Barrett's oesophagus--are British gastroenterologists denying their patients prevention of malignant change?

Adenocarcinoma of the lower oesophagus is rapidly increasing in industrialised countries. The importance of Barrett's oesophagus is because of the potential for it to progress to oesophageal adenocarcinoma. It has a strong correlation to chronic GORD. Symptomatic patients or those with a long segment, if dysplasia is present or the patient is under 50 years of age, should be offered anti-reflux surgery. Patients may be denied the procedure by some gastroenterologists

The Gut Microbiome and Colonic Motility Disorders: A Practical Framework for the Gastroenterologist.

PURPOSE OF REVIEW: Colonic motility disorders may be influenced by the gut microbiota, which plays a role in modulating sensory and motor function. However, existing data are inconsistent, possibly due to complex disease pathophysiology, fluctuation in symptoms, and difficulty characterizing high-resolution taxonomic composition and function of the gut microbiome. RECENT FINDINGS: Increasingly, human studies have reported associations between gut microbiome features and colonic motility disorders, such as irritable bowel syndrome and constipation. Several microbial metabolites have been ident

Management of pancreatic cysts and guidelines: what the gastroenterologist needs to know.

The prevalence of pancreatic cysts has increased significantly over the last decade, partly secondary to increased quality and frequency of cross-sectional imaging. While the majority never progress to cancer, a small number will and need to be followed. The management of pancreatic cysts can be both confusing and intimidating due to the multiple guidelines with varying recommendations. Despite the differences in the specifics of the guidelines, they all agree on several high-risk features that should get the attention of any clinician when assessing a pancreatic cyst: presence of a mural nodu

Getting to grips with sarcopenia: recent advances and practical management for the gastroenterologist.

Sarcopenia is a progressive and generalised disorder of skeletal muscle strength, function and mass, that is most commonly associated with the normal ageing process. It is increasingly recognised that sarcopenia can also develop as a consequence of malabsorptive and inflammatory conditions, such as those seen by gastroenterologists and hepatologists. It affects 1%-30% of the general population, but is seen in approximately 40% of patients with gastrointestinal conditions including inflammatory bowel disease and cirrhosis. Within this group of patients, it is associated with increased complicat…

A gastroenterologist's perspective on the role of barium esophagography in gastroesophageal reflux disease.

Although endoscopy is a sophisticated technique for evaluating patients with gastroesophageal reflux disease (GERD), the single diagnostic test that enables the most comprehensive assessment for GERD is a well-performed video esophagram. Not only does the barium study permit assessment of morphologic abnormalities in the pharynx and esophagus, but also oropharyngeal swallowing function, esophageal motility, and gastroesophageal reflux. These factors are especially important for detection of anatomic findings such as strictures and hernias, for assessment of esophageal motility before and after

Postural Tachycardia Syndrome (POTS) and the GI Tract: A Primer for the Gastroenterologist.

Postural tachycardia syndrome (POTS) is one of the most common causes of orthostatic intolerance and is being increasingly recognized in clinical practice. Gastrointestinal (GI) symptoms are reported commonly in patients with POTS and pose a considerable management challenge, making it imperative that gastroenterologists be aware of this condition and its GI comorbidities. Although the evidence presented herein does not prove causation, it does support an association between GI symptoms, GI dysmotility, and POTS. At present, the evaluation and treatment of GI symptoms in patients with POTS rem…

Primary Sclerosing Cholangitis: What the Gastroenterologist and Hepatologist Needs to Know.

Primary sclerosing cholangitis (PSC) is a chronic, idiopathic biliary tract disease characterized by segmental strictures. The disease is progressive with no proven treatments and may eventually lead to cirrhosis and end-stage liver disease. Abrupt changes in liver biochemistries, pain, and/or cholangitis may suggest a dominant stricture amenable to endoscopic therapy or the development of cholangiocarcinoma. Patients with PSC are at increased risk of cholangiocarcinoma. There is a strong association with inflammatory bowel disease, and an associated increased risk of colorectal cancer. Colono

Celiac Disease: Ten Things That Every Gastroenterologist Should Know.

There are 10 things that all gastroenterologists should know about celiac disease (CD). (1) The immunoglobulin A tissue transglutaminase is the single best serologic test to use for the detection of CD. (2) CD can be recognized endoscopically, and water immersion enhances villi detection, although a normal endoscopic appearance does not preclude the diagnosis. (3) It is recommended that 4 biopsies be taken from the second part of the duodenum and 2 bulb biopsies be taken at the 9 o'clock and 12 o'clock positions to maximize the sensitivity for histologic confirmation of CD. (4) Consider serolo…

[Functional results of ileo-anal anastomosis. The point of view of the gastroenterologist].

Ulcerative colitis is an illness that occurs unexpectedly in a young patient, with a generally unpredictable prognosis, and there is no medication to cure it. This illness profoundly impairs the patient's professional life, family life, sex life, leading to an overall change in lifestyle, most particularly in cases of pancolitis. The majority of patients who benefit from a total colectomy for their ulcerative colitis, are those whose cases are severe, chronic, incapacitating, and who respond poorly to prolonged, appropriately administered medical treatment, mainly continuous steroid therapy. Methotrexate in the management of Crohn's disease: A practice survey of gastroenterologists in France.

