"Click" amphotericin B in prodrug nanoformulations for enhanced systemic fungemia treatment.

Severe nephrotoxicity and infusion-related side effects pose significant obstacles to the clinical application of Amphotericin B (AmB) in life-threatening systemic fungal infections. In pursuit of a cost-effective and safe formulation, we have introduced multiple phenylboronic acid (PBA) moieties onto a linear dendritic telodendrimer (TD) scaffold, enabling effective AmB conjugation via boronate chemistry through a rapid, high yield, catalysis-free and dialysis-free "Click" drug loading process. Optimized AmB-TD prodrugs self-assemble into monodispersed micelles characterized by small particle

Pichia ohmeri fungemia associated with phlebitis: successful treatment with amphotericin B.

We report a case of fungemia caused by the yeast-form fungus Pichia ohmeriin a 59-year-old hospitalized patient. P. ohmeri was found in all of the patient's blood cultures collected on days 52, 57, 59, and 64 of his hospital stay. Intermittent fever developed on the 52nd hospital day and persisted for about 10 days. The patient had previously received intensive antimicrobial therapy for a ventriculoperitoneal shunt infection and subsequent nosocomial pneumonia. Although a central venous catheter was not used in the patient, he suffered from tender swelling of the right leg due to peripheral ph…

Hospital-onset bacteremia and fungemia in a regional burn intensive care unit.

BACKGROUND: Bacteremia is a common complication in burn patients. Hospital-onset bacteremia and fungemia (HOB) has been proposed as a quality measure for value-based purchasing programs. We sought to describe the characteristics of HOB in a regional burn center. METHODS: Patients admitted to the burn intensive care unit from September 2018 through December 2024 were identified through the burn registry. Microbiologic and patient data were extracted from the electronic medical record and Vizient database. RESULTS: One hundred seventy-nine (88.6%) of 202 bacteremia episodes were categorized as…

Machine learning prediction of mortality in pediatric fungemia using the Candida score.

Pediatric fungemia in pediatric intensive care units (PICUs) carries high mortality. We evaluated whether the Candida Score, combined with clinical variables, predicts mortality after diagnosis using a prespecified multivariable logistic regression (primary model) and benchmarked discrimination against Random Forest and Gradient Boosting Machine. We analyzed 85 pediatric fungemia cases from a PICU (2016-2020). The prespecified primary model was multivariable logistic regression with predefined covariates; Random Forest and Gradient Boosting Machine were exploratory comparators. Discrimination …

Fungemia Due to Saprochaete capitata in a Non-Neutropenic Critically Ill Patient.

Saprochaete capitata is an uncommon yeast species; its impact on non-neutropenic patients appears to be on the rise. We describe a case of S. capitata fungemia in a critically ill end-stage kidney disease (ESKD) patient on peritoneal dialysis. The patient presented with mesenteric ischemia and underwent several laparotomies during hospitalization. His hospital stay was complicated as fungemia developed and spread to multiple sites, which resulted in severe complications and ultimately led to fatal outcomes. S. capitata's diagnostic delay is a concern, but matrix-assisted laser desorption/Ioniz…

Candida dubliniensis fungemia in a patient with severe COVID-19: A case report.

Candida dubliniensis phenotypically mimics Candida albicans in its microbiological features; thus, its clinical characteristics have yet to be fully elucidated. Here we report the case of a 68-year-old Japanese man who developed C. dubliniensis fungemia during treatment for severe coronavirus disease 2019 (COVID-19). The patient was intubated and received a combination of immunosuppressants, including high-dose methylprednisolone and two doses of tocilizumab, as well as remdesivir, intravenous heparin, and ceftriaxone. A blood culture on admission day 11 revealed Candida species, which was con…

A Case of Fungemia Caused by Postoperative Chronic Lumbar Intervertebral Disc Infection.

Postoperative surgical site infection is one of the serious postoperative complications of spine surgery, especially fungal infections. Late-stage surgical site fungal infections often lack typical clinical symptoms and have a variable clinical presentation. In this case, the patient was a senior patient with usually only tolerable pain and discomfort, which was detected 2 years after the first surgery. Such cases are even rarer for fungal bloodstream infections caused by delayed postoperative chronic fungal osteomyelitis and deserve further study for early identification and intervention to m

The Simultaneous Occurrence of Histoplasmosis and Cryptococcal Fungemia: A Case Report and Review of the Literature.

Simultaneous infections with Cryptococcus neoformans and Histoplasma capsulatum are rare and typically occur in immunocompromised individuals, particularly AIDS patients. Because both of those fungi can spur the development of latent infections, it is generally unknown which organism was first present. The diagnosis of one fungus can hide the diagnosis of the other, leading to underdiagnoses and poor prognosis. We report a case of C. neoformans and H. capsulatum co-infection. We also performed a systematic review of the literature. Our patient, a 69-year-old HIV-negative man, presented with a

Reporting an outbreak of Candida pelliculosa fungemia in a neonatal intensive care unit.

