma after intramedullary fixation of a tibial fracture: A report of two cases.

We present two cases of metastatic lung cancer which occurred at the site of a previously united tibial fracture. Both patients were treated with a locked intramedullary nail. The patients presented with metastases at the site of their initial fracture approximately 16 and 13 months after injury respectively. We discuss this unusual presentation and review the relevant literature. We are unaware of any previous reports of a metastatic tumour occurring at the site of an orthopaedic implant used to stabilise a non-pathological fracture. These cases demonstrate the similar clinical presentation o

[Orthograde Kirschner wire osteosynthesis. Experiences with intramedullary fixation of the distal metacarpal V fracture].

Dislocated metacarpal V fractures, fixed by percutaneous Kirschner wires or other implants near the metacarpophalangeal joint, often show impaired movement despite excellent radiologic findings. We therefore chose an alternative method using intramedullary Kirschner wire osteosynthesis according to Foucher. Twenty-five patients were operated on in a 3-year period and movement of the fingers and complications were examined. Six weeks after operation all fractures were consolidated in anatomical reduction. One patient suffering from an early stage of Sudeck's dystrophy was healed, and no other c

Intramedullary nailing of femoral shaft fractures. Part II: Fracture-healing with static interlocking fixation.

A consecutive, prospective series of ninety-seven patients who had 100 fractures of the femoral shaft that were treated with static interlocking nailing was analyzed to determine the incidence of union of the fracture without planned conversion from static to dynamic intramedullary fixation as a technique to stimulate healing of the fracture. Eighty-four patients (eighty-seven fractures) were studied through union of the fracture (average follow-up, fourteen months). Eighty-five (98 per cent) of the eighty-seven fractures healed with static interlocking fixation. Two patients needed conversion

Operative Treatment of Adolescent Diaphyseal Clavicle Fracture: Elastic Stable Intramedullary Nail Versus Plate Fixation.

Background and Objectives: Adolescent diaphyseal clavicle fracture surgery has increased in recent years. However, the optimal operative method remains debated, particularly between elastic stable intramedullary nailing (ESIN) and plate fixation. This study compared postoperative outcomes and complication rates between ESIN and plate fixation for treating diaphyseal clavicle fractures in adolescent patients. Materials and Methods: We conducted a retrospective review of 35 adolescents who underwent surgery for diaphyseal clavicle fractures between 2010 and 2024. Patients were assigned to either

Reverse wedge effect following intramedullary nail fixation of trochanteric fracture, what does it imply?

Lag screw cut-out is the most common cause of fixation failure of trochanteric fractures. Intraoperative assessment of fracture reduction and fixation quality is vital to avoid fracture reduction and achieve good functional outcomes. In a recent study, Zhang et al. reported the occurrence of a reverse wedge effect after intraoperative nail insertion based on a new computed tomography(CT)-guided fracture classification system, which specifically happened to the basicervical facture type and resulted in valgus deformity with gapping at the medial inferior fracture line. Impingement between the r

Biomechanical Comparison of Intramedullary Screw Versus Low-Profile Plate Fixation of a Jones Fracture.

BACKGROUND: Intramedullary screw fixation of fifth metatarsal Jones fractures often produces satisfactory results, however, nonunion and refracture rates are not negligible. The low-profile "hook" plate is an alternative fixation method that has been promoted to offer improved rotational control at the fracture site, but this remains to be proven. The purpose of this study was to document biomechanical performance differences between this type of plate and a contemporary solid, dual-pitch intramedullary screw in a cadaveric Jones fracture model. METHODS: Simulated Jones fractures were created