BACKGROUND: Second-line treatment of Crohn's disease (CD) commonly involves immunosuppressants such as azathioprine, mercaptopurine, or methotrexate (MTX), used either alone or in combination. AIM: To investigate the current use of MTX among French gastroenterologists. METHODS: An online questionnaire was distributed between March and August 2023 to 116 French gastroenterologists managing CD. A total of 87 respondents completed the survey and were included in the analysis. RESULTS: Respondents reported a mean annual caseload of 140 CD patients (median: 50). Overall, 71% prescribed MTX, pred…

Imaging in perianal fistulising Crohn's disease: A practical guide for the gastroenterologist.

Perianal fistulising Crohn's disease is a challenging complication that can affect up to 20% of patients with Crohn's disease and is associated with significant morbidity. Despite advances in medical therapies, particularly anti-tumor necrosis factor agents, the majority of patients still require surgical intervention. Accurate diagnosis and monitoring are essential to optimise outcomes and guide multidisciplinary management. Although clinical scoring systems such as the perianal disease activity index are widely used, their subjective application limits their reproducibility and reliability,

Management of aero-digestive fistulas: the gastroenterologist's perspective, a narrative review.

BACKGROUND AND OBJECTIVE: Esophagorespiratory fistulas (ERFs) are pathologic communications between the esophagus and respiratory tract that may be congenital or acquired, and benign or malignant. ERFs can lead to significant respiratory compromise and increased mortality through recurrent respiratory infections. As the condition spans multiple organ systems, ERFs pose unique diagnostic and management challenges to pulmonologists, gastroenterologists, and thoracic surgeons. The aim of this manuscript is to provide an up-to-date review of the management of aero-digestive fistulas from the gastr…

A guide to laboratory diagnosis of Corona Virus Disease-19 for the gastroenterologists.

The outbreak of Corona Virus Disease-19 (COVID-19), caused by Severe Acute Respiratory Syndrome Corona Virus - 2 (SARS-CoV-2), a global pandemic, is having a significant impact on healthcare, especially the clinical microbiology laboratories all around the world. There are many reports which suggest that the disease can present with gastrointestinal symptoms such as nausea, vomiting, diarrhea, and loss of appetite, which the gastroenterologists may have to deal with. Hence, knowledge about the diagnosis of COVID-19 is important to gastroenterologists as well. The current review therefore cover…

Liver involvement in Gaucher disease: A practical review for the hepatologist and the gastroenterologist.

Gaucher disease (GD), a rare lysosomal storage disorder caused by deficient glucocerebrosidase activity and consequent accumulation of glycosphingolipids in the mononuclear phagocyte system, may progress to disabling and potentially life-threatening complications when left undiagnosed and untreated. Unfortunately, because of non-specific signs and symptoms and lack of awareness, patients with type 1 GD, the most common non-neuropathic variant, frequently experience diagnostic delays. Since splenomegaly and thrombocytopenia are the dominant clinical features in many GD patients leading to first

Acute graft-versus-host disease of the gut: considerations for the gastroenterologist.

Haematopoietic stem cell transplantation (HSCT) is central to the management of many haematological disorders. A frequent complication of HSCT is acute graft-versus-host disease (GVHD), a condition in which immune cells from the donor attack healthy recipient tissues. The gastrointestinal system is among the most common sites affected by acute GVHD, and severe manifestations of acute GVHD of the gut portends a poor prognosis in patients after HSCT. Acute GVHD of the gastrointestinal tract presents both diagnostic and therapeutic challenges. Although the clinical manifestations are nonspecific

Reflux-associated laryngitis and laryngopharyngeal reflux: a gastroenterologist's point of view.

Laryngopharyngeal reflux of liquid and gaseous gastric contents should be considered in every patient with unexplained hoarseness. Pathophysiology and treatment of reflux-associated laryngitis are different from those of reflux esophagitis and therefore remain an unsolved puzzle. The laryngeal mucosa is considerably more sensitive to acid and pepsin than the mucosa of the esophagogastric junction. Therefore definitions of acid and nonacid reflux used for gastroesophageal reflux disease may not be helpful for explaining pathophysiologic mechanisms in the larynx or pharynx. A reflux symptom inde

Acute gastrointestinal bleeding in the intensive care unit. The gastroenterologist's perspective.

Although SRES-associated hemorrhage previously constituted a significant cause of bleeding in the ICU, improvements in ICU management and the institution of prophylactic measures in high-risk patients have significantly reduced SRES-associated hemorrhage since the 1980s. Antacids, H2-receptor antagonists, and sucralfate have been shown to be effective in preventing clinically significant bleeding resulting from SRES, particularly when the intragastric pH is maintained at greater than 4. A selective approach should be adopted in SRES prophylaxis: Patients on mechanical ventilation, with coagulo