BACKGROUND: Fungemia in preterm infants is associated with high mortality and morbidity. This study reports an outbreak of unusual fungemia in a tertiary neonatal intensive care unit (NICU). METHODS: Ten Candida pelliculosa bloodstream isolates were identified from six infants hospitalized in the NICU from February to March 2009. Environmental study was performed, and genetic relatedness among the 10 clinical isolates of C pelliculosa and six control C pelliculosa strains was characterized by randomly amplified polymorphic DNA assay. In vitro susceptibility of isolates to six antifungal agent…

Trichosporon mucoides fungemia in a liver transplant recipient: case report and review.

Four months after receiving an orthotopic liver transplant, a 51-year-old man was admitted for progressive liver failure and severe hepatocellular necrosis thought to be due to tacrolimus. During his hospitalization he experienced bloodstream infections including fungemia due to Trichosporon mucoides and prolonged undulating fever despite antifungal and antibacterial treatment. He underwent removal of the allograft and implantation of another liver. Fever continued postoperatively until therapy with posaconazole was initiated. Initiation of posaconazole led to clinical improvement until the pa…

[Fungemia after oral treatment with Saccharomyces boulardii in a patient with multiple comorbidities].

HISTORY AND CLINICAL FINDINGS: A 48-year-old diabetic with multiple co-morbidities presented with generalized micro- and macroangiopathy inc … or supportive therapy of diarrhea. The present case and a review of the literature demonstrate that fungemia and sepsis are rare complications of the administration of S. boulardii in immunocompromised patients. For this reason the therapeutic usage of probiotics should be carefully considered regarding its risk-benefit potential.

Three-year survey of bacteremia and fungemia in a pediatric intensive care unit.

BACKGROUND: Knowledge of the pattern of blood stream infection (BSI) in patients in intensive care units (ICUs) can help determine antibiotic prescribing policy and infection control procedures. However, there have been few pediatric-based studies. METHODS: Surveillance of BSI in a pediatric ICU for 3 years, amounting to 131 episodes of significant bacteremia and fungemia. RESULTS: The incidence of BSI was 39.0 per 1,000 admissions (10.6 per 1,000 bed days). Eighty-four (64.1%) episodes were ICU-acquired, and 27 (20.6%) were community-acquired. Gram-positive, Gram-negative and anaerobic bact…

Management of catheter-related bacteremia and fungemia in patients on total parenteral nutrition.

To diminish the risk of serious complications from catheter-related bacteremias or fungemias, an optimized diagnosis and antimicrobial therapy is essential and early catheter removal should be considered. Prompt removal of the catheter and targeted antimicrobial treatment remains a common approach for febrile episodes in patients on total parenteral nutrition. However, novel tools allow diagnosis of catheter-related infections with the catheter in situ. Moreover, many of the established catheter-related infections caused by coagulase-negative staphylococci can successfully be treated with the …

Changing etiology of nosocomial bacteremia and fungemia and other hospital-acquired infections.

Serial surveys on the etiology of nosocomial bacteremia have been conducted over a period of years at Boston City Hospital (Boston) and Grady Memorial Hospital (Atlanta). A comparison of the information from these surveys with that from single-period surveys at 10 other hospitals in the United States illustrates changes in the etiology of nosocomial bloodstream infection over the past five decades. Prominent trends include an increased frequency of episodes of polymicrobial bacteremia, an increased frequency of sequential episodes of bacteremia in the same patient, a resurgence of infection du

Detection of experimental bacteremia and fungemia by examination of buffy coat prepared by a micromethod.

Rabbits received intravenous injections of bacteria or fungi, and a comparison was made of the abilities of broth cultures, plating after dilution either in saline solution or in distilled water containing Triton X-100, and buffy coat examinations to detect the organisms in heart blood. The most sensitive method was broth culture. By microscopy or subculture of buffy coat cells prepared by centrifugation of blood in microhematocrit tubes, organisms were rapidly and regularly detected when their viable counts increased to 300--1,000/ml as detected by plating. By micromodification, buffy coat ex

Saccharomyces cerevisiae fungemia after probiotic treatment in an intensive care unit patient.

Saccharomyces cerevisiae is a common colonizer of the human gastrointestinal system as a benign organism. Enteral supplementation of this yeast as a probiotic product is effective in the treatment of antibiotic associated diarrhae. In rare occasions it can cause invasive infections. We present two fungemia cases in an intensive care unit following probiotic treatment containing S. boulardii. We are warning the safety of probiotic treatment in critically ill patients.
